Book excerpt: This last year has seen the approval of the first targeted treatments for acute myeloid leukemia (AML) - following decades with no new developments and a poor prognosis for most patients with the disease. The new drugs reflect the remarkable progress that has been made in our understanding of the pathophysiology of AML and its underlying cytogenetic and molecular abnormalities - which differ not only between patients but also within a patient over time and with treatment. 'Fast Facts: Acute Myeloid Leukemia' provides a comprehensive yet concise foundation for understanding AML: from basic epidemiology, diagnosis, classification and the current 'standard' treatment, through to recent advances in our understanding of the cytogenetic and molecular underpinnings of the disease, such as the IDH2 mutation, and the future for tailored therapy. It will be useful to primary care providers, medical students, specialist nurses, junior doctors and allied healthcare professionals who want to develop a thorough grounding in our evolving understanding of AML and its treatment. Contents: • Epidemiology, pathophysiology and etiology • Diagnosis • Treatment • Supportive care • Prognosis and monitoring • Emerging treatments • Useful resources

This book was released on 2021-01-26 with total page 54 pages. Available in PDF, EPUB and Kindle. Book excerpt: The tyrosine kinase inhibitor (TKI) imatinib was the first treatment to specifically target cancer cells, rather than the relatively indiscriminate effects of conventional chemotherapy on any rapidly dividing cells. This concept of targeted treatment in cancer is one of the important advances in modern medicine in the last 30 years. Indeed, treatment with TKIs has transformed chronic myeloid leukemia (CML) from a cancer with a poor prognosis to one in which many patients can expect a normal lifespan. Success with the TKIs has prompted the question of whether it is desirable – or feasible – for patients to remain on treatment for long periods. While the TKIs are targeted, they are associated with considerable toxicity, and long-term treatment has important economic implications for health services and patients. Thus, the concept of treatment-free remission (TFR) has emerged for patients in deep clinical remission. Clinical research over the last decade has focused on whether treatment can be stopped, how to best monitor patients while off treatment, and how to intervene before a clinical relapse. As this research progresses, the tantalizing prospect of a cure for some patients seems increasingly feasible. This new Fast Facts title outlines this trail-blazing approach to the long-term management of patients living with CML in remission. It explains the concepts of molecular and hematologic relapse, the highly sensitive technologies that allow disease monitoring, and how TFR is best managed in practice. It is a concise educational resource, ideal for any healthcare professional involved in the treatment of patients with CML who wants to understand TFR, particularly clinical nurse specialists and pharmacists who increasingly help clinicians to run CML clinics. Table of Contents: • The concept of treatment-free remission • Measurement of disease burden • Clinical practice • Future directions

Although rare in adults, this aggressive cancer also shows a second smaller incidence peak in later middle age. Modern genetic profiling techniques are providing a better understanding of the molecular biology of ALL, which, in turn, is leading to significant advances in diagnosis, prognostication and therapy selection. Ongoing refinement of risk-adapted frontline treatment protocols, use of novel targeted therapies and better supportive care are improving outcomes for patients with ALL. Fast Facts: Acute Lymphoblastic Leukemia covers the epidemiology, etiology, diagnosis and classification of ALL as well as the measurement of minimal residual disease to determine prognosis. It provides guidance on frontline therapies and the treatment of relapsed and refractory ALL, including the use of newer targeted agents and the role of allogeneic stem-cell transplantation. Looking to the future, it also considers the approaches and refinements that are in development for ALL.Table of Contents: Epidemiology, pathophysiology and etiology Diagnosis Prognostic factors Treatment Relapsed or refractory ALL Future directions

