Peu de données existent sur la formation des internes de médecine générale (IMG) dans ce domaine. Cette étude cherche à évaluer les formations théorique et pratique à la psychiatrie au cours de l'internat de médecine générale en France. Un questionnaire a été diffusé aux représentants locaux des IMG des 35 départements de médecine générale (DMG). Les participants étaient sondés sur : 1) l'existence d'une formation théorique à la psychiatrie, son volume horaire, le contenu et les modalités d'enseignement ; 2) l'existence de stages en psychiatrie, leur nombre et leur type. Un enseignement théorique est proposé dans 29 DMG (83 °/o). Son volume horaire moyen est de 10,6 heures sur les trois années d'internat. Les thématiques les plus abordées sont : addiction et dépendance (77 °/o des 35 DMG), souffrance psychique ( 49 °/o), trouble dépressif caractérisé et troubles anxieux ( 46 °/o). Les médecins généralistes enseignants sont les intervenants principaux (80 °/o). La totalité des DMG propose au moins un stage de psychiatrie, surtout en milieu intra-hospitalier (94 °/o). La formation à la psychiatrie des IMG, hétérogène, gagnerait à être modernisée en proposant de nouveaux outils pédagogiques, des enseignements par des spécialistes psychiatres et davantage 'de stages ambulatoires.

L'objectif de cette étude était de recueillir le ressenti des internes de médecine générale de Grenoble lors de la prise en charge d'un patient présentant un trouble psychique et de mettre en évidence leurs attentes concernant la formation pratique et théorique de la psychiatrie en soins de santé primaires. Méthodes - Nous avons réalisé une étude qualitative en interrogeant dix internes de médecine générale. Les données recueillies ont été organisées avec le logiciel Nvivo 12 puis analysées en utilisant la méthode de l'analyse thématique de discours. Résultats - Huit thèmes principaux sont retrouvés : la complexité de la psychiatrie, une vision dichotomique des troubles psychiques, des ressentis variés, un rôle central du médecin généraliste, l'importance de la collaboration entre professionnels, un apprentissage essentiellement pratique, une formation théorique manquante et des suggestions d'amélioration nombreuses. Ils sont répartis à l'intérieur de deux axes thématiques : les expériences vécues et le vécu des acquisitions de compétences en psychiatrie. Conclusion - Le développement d'offres de stages de psychiatrie en ambulatoire et des enseignements spécifiques menés par des professionnels de la santé mentale en collaboration avec ceux de médecine générale pourraient améliorer la prise en charge des patients présentant un trouble psychique.

In Inheriting Madness, Ian Dowbiggin traces the rise in popularity of hereditarianism in France during the second half of the nineteenth century to illuminate the nature and evolution of psychiatry during this period. In Dowbiggin's mind, this fondness for hereditarianism stemmed from the need to reconcile two counteracting factors. On the one hand, psychiatrists were attempting to expand their power and privileges by excluding other groups from the treatment of the mentally ill. On the other hand, medicine's failure to effectively diagnose, cure, and understand the causes of madness made it extremely difficult for psychiatrists to justify such an expansion. These two factors, Dowbiggin argues, shaped the way psychiatrists thought about insanity, encouraging them to adopt hereditarian ideas, such as the degeneracy theory, to explain why psychiatry had failed to meet expectations. Hereditarian theories, in turn, provided evidence of the need for psychiatrists to assume more authority, resources, and cultural influence. Inheriting Madness is a forceful reminder that psychiatric notions are deeply rooted in the social, political, and cultural history of the profession itself. At a time when genetic interpretations of mental disease are again in vogue, Dowbiggin demonstrates that these views are far from unprecedented, and that in fact they share remarkable similarities with earlier theories. A familiarity with the history of the psychiatric profession compels the author to ask whether or not public faith in it is warranted.

People want to know what goes on in families that give rise to amazing leaders and/or terrorists. For the first time in history we can understand the systems in which we live. The social sciences have been accumulating knowledge since the early fifties as to how we are regulated by others. S. Milgram, S. Ashe, P. Zimbardo and J. Calhoun, detail the vulnerability to being duped and deceived and the difficulty of cooperating when values differ. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.

These two closely related perspectives, one philosophical and the other sociological, place human action at the center of their explanatory schemes. It has not mattered what aspect of social or psychological behavior was under scrutiny. Whether selves, minds, or emotions, or institutions, social structures, or social change, all have been conceptualized as forms of human activity. This view is the simple genius of these perspectives. Anselm Strauss always took ideas pertaining to action and process seriously. Here he makes explicit the theory of action that implicitly guided his research for roughly forty years. It is understood that Strauss accepts the proposition that acting (or even better, interacting) causes social structure. He lays the basis for this idea in the nineteen assumptions he articulates early in the book--assumptions that elaborate and make clearer Herbert Blumer's famous premises of symbolic interactionism. The task Strauss put before himself is how to keep the complexity of human group life in front of the researcher/theorist and simultaneously articulate an analytical scheme that clarifies and reveals that complexity. With these two imperfectly related issues before him, Strauss outlines an analytical scheme of society in action. It is a scheme that rests not on logical necessity but on research and observation, and the concepts he uses are proposed because they do a certain amount of analytical work. One would be well advised to take Continual Permutations of Action very seriously.

Le Médecin Généraliste est le médecin de premier recours dans la population générale et sa formation en psychiatrie parait insuffisante. Lors du Diplôme d'Etude Spécialisé de médecine générale, les stages obligatoires bénéficient d'une liste d'objectifs régie par le département de médecine générale (DMG). Afin de promouvoir la formation de l'Interne de Médecine Générale (IMG) en psychiatrie et d'améliorer sa pratique future, ce travail avait pour objet d'établir une liste de compétences à acquérir lors de la réalisation du stage en psychiatrie ; stage libre, à Lille. Méthode : Qualitative de consensus, par groupe nominal, réalisé à Lille, et regroupant des experts en psychiatrie. Les chefs de service de psychiatrie recevant des IMG ainsi que les psychiatres participant aux réunions de la Commission Médicale d'Etablissement (CME) ont été invités. Résultats : 39 chefs de service de psychiatrie et 30 psychiatres présents à la CME de psychiatrie ont été invités par mail. 12 réponses sont parvenues favorables. Le groupe nominal a finalement regroupé 10 experts en psychiatrie. Un travail initial de réflexion individuel a permis de formuler 92 items. Une harmonisation encadrée par un modérateur a abouti à une liste de 45 items. La hiérarchisation a été organisée par vote des experts attribuant une note de 1 à 10 par ordre de priorité aux 10 items leurs semblant les plus importants. Cette étape s'est déroulée par échange de mails aboutissant à une liste de 12 objectifs notés de 72 à 17. Conclusion : Cette étude conclut à une liste de 12 compétences que l'IMG doit acquérir lors de la réalisation de son stage en psychiatrie. A charge du DMG et des enseignants d'en définir la place au sein de la formation.
