Book excerpt: In the past decade, the Russian Federation has experienced many seemingly unfavorable demographic trends, the two most significant of which are a declining number of births and a rising number of deaths. These trends are likely to continue for some time. Some analysts fear that the Russian population, currently at about 145 million, could decline to less than 100 million. This demographic decline raises several issues for Russia, including the need for health care improvements; the challenges posed by a declining working-age population to support a growing elderly population; and still other issues affecting Russia's ability to reform its economy, government, and society. This report examines trends in overall population size, fertility rates, and mortality rates and issues in health care, elderly support, and national security arising from these trends. Since 1992, the population of Russia has declined by three million. The annual number of Russian births fell by 1.3 million between 1987 and 1999, while the annual number of Russian deaths increased by 500,000. Net immigration has prevented Russian population losses from being even greater, with many ethnic Russians migrating to Russia from borderlands formerly in the Soviet Union. The most recent statistics, however, indicate that this ethnic Russian immigration is declining and, as a result, it is unlikely to be an important source of population stabilization in the future. There is also public resistance to immigration and concerns about the security risks created by immigration of nonethnic Russians. If Russian immigration cannot be increased, then the only other alternatives for population stability are to increase birth rates or to reduce death rates.

With aging, the disease burden shifts toward noncommunicable diseases (NCDs) which can threaten healthy aging. However, securing the gains expected from the demographic dividend-where developing countries' working and nondependent population increases and per capita income thus rises- is both achievable and affordable through efficiently tacking NCDs with prevention and control efforts. This book looks primarily at cardiovascular disease (CVD) and tobacco use since they account for a disproportionate amount of the NCD burden-the focus is strategic, rather than comprehensive. The goal of this book is to encourage countries to develop, adopt, and implement effective and timely country and, where appropriate, regional responses that reduce both population-level risk factors and the NCD burden. The work develops (i) an NCD burden and risk factor profile for all countries and the region as a whole; (ii) a rationale for public policy and action for NCDs; (iii) a framework to guide the formulation of public policies and strategies for NCDs; (iv) a country profile, including capacity and ongoing NCD activities, as well as policy options and actions for NCDs that will help stimulate policy dialogue within and among countries; and (v) a regional strategy for NCD prevention and control where regional collaboration offers added value. Despite much interest in the consequent growth dividend, the size and circumstances of the potential gains remain under-explored. This study makes progress by focusing on India, which will be the largest individual contributor to the global demographic transition ahead. It exploits the variation in the age structure of the population across Indian states to identify the demographic dividend. The main finding is that there is a large and significant growth impact of both the level and growth rate of the working age ratio. This result is robust to a variety of empirical strategies, including a correction for inter-state migration. The results imply that a substantial fraction of the growth acceleration that India has experienced since the 1980s - sometimes ascribed exclusively to economic reforms - is attributable to changes in the country’s age structure. Moreover, the demographic dividend could add about 2 percentage points per annum to India’s per capita GDP growth over the next two decades. With the future expansion of the working age ratio concentrated in some of India’s poorest states, income convergence may well speed up, a theme likely to recur on the global stage.
