Book excerpt: Inflammatory bowel disease (IBD) comprises two idiopathic chronic relapsing and remitting inflammatory disorders of the gastrointestinal tract: ulcerative colitis and Crohn’s disease. Understanding of the etiopathogenesis of IBD, and how best to treat it, continues to advance rapidly and has led to the need for this new edition. Chapter 1 has been substantially revised, emphasizing recent data that indicate how the gut microbiome and its human host may interact to cause IBD in genetically susceptible people. In later chapters, the latest thinking on how best to use immunomodulatory and biological drugs is discussed, along with new sections on recently introduced and imminent therapies. This book is aimed at non-specialist doctors (particularly primary care providers and hospital doctors in training), nurses, stoma therapists, dieticians, psychologists, counselors, social workers and other professionals involved in the care of patients with IBD. Contents: • Etiopathogenesis • Clinical features and intestinal complications • Extraintestinal manifestations and complications • Diagnosis • Drugs used to treat IBD • Principles of management • Medical management of ulcerative colitis • Medical management of Crohn’s disease • Surgery • IBD in pregnancy, childhood and old age • Prognosis

Available in PDF, EPUB and Kindle. Book excerpt: Inflammatory bowel disease (IBD), which includes ulcerative colitis and Crohn's disease, affects 1 person in every 400 in the UK. Between 6,000 and 12,000 new cases of ulcerative colitis are diagnosed every year, and research shows that the number of people with Crohn's disease has been rising steadily. While there are numerous volumes covering the management of IBD aimed at clinicians and several volumes focusing on either ulcerative colitis or Crohn's disease for patients, there are few volumes covering the full spectrum of IBD for patients, families and carers. This patient-centred guide covers all aspects of IBD including common symptoms, tests used to make a diagnosis, treatment and overall management of the condition. Providing up-to-date factual information, it is easy to read and allows quick access to key facts and helpful advice. With consideration given to new developments in the understanding and treatment of inflammation of the bowel, including complementary and alternative therapies, this book will help to improve the overall quality of life for all those affected by this chronic disease.

Available in PDF, EPUB and Kindle. Book excerpt: Inflammatory bowel disease (IBD) encompasses a spectrum of autoimmmune diseases which include ulcerative colitis, Crohn’s disease, and indeterminate colitis. Ulcerative colitis is a relapsing nontransmural inflammatory disease restricted to colon, which may be classified as proctitis, left-sided colitis, or pancolitis depending on the extent of involvement. Some patients also develop ileal inflammation (backwash ileitis), which occasionally complicates its differentiation from Crohn’s ileocolitis. Crohn’s disease is defined as a relapsing transmural inflammatory disease of gastrointestinal mucosa that may affect the entire gastrointestinal tract (GIT) from the mouth to the anus. There is discontinuous involvement of various portions of the GIT and may get complicated by formation of strictures, abscesses, or fistulas. It closely resembles intestinal tuberculosis both clinically and on investigations. GI tuberculosis is often suspected on the basis of its clinical, radiological, and endoscopic features, but histological or microbiologic proof of the disease is often difficult to achieve. Therefore, the majority of patients are treated with antitubercular drugs on presumptive diagnosis only. Similar clinical, endoscopic, radiological, and histological features of GI tuberculosis and Crohn’s disease pose a serious challenge to the treating physician. The rate of misdiagnosis of these conditions using conventional diagnostic measures has been reported to be around 50–70%. Thus differentiation and diagnosis of these conditions is difficult but very important for correct treatment of the patients. Recently some serological tests have come up for diagnosing Crohn’s disease and ulcerative colitis, which may help to distinguish these disorders from each other and from GI tuberculosis. Besides diagnosis, treatment for IBD is also an emerging field in which active research is going on. The traditional drugs used in treatment of IBD include steroids and aminosalicylates. Recent studies have evaluated the role of azathioprine, methotrexate, and cyclosporine. Apart from improving efficacy of therapy in short term, these agents have raised debate on complications of therapy, long-term efficacy, and cost. Management of IBD during pregnancy and fistulising Crohn’s disease are other areas of concern.

Available in PDF, EPUB and Kindle. Book excerpt: One of the most vivid memories from my medical school training was seeing my first surgical operation on a patient with Crohn’s disease. The senior surgeon at Mount Sinai Hospital in New York City, the same institution at which Burrill Crohn, Leon Ginzburg, and Gordon Oppenheimer had first described the disease “terminal ileitis,” had - doubtedly done countless operations on patients with inflammatory bowel disease in the past. Yet as we both gazed down into the patient’s open abdomen, at the “creeping fat” that seemed to be wrapping its sticky fingers around the young man’s intestines, he stated, “this is the m- tery of Crohn’s disease—no two patients are ever the same. ” What is it about the inflammatory bowel diseases, Crohn’s disease, and ulcerative colitis, that we find so intriguing? Is it the young age of the patients, many who are younger than even the medical students - tending to them? Or is it the elusive etiology, the theory of a “mystery organism” that has yet to be identified? Perhaps it is the familial pattern of disease, where many patients have relatives with similar diseases, yet in some instances only one of a pair of identical twins is affected. Regardless of the cause, these chronic diseases with a typically early age of onset, result in a long-term commitment of the patient, their fa- lies, friends, health care providers, researchers, employers, and even health care insurers and other health-related industries.

Book excerpt: Over the course of their Crohn's disease, up to half of affected individuals will develop a perianal penetrating complication (either perianal fistula or abscess). Symptoms can be debilitating, and the impact on quality of life significant. This first edition of 'Fast Facts: Complex Perianal Fistulas in Crohn's Disease' explains how fistulas form, who is at risk and the principles of diagnosis and classification. Later chapters explore the medical and surgical options, including innovative therapies. The intention throughout is to emphasize the importance of ‘joined-up thinking’ when caring for patients. Perianal fistulas are often challenging to treat in patients with Crohn’s disease, and a multidisciplinary approach is needed to get the best outcomes. Table of Contents: • Epidemiology, etiology and pathology • Anatomy and classification • Diagnosis • Management principles • Medical treatment Local perianal surgical (sphincter-preserving) interventions • Innovative therapies • Managing complications
