The opening section describes the history of “making a thumb” and covers the fundamentals of anatomy, embryology, and functional dynamics. After careful presentation of the surgical procedures for various developmental anomalies of the thumb, subsequent sections focus on the treatment of bone and joint, tendon, and nerve problems encountered in patients with different diseases and injuries. All aspects of the surgical management of benign and malignant tumors of the thumb are then described. The final section is devoted to current and emerging treatments for trauma, including amputation and microsurgical and non-microsurgical reconstruction. The text is supported by superb clinical photographs as well as high-quality schematic drawings and video clips. Book excerpt: Since the early days of medicine one concern of doctors has been the removal of kidney stones and prevention of recurrence. Owing to the hesitancy of progress in the prevention of initial stone formation and of relapse, however, removal of stones from the kidney and ureter were developed to highly refined techniques and they formerly accounted for a major proportion of the urological operations performed. In the last few years developments in the treatment of kidney stones have taken a completely different turn. In the majority of cases suit able methods are available to bring about spontaneous passage of the stones, while in a smaller proportion drug-induced litholysis is pos sible. Stones that cannot be passed are now treated mainly with extra corporeal shockwave lithotripsy, percutaneous litholapaxy or uretero renoscopy. These methods are often used in combination and comple ment each other. Nonetheless, despite the accumulating experience with the new methods there will still be situations in which stones can not be removed except by open surgery. "Our skill as surgeons and the management of the brilliantly designed equipment would amount to nothing more than highly skilled mechanical work if they did not go hand in hand with enhanced insight into the cause of lithiasis and thus into ways of preventing it - or at least of preventing the relapse that is the lot of most patients.

Book excerpt: As an addition to the European postgraduate training system for young neurosurgeons we began to publish in 1974 this series of Advances and Technical Standards in Neurosurgery which was later sponsored by the European Association of Neurosurgical Societies. This series was first discussed in 1972 at a combined meeting of the Italian and German Neurosurgical Societies in Taormina, the founding fathers of the series being Jean Brihaye, Bernard Pertuiset, Fritz Loew and Hugo Krayenbiihl. Thus were established the principles of European co operation which have been born from the European spirit, flourished in the European Association, and have throughout been associated with this series. The fact that the English language is well on the way to becoming the international medium at European scientific conferences is a great asset in terms of mutual understanding. Therefore we have decided to publish all contributions in English, regardless of the native language of the authors. All contributions are submitted to the entire editorial board before publi cation of any volume. Our series is not intended to compete with the publications of original scientific papers in other neurosurgical journals. Our intention is, rather, to present fields of neurosurgery and related areas in which important recent advances have been made. The contributions are written by specialists in the given fields and constitute the first part of each volume.

Available in PDF, EPUB and Kindle. Book excerpt: Renowned experts present the latest knowledge in ... In this volume of Developments in Ophthalmology, a panel of renowned glaucoma surgeons reports its findings and the updated evidence-based literature data on indications, outcomes, pearls and pitfalls of techniques in which they all have an undisputable 'hands-on' everyday experience. The publication begins with chapters on conventional procedures, i.e. trabeculectomy (still the gold standard), deep sclerectomy, and glaucoma drainage device implantation explicitly focusing on postoperative management. The role of the ocular surface in glaucoma surgery and the issue of wound healing and its modulation through antimetabolites are further discussed. In the second part, the attention shifts to new techniques which still have a marginal role, but are promising for particular indications. In the last part, the greatest challenges of glaucoma surgery such as the surgical approach to angle closure, concomitant glaucoma and cataract as well as pediatric glaucoma are discussed in detail. Finally, the last chapter is dedicated to cyclodestructive techniques. This volume is recommended to glaucoma surgeons and glaucoma specialists seeking information on the best therapy for their patients, but also to general ophthalmologists who refer their patients for surgery or follow them up afterwards.

Available in PDF, EPUB and Kindle. Book excerpt: This book has been designed to provide the full description of the comprehensive management of peritoneal surface malignancies as a new emerging specialty. Combined treatment of cytoreductive surgery (CRS) and hyperthermic intraoperative intraperitoneal chemotherapy (HIPEC) introduced by our leader Paul H. Sugarbaker are performed to treat peritoneal metastases by surgeons all around the world. Therefore this book is focused on detailed surgical anatomy of the peritoneum, preoperative clinical assessment of the peritoneal surface malignancy, patient preparation and operation room equipments, different surgical procedures for CRS and reconstruction, intraoperative hyperthermic intraperitoneal chemotherapy (HIPEC) and neoadjuvant intraperitoneal chemotherapy, early postoperative intraperitoneal chemotherapy (EPIC) and molecular basis of peritoneal surface malignancies. The chapter on molecular mechanisms of the formation of peritoneal carcinomatosis provides insight into a rapidly expanding knowledge within this speciality. This book should be valuable for surgical oncologists who deal with multimodal treatment for peritoneal surface malignancies, as well as for the trained peritonectomy surgeons. For the senior surgeons, it will also introduce new techniques and approaches in this field such as dealing with the omental cakes and massive organ involvement that requires multi-organ resection.

This book was released on 2013-06-16 with total page 418 pages. Available in PDF, EPUB and Kindle. Book excerpt: International comparison of health system performance has become increasingly popular, made possible by the rapidly expanding availability of health data. It has become one of the most important levers for prompting health system reform. Yet, as the demand for transparency and accountability in healthcare increases, so too does the need to compare data from different health systems both accurately and meaningfully. This timely and authoritative book offers an important summary of the current developments in health system performance comparison. It summarises the current state of efforts to compare systems, and identifies and explores the practical and conceptual challenges that occur. It discusses data and methodological challenges, as well as broader issues such as the interface between evidence and practice. The book draws out the priorities for future work on performance comparison, in the development of data sources and measurement instruments, analytic methodology, and assessment of evidence on performance. It concludes by presenting the key lessons and future priorities, and in doing so offers a rich source of material for policy-makers, their analytic advisors, international agencies, academics and students of health systems.
