This report examines how Ethiopia s Health Extension Program (HEP) has contributed to the country s move toward Univeral Health Coverage (UHC), and to shed light on how other countries may learn from Ethiopia s experiences of HEP when designing their own path to UHC. HEP is one of the government s UHC strategies introduced in a context of limited resources and low coverage of essential health services. The key aspects of the program include the capacity building and mobilization of more than 30, 000 Health Extension Workers (HEWs) targeting more than 12 million model families, and the mobilization of health development army ? to support the community-based health system. Using the HEP-UHC conceptual model and data from Demographic and Health Surveys, the study examines how the HEP has contributed to the country s move toward UHC. During the period that the HEP has been implemented, the country has experienced significant improvements in many dimensions: in terms of socioeconomic, psychological, behavioral, and biological dimensions of the beneficiaries; and in terms of the coverage of health care services. The study finds an accelerated rate of improvements among the rural, less-educated, and the poor population, which is leading to an overall reduction in equity gaps and improvements in the equity indicators including the concentration indices - that suggest a more equitable distribution of resources and health outcomes. The HEP in Ethiopia has demonstrated that an institutionalized community approach is effective in helping a country make progress toward UHC. The elements of success in the HEP include the emphasis on community mobilization which identifies community priorities, engages and empowers community members, and supports their ability to solve local problems. The other aspect of HEP is the emphasis on institutionalization of the activities, which addresses the sustainability of community programs through high level of political commitment, and effective coordination of national policies and leveraging of support from partners. These findings may offer useful lessons for other low income countries facing similar challenges in developing and implementing a sustainable UHC strategy.

It reviews the global progress being made towards achieving the Millennium Development Goal (MDG) target on sanitation. A literature review is presented on sanitation provision including human health impacts and the estimated costs and benefits of achieving the MDG target. The report also provides a critique in that the UN has not yet introduced the concept of sustainability into the MDG programme in general and in particular into the sanitation sector which is highly dysfunctional and suffering from limited political leadership at both the local and global levels. It introduces the various sustainable sanitation options available and what approaches can be taken to improve sanitation systems – not just toilets which are only a small part of the overall system of food, nutrients and water cycles. The study estimates the numbers of urban and rural households, including slum populations that are being targeted in all world regions. It also evaluates the historic trends in morbidity and mortality linked to diarrhoea arising from lack of functioning sanitation services comparing these to the UN data on sanitation coverage. The report estimates the potential fertiliser replacement capacity that reuse of human excreta can have for all world regions. Finally it provides a vision for future development within the sector where more sustainable options like source separation and reuse are promoted giving positive environmental or “green” impacts but also catalysing greater involvement and understanding on the part of individuals in society.

Book excerpt: This literature review explores how political, economic and resource management policies and programs can reduce forest degradation and increase the contribution of forest goods and services to sustainable livelihood strategies. In Ethiopia, studies indicate that forest dependency is strong throughout the country, but the importance of forest income varies across different regions and wealth categories. Research suggests that improving forest product market governance is key to strengthening forest livelihood resiliency. Recent experiments with forest governance devolution have shown mixed results in terms of improving forest conditions and livelihoods. Smallholder land certification has met with considerable success, whereas participatory forest management schemes have positive ecological outcomes but fall short in terms of livelihood gains. In South Sudan, civil war has limited the depth and scope of research on dryland forests and livelihoods. Food security analyses indicate that the importance of forest income varies by region and season. Markets are poorly developed and forest governance systems are weak in many parts of the country. Key threats to forest livelihoods in both countries include: shifting climatic conditions, large-scale population movements, large-scale land acquisitions and weakened governance institutions; and in South Sudan, continuing violent conflict. In Ethiopia, research and policy reform should focus on the relationship between forest rights devolution, livelihoods, forest management practices and forest conditions as well as on the impacts of demographic change on forest-based livelihoods, forest management and forest cover. In South Sudan, research should focus on documenting the impacts of conflict on forest-based livelihoods with an view to structuring humanitarian aid programs in ways that mitigate the negative impacts.
