Book excerpt: Fruit and vegetable intake is not uniform across levels of socioeconomic status (SES) and researchers have identified low SES as a risk factor for poor intake of fruits and vegetables. In an effort to eliminate public health disparities and increase fruit and vegetable intake, the Women, Infant, and Children (WIC) program implemented additional food assistance programs, with a specific emphasis on fresh fruits and vegetables. The Farmers' Market Nutrition Program (FMNP) provides pre-existing WIC clients with coupons to purchase fresh, locally grown produce at farmers' markets. In addition, Congress also approved the WIC Cash Value Voucher (CVV) program, which provides WIC participants with vouchers to purchase fresh fruits and vegetables at farmers' markets or grocery stores. The purpose of this thesis was to investigate the relation of FMNP coupon use with accessibility and WIC CVV redemption rates at farmers' markets. Furthermore, this thesis addressed whether WIC shoppers redeemed a higher percentage of their WIC CVV value at farmers' markets or grocery stores. WIC CVV and FMNP issuance and redemption data were analyzed to establish overall redemption rates and total perecent of WIC CVV value redeemed. Accessibility was assessed using the Geographic Information System, which allowed me to calculate the distance that WIC participants would have to travel to redeem their FMNP coupons at FMNP-approved farmers' markets. The results showed that less than 1% of WIC shoppers redeem their WIC CVVs at farmers'markets in Arizona. However, the redemption of WIC CVV was significantly higher during the months when shoppers had the option of using both WIC CVV and FMNP coupons at farmers' markets. Furthermore, the percent of total CVV value redeemed at farmers' markets was 99%, significantly higher than grocery stores (93.5%). Average FMNP coupon redemption rates for 2008-2010 was 43.3%, well below the national average of 59%. However, my spatial analysis revealed that there was no significant association between the distance traveled to farmers' markets and FMNP redemption rates. This indicates that the distance traveled to farmers' markets is not a major barrier to redemption of FMNP coupons in Arizona.

Book excerpt: The Special Supplemental Nutrition Program for Women, Infants and Children (WIC) is the third largest food assistance program in the United States and many studies have demonstrated positive health benefits from participation in WIC. However, several barriers prevent potential recipients from joining and fully utilizing available benefits. First, the participation rate is disturbingly low and has fallen in recent years to about 55 percent. Second, I have uncovered evidence of a high rate of partial or non-redemption of benefits by program participants, meaning the participant is obtaining none or only part of the food benefits that have been prescribed. Third, WIC is not anentitlement program, so its ability to serve clientele depends on cost efficiency, but many aspects of the program are ill-suited to achieving cost efficiency. This dissertation focuses on these major problems of the WIC program and investigates the impact of various WIC polices on WIC participants and WIC program cost. WIC has mandated changes to its food issuance and redemption method from paper vouchers to electronic benefit transfer (EBT) by 2020. In the first chapter of my dissertation, I study the changes in WIC program participation and program food costs after EBT transition. County level WIC enrollment data and WIC food issuance and redemption data in Oklahoma are used to perform the empirical analysis. The transition to EBT has been anticipated to increase participation and decrease food costs because it provides WIC participants more flexibility in redeeming food benefits, reduces the time cost and stigma cost of WIC participants, and prevents redemption of expensive, non-WIC-eligible food items. However, I find no statistically significant change in program participation after EBT transition. But EBT reduced average participant food costs about $8.24 per month in Oklahoma. Applying these savings to WIC participants in all the states generates $56 million in estimated cost savings annually. The second chapter of my dissertation focuses on partial redemptions and factors that affect WIC participants' redemption behavior. A partial redemption occurs when a participant redeems only a portion of the prescribed benefit, thereby not obtaining the full nutritional benefit. Partial redemptions are a major issue for the WIC program because considerable research has demonstrated the positive health benefits from WIC, but those benefits are mitigated if participants don't purchase and consume their prescribed foods. Using the transaction-level WIC redemption data, I study the probability that a household partially redeems its benefits in a given month, andthe factors that could affect this probability. The results identify WIC products that are most likely to be partial redeemed and household characteristics associated with partial redemptions, enabling targeted nutritional counseling. The third chapter of my dissertation focuses on WIC participants' food choices and explores the role that choice of WIC-authorized brands, package sizes, product types, etc. have in determining program food costs. State programs have discretion in designating foods eligible for the WIC program, subject to regulations set by the federal government. I combine WIC administrative data with IRI supermarket scanner data to simulate WIC participants' shopping behavior and compare the results with the actual food redemption data from California WIC. I find that the actual shopping patterns of California WIC participants closely resemble a situation wherein participants select the most expensive authorized products, brands, and package sizes. The policy implications from this study suggest that restricting participant product selection to least cost brands WIC foods leads to program cost savings between 4.6 percent and 9.5 percent, depending upon the food packages.

