Book excerpt: Cancer has become a leading cause of death and disability and a serious yet unforeseen challenge to health systems in low-and middle-income countries. A protracted and polarized cancer transition is under way and fuels a concentration of preventable risk, illness, suffering, impoverishment from ill health, and death among poor populations. Closing this cancer divide is an equity imperative. The world faces a huge, unperceived cost of failure to take action that requires an immediate and large-scale global response. Closing the Cancer Divide presents strategies for innovation in delivery, pricing, procurement, finance, knowledge-building, and leadership that can be scaled up by applying a diagonal approach to health system strengthening. The chapters provide evidence-based recommendations for developing programs, local and global policy-making, and prioritizing research. The cases and frameworks provide a guide for developing responses to the challenge of cancer and other chronic illnesses. The book summarizes results of the Global Task Force on Expanding Access to Cancer Care and Control in Developing Countries, a collaboration among leaders from the global health and cancer care communities worldwide, originally convened by Harvard University. It includes contributions from civil society, global and national policy-makers, patients and practitioners, and academics representing an array of fields.

Available in PDF, EPUB and Kindle. Book excerpt: Two leading oncologists, along with experts spanning several medical disciplines, shed light on the global pandemic of cancer, particularly the difference in diagnosis, treatment, and care between global communities. Despite advancing globalization and amazing breakthroughs in modern medicine, developing countries continue to struggle with the prevention and treatment of the most common killer in the world today—cancer. Logistical barriers, scarceness of resources, and economic hardships in these regions make the screening, detection, and care of this disease difficult at best. This book is the only one of its kind to review the pandemic of cancer from a global and epidemiological perspective. The work is presented in three sections, focusing on key issues in cancer management, treatment of specific types of the disease, and the difference in medical care between low-, medium-, and high-resource countries. Chapters address the history, incidence, and treatment across nations; presiding cultural attitudes which may delay or prevent treatment in many parts of the world; and the geopolitics of cancer care and funding. Patients and caregivers from all around the globe explain the daily challenges of living with the disease in their nation.

The book is structured into three main sections with several chapters each, reflecting the authors' individual, real-life experiences. It explores ways to improve oncology education and scientific training, how to set up and run a clinical research facility ethically and efficiently in low- and middle-income settings, addressing the challenges that the workforce encounters in the real world. The main challenges of today’s oncologists seem to be the ever-growing patient care and administrative workload and the risk of burn-out. What are the best strategies to maintain a healthy work-life for the benefit of the patients, the physicians and society, taking into account the different needs, depending on factors like peace, social and gender equality? This book addresses oncologists all over the world and their allies throughout the associated industries to highlight the importance of shared and sustainable education, clinical research and global cancer care.

Book excerpt: A medical emergency forces a brilliant Harvard oncologist to reveal that she has been hiding her advanced breast cancer for a decade. Her husband—also an oncologist—must set aside his anger and feelings of betrayal so that he can care for her during her final year of life. When Jane, a world-famous Harvard oncologist, suddenly collapses at work, the medical team resuscitating her makes a shocking discovery: she has advanced breast cancer that she’s been hiding for years. The results are catastrophic. In Sickness shows how even the most rational people can be nearly destroyed by their irrational fears. Tragic, moving, and wryly funny at times, this is an unflinching portrayal of a complicated marriage and its secrets.

With riveting stories and moving compassion, Scadden and D’Antonio paint a still rapidly changing landscape in the context of all too common stories of loss. Ranging from Scadden’s personal childhood memories to his triumphs and regrets as a doctor, Scadden illuminates a light at the end of a dark tunnel. Through opening a window into the science of medicine in the world of the unknown, Scadden and D’Antonio humanize cancer while inspiring action that we all so desperately need.

Available in PDF, EPUB and Kindle. Book excerpt: In the United States, much of the research is focused on developing new and very expensive technologies and drugs - often without a major therapeutic benefit. In resource limited countries, basic oncology care is frequently lacking. In addition, the benefits of various chemo-radiotherapy combinations for a number of malignancies are unknown as these populations have not been adequately investigated. For oncologists in these countries who have marginal to adequate resources, accrual to clinical trials is virtually non-existent to minimal, due to the complexities of their population and competing co-morbidities. As a result, there is a tremendous disparity in treatment outcomes for these populations, compared to those in developed countries. Therefore, we have asked a number of oncologists from different parts of the world to report their experience. Topics that will be covered include locally advanced breast and cervical cancer (India, South Africa), human resources for cancer control in India, systematic review of radiation resources in low and middle income countries, planning national radiotherapy services, building sustainable partnerships through the newly formed ICEC (International Cancer Export Corps), cancer disparities among American Indians, and training radiation oncologists in these under served parts of the world. Authors will discuss "lessons learned" from their populations, practical suggestions to address these disparities, and how we as a global oncology community can address, and mitigate these global challenges. The editorial by Dr. Coleman and myself highlights the invaluable contributions from our global contributors. Thank you for taking the time to read this special issue on global cancer disparities. We are all energized to begin addressing the needs of our cancer patients worldwide.
