Cette situation entraîne des risques de grossesse pathologique (prématurité, RCIU, risque de souffrance psychologique ou psychiatrique de la mère puis de l'enfant ...) ce qui doit en tant que soignant, nous alerter pour prendre en charge ces femmes de façon adaptée. Sur Toulouse, notre étude de plusieurs cas souligne que cette prise en charge peut encore être améliorée sur plusieurs points (dépistage, prise en charge psychologique, coordination entre les différents intervenants...). Pour cela, la mise en place d'un réseau de soins avec des acteurs pluridisciplinaires (médicaux, paramédicaux, autres...) semblait adaptée. Or, un réseau de soins concernant les personnes en précarité existe depuis 2000 sur Toulouse. Nous démontrons donc qu'il serait plus judicieux d'utiliser pleinement ce réseau pour le cas particulier de ces femmes enceintes sans domicile fixe. Par ailleurs, en intra hospitalier, elles pourraient être orientées vers un service de consultation plus adapté : pluridisciplinaire.

Ce temps de rencontre, matérialisé sous forme de réunion de concertation pluridisciplinaire (RCP) est organisé dans le but d'élaborer un parcours personnalisé de soins, centré et adapté aux besoins des femmes enceintes précaires/vulnérables. Ainsi est née la RCP « Périnatalité - Grande vulnérabilité » du réseau Périnat Aquitaine (RPA). Après quelques années de recul, il nous est apparu nécessaire de réaliser un « état des lieux « et d'évaluer l'intérêt d'une telle approche pluridisciplinaire sur le parcours de soins de ces femmes et de leurs enfants. Objectif : décrire le parcours de soins et le profil médico-psycho-social des femmes 1 à 3 ans après étude de leur situation en RCP et évaluer la place de ce passage en RCP dans leur trajectoire de vie. Matériels et méthodes : étude observationnelle descriptive transversale à partir des femmes dont les dossiers ont été étudiés en RCP de novembre 2014 à juin 2016. Résultats : concernant les motifs d'orientation en RCP, moins de la moitié des femmes (7 femmes, soit 44%) a été orientée pour des raisons d'accès aux soins difficiles ou des difficultés observées en matière de continuité des soins. Sur le plan social, la majorité des femmes incluses était en situation régulière, avait une couverture sociale et un logement fixe et stable. En revanche, peu d'entre elles ont un entourage social disponible. Sur le plan médical, toutes les femmes déclarent être en assez bonne, bonne voire très bonne santé ; 13 d'entre elles (81%) ont un Médecin généraliste traitant qui est le principal acteur du suivi médical (n=13 ; 81%) ; une seule femme a renoncé à des soins pour des raisons financières et la plupart n'ont pas eu besoin de recourir à un service d'Urgences depuis leur inclusion. Sur les plans psychologique/psychiatrique, on constate de lourds antécédents puisqu'on ne peut a priori exclure un EDM que chez 4 femmes (25%) et un ESPT que chez 6 d'entre elles (38%). En revanche, concernant les toxiques, leur consommation est inexistante ; de même, les consommations d'alcool et de tabac sont pour ainsi dire aussi inexistantes. Conclusion : la nécessité de travailler en réseau est cruciale. C'est ce que nous avons souhaité mettre en avant à travers ce travail. Il faut impérativement poursuivre des recherches visant l'harmonisation des réseaux de santé en périnatalité et la formalisation de staff parentalité ou staff médico-psycho-sociaux dans la prise en charge des femmes enceintes en situation de vulnérabilité.

It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? In order to combat the uprisings, the French civilian and military authorities reorganised the entire territory of the country, swiftly erected new infrastructures and pursued building policies that were ultimately intended to stabilize French dominance in Algeria.The study describes the architectural responses undertaken in the midst of this protracted and bloody armed conflict. It analyses their origins, evolutions and objectives, identifies the actors involved and reveals the underlying design methods.