Book excerpt: In October, 2009, participants of the Arizona Special Supplemental Nutrition Program for Women, Infants and Children (WIC) began receiving monthly Cash Value Vouchers (CVV) worth between six and 10 dollars towards the purchase of fresh fruits and vegetables. Data from the Arizona Department of Health Services (ADHS) showed CVV redemption rates in the first two years of the program were lower than the national average of 77% redemption. In response, the ADHS WIC Food List was expanded to also include canned and frozen fruits and vegetables. More recent data from ADHS suggest that redemption rates are improving, but variably exist among different WIC sub-populations. The purpose of this project was to identify themes related to the ease or difficulty of WIC CVV use amongst different categories of low-redeeming WIC participants. A total of 8 focus groups were conducted, four at a clinic in each of two Valley cities: Surprise and Mesa. Each of the four focus groups comprised one of four targeted WIC participant categories: pregnant, postpartum, breastfeeding, and children with participation ranging from 3-9 participants per group. Using the general inductive approach, recordings of the focus groups were transcribed, hand-coded and uploaded into qualitative analysis software resulting in four emergent themes including: interactions and shopping strategies, maximizing WIC value, redemption issues, and effect of rule change. Researchers identified twelve different subthemes related to the emergent theme of interactions and strategies to improve their experience, including economic considerations during redemption. Barriers related to interactions existed that made their purchase difficult, most notably anger from the cashier and other shoppers. However, participants made use of a number of strategies to facilitate WIC purchases or extract more value out of WIC benefits, such as pooling their CVV. Finally, it appears that the fruit and vegetable rule change was well received by those who were aware of the change. These data suggest a number of important avenues for future research, including verifying these themes are important within a larger, representative sample of Arizona WIC participants, and exploring strategies to minimize barriers identified by participants, such as use of electronic benefits transfer-style cards (EBT).

Twenty three WIC participants in five focus groups. Phenomenon of interest: Barriers and enhancers of WIC check redemption, especially the cash value voucher (CVV) for fruits and vegetables. Transcriptions were coded; coded statements were organized into major themes across a socioecologic framework. Coding structures and analysis were strengthened by iterative interactions between researchers. Barriers to redemption arise from multiple sociological levels, but benefits and positive factors, as well as solutions participants use, may lessen the effects of barriers. Factors negatively impacting the WIC shopping experience included food package policies, grocery store experiences, and personal misunderstanding and embarrassment; positive factors included helpful vendors and both vendor and participant understandings about the use of CVV. WIC check redemption may be improved by state and local actions to educate participants and vendors and improve the shopping experience.

Today the program serves more than a quarter of the pregnant women and half of the infants in the United States, at an annual cost of about $6.2 billion. Through its contribution to the nutritional needs of pregnant, breastfeeding, and post-partum women; infants; and children under 5 years of age; this federally supported nutrition assistance program is integral to meeting national nutrition policy goals for a significant portion of the U.S. population. To assure the continued success of the WIC, Congress mandated that the Food and Nutrition Service of the U.S. Department of Agriculture (USDA) reevaluate the program's food packages every 10 years. In 2014, the USDA asked the Institute of Medicine to undertake this reevaluation to ensure continued alignment with the goals of the Dietary Guidelines for Americans. In this third report, the committee provides its final analyses, recommendations, and the supporting rationale.
