Book excerpt: Transplant medicine and transplantation have progressed during the last decades in a way that nobody would have imagined before. The transplantation of organs offers major therapeutic benefits and improvements to quality of life and is, in many cases, the only life-saving treatment for end-stage organ failure. This new 6th Edition of the Guide collates updated information to provide professionals identifying organ donors, transplant co-ordinators managing the donation process and transplant physicians responsible for organ allocation and utilisation with a useful overview of the most recent advancements in the field. It is intended to provide easy-to-use information and guidance to all professionals involved in donation and transplantation of organs so as to maximise their quality and to minimise risks and, thereby, improve the success rate of transplants. The transplantation of organs offers major therapeutic benefits and improvements to quality of life and is, in many cases, the only life-saving treatment for end-stage organ failure. The most critical factor remains the supply of organs for transplantation, but only organs recovered following strict quality and safety standards are likely to function satisfactorily, and careful evaluation of donors is essential to minimise the risk of transmission of infections or malignancies. Furthermore, since human organs can currently only be derived from the body of a person, strong ethical principles need to be associated with their use. The Council of Europe approaches organ transplantation in compliance with the principles of non-commercialisation and voluntary donation of materials of human origin. This 6th Edition of the Guide to the quality and safety of organs for transplantation contains updated information on organ donation and transplantation to provide professionals identifying organ donors, transplant co-ordinators, managing the donation process and transplant physicians responsible for organ allocation and utilisation with a useful overview of the most recent advancements in the field. This will help them on a practical level by providing easy-to-use information at the bedside.

Book excerpt: The use of blood components represents the only therapy available for many seriously ill patients who suffer from acute or chronic diseases. To provide all those working in the field of transfusion medicine - from blood services to hospital departments to regulators - with a compendium of measures designed to ensure the safety, quality and efficacy of blood components, the Council of Europe has developed a guide as a technical annex to its Recommendation No. R (95) 15 on the preparation, use and quality assurance of blood components. The Guide contains recommendations for blood establishments on blood collection, blood components, technical procedures, transfusion practices and quality systems. It represents the basis for a large number of national regulations, as well as for the blood directives of the European Commission. This is the 19th Edition of the Guide, compiled by leading European experts under the aegis of the European Committee (Partial Agreement) on Blood Transfusion (CD-P-TS). For matters dealing with the use of organs and tissues and cells, see the Council of Europe Guide to the quality and safety of organs for transplantation and Guide to the quality and safety of tissues and cells for human application, respectively.

Available in PDF, EPUB and Kindle. Book excerpt: Addressing all aspects of brain death and thoroughly detailing how a potential organ donor should be maintained to ensure maximum use of the organs and cells, The Brain-Dead Organ Donor: Pathophysiology and Management is a landmark addition to the literature. This first-of-its-kind, multidisciplinary volume will be of interest to a large section of the medical community. The first section of the book reviews the historical, medical, legal, and ethical aspects of brain death. That is followed by two chapters on the pathophysiology of brain death as investigated in small and large animal models. This includes a review of the many hormonal changes, including the neuroendocrine- adrenergic ‘storm’, that takes place during and following the induction of brain death, and how they impact metabolism. The next section of the book reviews various effects of brain death, namely its impact on thyroid function, the inflammatory response that develops, and those relating to innate immunity. The chapters relating to assessment and management of potential organ donors will be of interest to a very large group of transplant surgeons and physicians as well as critical care and neurocritical care physicians and nurses. Neurologists, endocrinologists, neurosurgeons, and pathologists will also be interested, especially in the more basic science sections on various aspects of brain-death and hormonal therapy. Organ procurement organizations and transplant coordinators worldwide will also be interested in this title. Other chapters will be of interest to medical historians, medico-legal experts, and ethicists.

It presents 29 contributions from a highly interdisciplinary group of world experts and upcoming professionals in the field. Every year thousands of patients die while waiting for organ transplantation. Health authorities, medical professionals and bioethicists worldwide point to the urgent and yet unsolved problem of organ shortage, which will be even intensified due to the increasing life expectancy. Even though the practical problem seems to be well known, the search for suitable solutions continues and often restricts itself by being limited through disciplinary and national borders. Combining philosophical reflection with empirical results, this volume enables a unique insight in the ethics of organ transplantation and offers fresh ideas for policymakers, health care professionals, academics and the general public.

Available in PDF, EPUB and Kindle. Book excerpt: This book shows how a successful public health policy designed to foster the culture of brain death diagnosis and improve the interaction between Organ Procurement Organizations and Intra-Hospital Organ and Tissue Donation Committees for Transplants led the Brazilian state of Paraná to reach one of the highest rates of effective organ donors per million population in the world. Brazil has the largest public transplant system in the world, and each of the country’s 26 states is responsible for organizing its own transplant system. The state of Paraná, with 11 million inhabitants, has stood out in this regard in the last decade. It reached 47.7 effective donors per million population (pmp), performing better than countries such as the United States (33.3/pmp), Canada (21.9/pmp), Portugal (33.6/pmp) and Croatia (41.2/pmp). The remodeling of Paraná’s transplant system, between 2011 and 2020, was based on the recognized Spanish model, but has some unique characteristics. This book seeks to describe all the steps of this remodeling, describing its characteristics and explaining how the new system was built. By presenting an in-depth analysis of a local successful case, Effective Public Health Policy in Organ Donation: Lessons from a Universal Public Health System in Brazil seeks to provide useful information to policy makers, health professionals and students from different fields within the health sciences interested in understanding how public health policies can improve organ donation rates, especially in developing countries.
