Book excerpt: Quick vaccine rollouts are crucial for a strong economic recovery, but vaccine hesitancy could prolong the pandemic and the need for social distancing and lockdowns. We use individual-level data from nationally representative surveys developed by YouGov and Imperial College London to empirically examine the determinants of vaccine hesitancy across 17 countries and over time. Vaccine demand depends on demographic features such as age and gender, but also on perceptions about the severity of COVID-19 and side effects of the vaccine, vaccine access, compliance with protective behaviors, overall trust in government, and how information is shared with peers. We then introduce vaccine hesitancy into an extended SIR model to assess its impact on pandemic dynamics. We find that hesitancy can increase COVID-19 infections and deaths significantly if it slows down vaccine rollouts, but has a smaller impact if all willing adults can be immunized rapidly.

Available in PDF, EPUB and Kindle. Book excerpt: This book is an expanded version of the Kahn's widely used text, An Introduction to Epidemiologic Methods (Oxford, 1983). It provides clear insight into the basic statistical tools used in epidemiology and is written so that those without advanced statistical training can comprehend the ideas underlying the analytical techniques. The authors emphasize the extent to which similar results are obtained from different methods, both simple and complex. To this edition they have added a new chapter on "Comparison of Numerical Results for Various Methods of Adjustment" and also one on "The Primacy of Data Collection." New topics include the Kaplan-Meier product-limit method and the Cox proportional hazards model for analysis of time-related outcomes. An appendix of data from the Framingham Heart Study is used to illustrate the application of various analytical methods to an identical set of real data and provides source material for student exercises. The text has been updated throughout.

Book excerpt: Having a child remains one of the biggest health risks for women worldwide. Fifteen hundred women die every day while giving birth. That's a half a million mothers every year. UNICEF's flagship publication, The State of the World's Children 2009, addresses maternal mortality, one of the most intractable problems for development work. The difference in pregnancy risk between women in developing countries and their peers in the industrialised world is often termed the greatest health divide in the world. A woman in Niger has a one in seven chance of dying during the course of her lifetime from complications during pregnancy or delivery. That's in stark contrast to the risk for mothers in America, where it's one in 4,800 or in Ireland, where it's just one in 48,000. Addressing that gap is a multidisciplinary challenge, requiring an emphasis on education, human resources, community involvement and social equality. At a minimum, women must be guaranteed antenatal care, skilled birth attendants and emergency obstetrics, and postpartum care. These essential interventions will only be guaranteed within the context of improved education and the abolition of discrimination.

Issues discussed include the innovative use of vaccines against diseases such as meningococcal infection in Africa, Haemophilus influenza type b, varicella, and hepatitis, efforts to develop a new generation of vaccines against cholera and typhoid, shigella and Helicobacter pylori, as well as developments in the quest for vaccines against tuberculosis, HIV/AIDS, dengue, malaria, and hookworm. It also deals with the use of vaccines to fight bioterrorism attacks; regulatory and safety issues; financing issues, impact of health sector reform and the sustainability of immunisation programmes.

Available in PDF, EPUB and Kindle. Book excerpt: Measles, also called the greatest killer of children in history, still annually affects about 50 million individuals and causes close to a million deaths primarily in developing countries. Before the advent of measles vaccine some 30 years ago, these figures were roughly three times higher. Attenuated measles virus (MV) strains, all quite closely related to the original Edmonston isolate, have a very good record as a safe and highly efficacious vaccine and have brought down the measles toll in industrialized countries to almost negligible levels. However, recent outbreaks in the USA and Europe have again brought the measles problem to public attention. Sadly enough, these outbreaks were more instrumental in inducing activities to drastically reduce and hopefully finally eradicate measles than were the ten thousand times higher number of victims in developing countries. To reach this goal, as detailed in this volume, apparently it is not enough to of the existing vaccine as was the rigorously enforce use case with smallpox eradication: the intricacies of measles disease phenomena, in particular the generalized immune suppression which favors secondary infections, require more basic knowledge of the virus-host interactions and probably the development of new vaccines for special applications such as first immunizations of very young infants in developing countries.
