Intermittent exacerbations are provoked by airway mucosal exposure to pro-inflammatory stimuli, with RNA viral infections or inhaled allergens representing the two most common precipitants. In this setting, inducible signaling pathways the airway mucosa play a central role in the initiation of airway inflammation through production of antimicrobial peptides (defensins), cytokines, chemokines and arachidonic acid metabolites that coordinate the complex processes of vascular permeability, cellular recruitment, mucous hyper-secretion, bronchial constriction and tissue remodeling. These signals also are responsible for leukocytic infiltration into the submucosa, T helper-lymphocyte skewing, and allergic sensitization. Currently, it is well appreciated that asthma is a heterogeneous in terms of onset, exacerbants, severity, and treatment response. Current asthma classification methods are largely descriptive and focus on a single aspect or dimension of the disease. An active area of investigation on how to collect, use and visualize multidimensional profiling in asthma. This book will overview multidimensional profiling strategies and visualization approaches for phenotyping asthma. As an outcome, this work will facilitate the understanding of disease etiology, prognosis and/or therapeutic intervention.

Going beyond single or pairwise gene interactions with a phenotype, PGMs are able to account for complex gene interactions and make predictions of a phenotype. Most PGMs have limited scalability with large genetic datasets. Here, a procedure called phenocentric Bayesian networks that is tailored for the discovery of complex multivariate models for a trait using large genomic datasets is presented. Resulting models can be used to predict outcomes of a phenotype, which allows for meaningful validation and potential applicability in a clinical setting. The utility of phenocentric Bayesian networks is demonstrated with the creation of predictive models for two complex traits related to asthma management: exacerbation and bronchodilator response. The good predictive accuracy of each model is established and shown to be superior to single gene analysis. The results of this work demonstrate the promise of using the phenocentric Bayesian networks to study the genetic architecture of complex traits, and the utility of multigenic predictive methods compared to traditional single-gene approaches.

Book excerpt: Genome-wide association studies (GWAS) of asthma have been successful in identifying novel asthma-associated loci, but the genes at these loci account only for a part of the whole genetic component. One limitation of GWAS is that they rest on single-marker analyses which are underpowered to detect variants with small marginal effects but rather influence jointly disease risk. To complement the single-marker approaches, more sophisticated strategies, which integrate biological knowledge, such as protein-protein interactions (PPI) or gene networks with GWAS outcomes to identify disease-associated gene modules, have become prominent. The objectives of this thesis were to develop network-based analysis methods, and apply them to asthma GWAS data to identify biological processes and prioritize new candidate genes related to asthma.This thesis consists of two main studies. The first study was to extend an existing network-based method (dmGWAS) to identify novel genes associated with asthma. We used two GWAS datasets, each consisting of the results of a meta-analysis of nine childhood-onset asthma GWAS (5,924 and 6,043 subjects, called META1 and META2, respectively). We developed a novel method to compute gene-level p-values from SNP p-values (fastCGP), and proposed a bi-directional module search method to identify asthma-associated gene modules. Book excerpt: Motivated by the explosion of molecular data on humans-particularly data associated with individual patients-and the sense that there are large, as-yet-untapped opportunities to use this data to improve health outcomes, Toward Precision Medicine explores the feasibility and need for "a new taxonomy of human disease based on molecular biology" and develops a potential framework for creating one. The book says that a new data network that integrates emerging research on the molecular makeup of diseases with clinical data on individual patients could drive the development of a more accurate classification of diseases and ultimately enhance diagnosis and treatment. The "new taxonomy" that emerges would define diseases by their underlying molecular causes and other factors in addition to their traditional physical signs and symptoms. The book adds that the new data network could also improve biomedical research by enabling scientists to access patients' information during treatment while still protecting their rights. This would allow the marriage of molecular research and clinical data at the point of care, as opposed to research information continuing to reside primarily in academia. Toward Precision Medicine notes that moving toward individualized medicine requires that researchers and health care providers have access to very large sets of health- and disease-related data linked to individual patients. These data are also critical for developing the information commons, the knowledge network of disease, and ultimately the new taxonomy.