Realization that hematopoietic stem cells give rise to the immune compartment has resulted in clinical trials of hematopoietic stem cell transplantation for patients with autoimmune diseases. Allogeneic hematopoietic transplants are a form of adoptive immunotherapy resulting in beneficial graft versus tumor effects. Large numbers of hematopoietic cells can be collected with ease. Therefore, a renewable source of cells for ex vivo genetic manipulations is readily available. Multiple trials combining hematopoietic transplants and gene therapy are in progress. One such application is the infusion of allogeneic lymphocytes containing a suicide gene to abort graft versus host disease. Hematopoietic stem cell transplantation is in reality the clinical and practical application of cellular therapy. Hematopoietic transplant physicians are by design or by practical application evolving into cell and gene therapy specialists. The excitement and enthusiasm in hematopoietic transplantation is that it offers a door to the future. A future not of drugs or titrating poisonous chemotherapy but rather of cellular and gene therapy. 1 ALLOGENEIC PERIPHERAL BLOOD STEM CELL TRANSPLANTATION FOR HEMATOLOGIC DISEASES Martin Korbling University o/Texas MD. Anderson Cancer Center, Houston, Texas 77030 INTRODUCTION Circulating hematopoietic stem cells have emerged as an alternative to bone marrow (BM) stem cells for allografting. For many years the reconstitutive potential of circulating stem cells was questioned; peripheral blood stem cells (PBSC) were even characterized a waste product (1).

Book excerpt: Presentation of the myeloproliferative neoplasms (MPNs) varies widely, and correct diagnosis and management can be challenging, and is becoming more complex as understanding of the underlying molecular basis for these disorders emerges. Appropriate management is increasingly informed by accurate risk stratification founded on understanding of cytogenetic and molecular markers, as well as the clinical presentation. 'Fast Facts: myeloproliferative neoplasms' focuses on the three most common chronic MPNs: • Essential thrombocythemia (ET) – characterized by increased platelet count • Polycythemia vera (PV) – characterized by excessive production of red blood cells • Primary myelofibrosis – a more severe and heterogenous disorder that may overlap with ET and PV but is commonly associated with anemia. Written by two leading experts in the field, Fast Facts: myeloproliferative neoplasms provides concise, up-to-date and practical guidance on the accurate diagnosis, risk stratification and management of these MPNs. It also provides key insights into our growing understanding of the underlying molecular and genetic basis of these disorders, and how this is informing risk stratification and management strategies. This concise handbook will be invaluable to clinicians, medical students, nurses, pharmacists and scientists in understanding and overcoming the everyday and rarer challenges associated with MPN.

Book excerpt: Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare fast-growing hematodermic neoplasm that most often occurs in older men. It is notable for its highly aggressive behavior, with cutaneous, lymph node and bone marrow involvement. In the past, BPDCN has been poorly understood, recognized and treated, and consequently has had a poor prognosis. Today, it has been reclassified as a myeloid neoplasm and there is greater understanding of the disease’s clinical features, course and pathology, a new diagnostic test that makes prompt diagnosis possible and a new targeted therapy that, so far, has been shown to at least double survival. The complexity of caring for patients with BPDCN stems from both its rarity and its multiorgan involvement. 'Fast Facts: Blastic Plasmacytoid Dendritic Cell Neoplasm' is designed to bring hematologists, oncologists, dermatologists, pathologists, clinical nurse specialists and trainees in all these fields up to speed on the latest developments, as well as providing the most up-to-date information on first-line chemotherapy, consolidation treatments and stem cell transplantation. It will aid readers of all relevant medical disciplines to implement prompt diagnosis and effective management. Table of Contents: • What is BPDCN? Available in PDF, EPUB and Kindle. Book excerpt: Detection of minimal residual disease (MRD) is increasingly used in the management of leukemia patients. A wide variety of methods have been developed and include technologies designed to detect residual malignant cells beyond the sensitivity of conventional approaches such as morphology and banding cytogenetics in leukemia. The choice of the best method depends on the biology of the individual malignancy, i.e. on the determination of specific markers which are useful to differentiate between leukemic cells and normal hematopoiesis in leukemic patients. These markers include leukocyte differentiation antigens, fusion transcripts, transcripts overexpressed by mutated or nonmutated genes, rearranged genes, and individual markers like polymorphic repetitive DNA sequences. The major technologies for MRD detection, their advantages and disadvantages and their clinical applications are discussed in this special issue - from 'bench to bedside'. Providing a comprehensive overview on the significance of MRD in the evaluation, treatment and follow-up of hematologic malignancies, it will be of great value to hematologists, researchers interested in leukemias and lymphomas as well as laboratory technicians.
