Book excerpt: L'ostéosynthèse des fractures fraiches du radius distal (FRD) par plaque antérieure verrouillée est présentée dans la littérature comme une avancée majeure et récente. Nous l'utilisons largement dans notre unité. Quelques articles récents font état de complications fréquentes qui pourraient modifier cette tendance. Nous avons voulu savoir quel était dans notre unité la fréquence des complications liées au traitement par plaque antérieure verrouillée des FRD fraiches. Tous les dossiers cliniques et radiologiques des fractures fraiches unilatérales du radius distal (FRD) traitées consécutivement dans notre unité spécialisée en 2012 et 2013 ont été revus par un observateur indépendant, soit 365 poignets chez 365 patients. Ont été exclus 85 cas (21 traitements orthopédiques, 35 autres traitements chirurgicaux, 27, perdus de vue et 2 patients décédés). Les FRD traitées par plaque verrouillée antérieure représentaient 280 poignets. L'âge moyen des 280 patients était de 57 ans (16-94 ans). Il y avait 192 femmes (69%). L'âge moyen était plus jeune chez les hommes (45 ans) que chez les femmes (62 ans). Les patients en bonne santé ayant des besoins fonctionnels maximums (3-3 de la classification PAF) étaient 55% (155 des 280 dont 86 femmes). Les patients ayant des comorbidités et des besoins fonctionnels intermédiaires (2-2 de la classification PAF) étaient 26% (74 des 280, dont 86% de femmes). Les patients dépendants ayant des besoins fonctionnels minimums (1-1 de la classification PAF) étaient 5% (14 des 280, uniquement des femmes).Chez ces 280 patients opérés par plaque antérieure pour fracture du radius distal pendant une période continue de 2 ans, une liste exhaustive de toutes les complications possibles au sens large (ostéoarticulaires, tendineuses, nerveuses, vasculaires, algodystrophie) a été établie. Npus avons voulu mettre en évidence chez ces 280 patients les complications spécifiques liées au traitement utilisé. Le recul moyen des 280 dossiers à la dernière revue était de 16 mois (minimum 3 mois, maximum 37 mois). Nous avons recensé des complications qui n'étaient pas directement liées à la méthode thérapeutique utilisée:24 algodystrophies post-opératoires (8%) 9 syndromes carpiens (3%) dont 3 représentaient la décompensation d'un syndrome carpien pré-existant. Parmi les complications directement en rapport avec le traitement par plaque antérieure, nous avons recensé : 11 anomalies des vis (1 recul de vis spontané, 1 vis trop longue avec rupture tendon extensor pollicis longus, 9 vis intraarticulaires dont 1 intra-RUD). Ces anomalies ont donné lieu à 10 ré-interventions.13 plaques trop saillantes ayant provoqué une rupture du tendon fléchisseur du pouce et 2 tendinites des tendons fléchisseurs. Quatre ré-interventions ont été réalisées à cause de ces plaques trop saillantes.3 déplacements secondaires extra-articulaires. Au total, Nous avons eu à déplorer dans cette série 27 complications directement liées à la technique chirurgicale utilisée (9%), dont 15 (5%) ont nécessité une ré-intervention. Cette analyse d'une série mono-centrique prospective de fractures fraiches du radius distal sur une période de 2 ans, révèle un taux de complications directement lié au traitement par plaque antérieure verrouillée comparable à celui de la littérature, c'est-à-dire environ 10%. Toutefois, le nombre de complications sérieuses ayant conduit à une ré-intervention s'établit à 5%. Ceci nous semble devoir modérer les taux de complications parfois très importants que l'on retrouve dans certaines séries. En particulier, nous n'avons pas retrouvé un taux important de rupture des tendons, ni du côté des extenseurs, ni du côté des fléchisseurs. Cette série nous conforte dans l'utilisation large des plaques verrouillées antérieures pour traiter les fractures fraiches du radius distal. Nous pensons pouvoir minimiser les complications observées en faisant de meilleurs choix des formes de plaques en fonction de la fracture ainsi qu'en utilisant plus largement le contrôle arthroscopique

Where did they come from? How did they organize technology, especially in the context of settlement behavior? During the Pleistocene era, the people now known as Beringians dispersed across the varied landscapes of late-glacial northeast Asia and northwest North America. The twenty chapters gathered in this volume explore, in addition to the questions posed above, how Beringians adapted in response to climate and environmental changes.
