In Chicago in December of 1903, a 34-year-old physician, Elizabeth Hopkins Dunn, working as a research assistant in neurology, initiated a series of experiments to examine the ability of neonatal rat cerebral tissue to survive transplantation into the brain of matched littermates. Out of 46 attempts, four clearly successful grafts were identified. The publication of Dunn's results in 1917, the first credible report to demonstrate the feasibility of mammalian CNS transplants, generated little interest. In fact, the next significant experiment in this field did not appear until 1930. The field continued to grow slowly and quietly as investigators gradually realized the value of neural trans plantation to study problems of development and plasticity in the mammalian nervous system. With the discovery in 1979 that grafted neurons were capable of appropriate and functional interactions with the host brain, interest in neural transplantation esca lated sharply. The extraordinary opportunities created by using functional neural trans plants in investigating basic issues in neurobiology, as well as the clinical implications, excited both scientists and the public. The popularity of neural transplantation has been growing rapidly in the past five years and shows no signs of abating. The present volume was designed to meet two needs created by the rapid growth of this subdiscipline of neurobiology. The first was to provide a thorough review of the experimental foundations of neural transplantation.

In particular, the grafted cells can sustain functional recovery in animal models of a range of neurodegenerative conditions including genetic and idiopathic neurodegenerative diseases of adulthood and aging, ischemic stroke, and brain and spinal cord trauma. In a restricted subset of such conditions, cell transplantation has progressed to application in humans in early-stage clinical trials. At the present stage of play, there is clear evidence of clinical efficacy of fetal cell transplants in Parkinson disease (notwithstanding a range of technical difficulties still to be fully resolved), and preliminary claims of promising outcomes in several other severe neurodegenerative conditions, including Huntington disease and stroke. Moreover, the experimental literature is increasingly suggesting that the experience and training of the graft recipient materially affects the functional outcome. For example, environmental enrichment, behavioral activity, and specific training can enhance the recovery process to maximize functional recovery. There are even circumstances where the grafted cells have been demonstrated to restore the neural substrate for new learning. Consequently, it is not sufficient to replace lost cells anatomically; rather, for the grafts to be effective, they need to be integrated functionally into the host circuitry, and the host animal requires training and rehabilitation to maximize function of the reconstructed graft−host circuitry. Such observations require reconsideration of the design of the next generation of clinical trials and subsequent service delivery, to include physiotherapists, cognitive therapists, and rehabilitation experts as core members of the transplant team, along with the neurologists and neurosurgeons that have conventionally led the field.

In order to address these recent developments as we enter the 21st century, 10 years after conception and 6 years after the publication of Functional Neural Transplantation, the Editors have now sought to co-ordinate and edit an updated version, Functional Neural Transplantation II. Functional Neural Transplantation II is not just a revised edition of the previous volume, but an entirely new and complementary second volume to update the field to 'state of the art' for the new millennium. For many major topic areas (such as functional repair in neurodegenerative disorders of the basal ganglia) there has been a continual flow of significant advances. Although these topics are covered in both volumes, the Editors have sought to avoid duplication by requiring that the present coverage emphasises the major developments over the last decade while relying on the first volume for the background overviews. Other topics though are entirely new in the present volume, such as novel applications of neural transplantation to other major clinical relevance such as retinal repair or alleviation of pain. A key departure of the present volume is the addition of several early chapters addressing the technical and developmental issues associated with identification of alternative sources of cells for transplantation - cell lines, immortalised cells, precursor/stem cells and xenotransplantation.
