Book excerpt: Rapid readmission (RR) of psychiatric patients within 30 days of discharge places a costly burden on state psychiatric facilities and may be an indicator of suboptimal service provision. Several studies have previously considered RR to inpatient facilities, but there is a lack of information available about the variables associated with RR of psychiatric patients to state-operated inpatient facilities in Nevada. This study attempts to identify factors associated with RR at a southern Nevada state psychiatric hospital. Participants included 7,177 patients admitted between May 1, 2012 and April 30, 2014. All 12,068 admissions, including 2,220 RR, were reviewed, and rapid readmits were compared to their counterparts who were not readmitted within 30 days in a multivariate model using logistic regression. Multinomial logistic regression was utilized to determine if risk factors varied based on the number of RR, and to ascertain whether or not previous length of stay (LOS) had an impact on time to readmission. Whenever possible, analyses were run separately including all admissions and each patient's first admission only. Results from multiple logistic regression consistently demonstrated that those aged 35-44, never married, divorced, and living in a homeless shelter or other residential or institutional setting are at increased risk for RR; in contrast, females, persons not receiving social security disability, and those over 55 years of age were at reduced risk. Significant factors associated with RR also included a history of legal problems, medication noncompliance, lack of stable housing, and diagnosis with a psychotic or substance use disorder. Persons living in a homeless shelter and having a history of legal issues were more likely to be in the group most often rapidly readmitted (four or more times), and those that did not receive social security disability were less likely to be included in this high risk group. Previous LOS did not contribute significantly in multivariate modeling of time to readmission. Nevada currently suffers from budget cuts in mental health care spending, which were a result of the recent economic crisis, and a severe lack of bed space in southern Nevada. This study demonstrates that it may be possible to reduce rates of costly RR by focusing on those with a history of rapid readmission and modifiable factors including social and financial supports and housing.

Book excerpt: Abstract: The purpose was to examine service providers' perceptions of factors contributing to frequent psychiatric hospital readmission for individuals with severe mental illness and the system's capability to intervene regarding those factors. The study used a quantitative survey completed by 25 staff in an inpatient psychiatric hospital. Staff rated the capability of the system to intervene lower than the frequency of occurrence of most readmission factors, indicating that interventions are insufficient. Results were especially significant for substance use, medication noncompliance, and poor psychiatric follow up, with staff tending to view these factors as high in frequency but low in terms of the system's capability to intervene. Increased funding and outpatient services were suggested by participants.

Book excerpt: Background: Hospital readmission with in thirty day time period has been a major issue in the United States and as per data from Center forMedicare & Medicaid Services, the cost of readmission is estimated to be 26$ billion annually. There is an enormous push to reduce these readmission and in New York. Delivery System Reform Incentive Payment Program (DSRIP) has been established to restructure the health care delivery system with the primary goal of reducing re-admissions by 25% over 5 years with the budget of up to $6.42 billion dollars. DSRIP uses LACE score (Length of stay, Acuity of admission, Co-morbidities & Emergency Department visits in the previous six months) as an assessment tool for determining the high-risk patients for emergent hospitalizations in medicine, but this score has limited validity to psychiatric patients. Relatively new tools developed for psychiatric hospitalization, READMIT score incorporates a weighed scale based on the following factors: repeat admissions, emergent admissions (i.e.harm to self/others), diagnosis (psychosis, bipolar and orpersonality disorder), unplanned Discharge, medical co-morbidity,prior service use intensity, and time in hospital. In an attempt to provide better outcomes for our patient population BLHC has developed a scale that attempts to incorporate factors specific to its own patient populations. This graded scale was coined ZAC score based on risk factors identified on literature review and with 4 components as shownbelow and the score range is from 0 to 12. Methods: We scored the available tools LACE, READMIT and ZAC score for 170 patients, who were admitted to our inpatient units, to assess the accuracy of both measures in predicting psychiatric inpatient readmission within 30 days. The sample consisted of consecutively admitted patients to our inpatient psychiatric units at Bronx Lebanon hospital between June 2014 to June 2015 with Medicaid as health insurance and The follow up data was collected using Office of mental health database PSYCKES fromNew York State Medicaid claims database. Cox Regression analysis was performed for multi-factorial prediction model for 30 day readmission.Results:The readmission rate within 30 days was 28 percent. The variables predicting early readmission were severity of pathology (history of prior hospitalization >= 1 in six months and >=3 in past three years), non compliance (patients on TOO, AOT and ACT teams) and violent behavior (prior to admission, requiring > 2 IM injections within 72 hours). Length of hospitalization was not a predictor for 30 day readmission on multi-factorial analysis.Conclusion: To date, there is no validated tool, used to predictre-admissions in the psychiatric patient population. READMIT & ZAC scores were better in predicting 30 day readmission in psychiatric setting when compared to LACE score. The variables predicting early readmission in our data set were severity of illness, non compliance and violent behavior prior to readmission. Length of hospitalization was not a predictor for 30 day readmission.

Book excerpt: The present study examines the impact of psychotropic polypharmacy and medication non-adherence on hospital inpatient readmissions within the mental health population. Previous research attempted to determine whether or not there are factors associated with predicting readmissions and has found conflicting results. However, there has been consistent research that demonstrated the relationship between medication non adherence and hospital readmissions. This study expands the research by investigating psychotropic polypharmacy as a factor that coincides with medication non-adherence that leads to rapid readmissions. It was hypothesized that participants who were prescribed a higher quantity of psychotropic medications and scored lower on the Medication Adherence Rating Scale (MARS) would have a higher number of inpatient incidences within a 90 day time frame. The results of the Pearson and Spearman correlation failed to find a significant correlation between quantity of psychotropic medications and MARS scores. Additionally, the logistic regression indicated that neither quantity of psychotropic medications nor MARS scores significantly predicted hospital readmissions. However, the MARS score and number of hospital incidences was in the hypothesized direction. These results may have been impacted by the low sample size and the method used to measure medication adherence. A cohort matched design with an equal amount of participants in the readmission and non readmission group may have been beneficial for this study to use. Overall, this study has found that polypharmacy is prevalent in the psychiatric community and that the use of 2-3 psychotropic medications may be beneficial in the treatment and management of mental illness.
