Book excerpt: This completely revised and updated edition offers a comprehensive overview of neonatal pain assessment and treatment. It includes the field of fetal surgery, and many other topics have been updated or added, such as circumcision analgesia, new drugs, new insights into neurophysiologic pathways of neonatal pain and new drawbacks of analgesic drugs. While in the early years of the 21st century pain treatment in neonates was still optional, it is now a tenet, and more and more institutions are looking for inspiration and good references to create their own guidelines. Written by leading researchers in the field, this book provides that inspiration and offers a valuable tool for neonatologists, anesthetists, nurses and physiotherapists. Since it also deals with prenatal and postnatal surgery, it also appeals to surgeons.

Book excerpt: Expanding on the National Research Council's Guide for the Care and Use of Laboratory Animals, this book deals specifically with mammals in neuroscience and behavioral research laboratories. It offers flexible guidelines for the care of these animals, and guidance on adapting these guidelines to various situations without hindering the research process. Guidelines for the Care and Use of Mammals in Neuroscience and Behavioral Research offers a more in-depth treatment of concerns specific to these disciplines than any previous guide on animal care and use. It treats on such important subjects as: The important role that the researcher and veterinarian play in developing animal protocols. Methods for assessing and ensuring an animal's well-being. General animal-care elements as they apply to neuroscience and behavioral research, and common animal welfare challenges this research can pose. The use of professional judgment and careful interpretation of regulations and guidelines to develop performance standards ensuring animal well-being and high-quality research. Guidelines for the Care and Use of Mammals in Neuroscience and Behavioral Research treats the development and evaluation of animal-use protocols as a decision-making process, not just a decision. To this end, it presents the most current, in-depth information about the best practices for animal care and use, as they pertain to the intricacies of neuroscience and behavioral research.

Book excerpt: Pain touches sensitive nerves in American liberalism, conservatism, and political life. In this history of American political culture, Keith Wailoo examines how pain has defined the line between liberals and conservatives from just after World War II to the present. From disabling pain to end-of-life pain to fetal pain, the battle over whose pain is real and who deserves relief has created stark ideological divisions at the bedside, in politics, and in the courts. Beginning with the return of soldiers after World War II and fierce medical and political disagreements about whether pain constitutes a true disability, Wailoo explores the 1960s rise of an expansive liberal pain standard along with the emerging conviction that subjective pain was real, disabling, and compensable. These concepts were attacked during the Reagan era, when a conservative backlash led to diminished disability aid and an expanding role of courts as arbiters in the politicized struggle to define pain. New fronts in pain politics opened nationwide as advocates for death with dignity insisted that end-of-life pain warranted full relief, while the religious right mobilized around fetal pain. The book ends with the 2003 OxyContin arrest of conservative talk show host Rush Limbaugh, a cautionary tale about deregulation and the widening gaps between the overmedicated and the undertreated.

Until as late as the 1980s it was common to operate on newborns without using systemic opioids. Today things have changed and only a few bioethicists argue that pain is not completely felt in early life.

Available in PDF, EPUB and Kindle. Book excerpt: During the past several decades, the fetus has been diversely represented in political debates, medical textbooks and journals, personal memoirs and autobiographies, museum exhibits and mass media, and civil and criminal law. Ourselves Unborn argues that the meanings people attribute to the fetus are not based simply on biological fact or theological truth, but are in fact strongly influenced by competing definitions of personhood and identity, beliefs about knowledge and authority, and assumptions about gender roles and sexuality. In addition, these meanings can be shaped by dramatic historical change: over the course of the twentieth century, medical and technological changes made fetal development more comprehensible, while political and social changes made the fetus a subject of public controversy. Moreover, since the late nineteenth century, questions about how fetal life develops and should be valued have frequently intersected with debates about the authority of science and religion, and the relationship between the individual and society. In examining the contested history of fetal meanings, Sara Dubow brings a fresh perspective to these vital debates.

Available in PDF, EPUB and Kindle. Book excerpt: In the later stages of gestation, fetal functions undergo increasing change and development, preparing the fetus for the transition to its postnatal environment. Rapid maturation is witnessed in breathing, swallowing, sensory functions, sleep, and many other processes, with corresponding behavioral changes. By 35 to 40 weeks of gestation, fetuses are capable of living ex utero without support, but it is increasingly appreciated that even infants born at between 35 and 36 weeks can suffer long-term consequences. This book, which complements the author’s previous volume on development of normal fetal movements during the first 25 weeks of gestation, discusses in detail the full range of behavioral phenomena observed during the final 15 weeks of gestation, with careful analysis of their mutual relationships. A key feature is the outstanding photographic material, difficult to obtain at this late stage, and the instructive graphs that are also included. The information provided will alert clinicians to deviations from the norm and to physiologic phenomena that can turn pathologic in infants born prematurely.

Book excerpt: Despite today's historically low maternal and infant mortality rates in the United States, labor continues to evoke fear among American women. Rather than embrace the natural childbirth methods promoted in the 1970s, most women welcome epidural anesthesia and even Cesarean deliveries. In Deliver Me from Pain, Jacqueline H. Wolf asks how a treatment such as obstetric anesthesia, even when it historically posed serious risk to mothers and newborns, paradoxically came to assuage women's anxiety about birth. Each chapter begins with the story of a birth, dramatically illustrating the unique practices of the era being examined. Deliver Me from Pain covers the development and use of anesthesia from ether and chloroform in the mid-nineteenth century; to amnesiacs, barbiturates, narcotics, opioids, tranquilizers, saddle blocks, spinals, and gas during the mid-twentieth century; to epidural anesthesia today. Labor pain is not merely a physiological response, but a phenomenon that mothers and physicians perceive through a historical, social, and cultural lens. Wolf examines these influences and argues that medical and lay views of labor pain and the concomitant acceptance of obstetric anesthesia have had a ripple effect, creating the conditions for acceptance of other, often unnecessary, and sometimes risky obstetric treatments: forceps, the chemical induction and augmentation of labor, episiotomy, electronic fetal monitoring, and Cesarean section. As American women make decisions about anesthesia today, Deliver Me from Pain offers them insight into how women made this choice in the past and why each generation of mothers has made dramatically different decisions.
