Book excerpt: L’objectif de cette étude était de dresser un état des lieux et de décrire les infiltrations auprès des médecins généralistes du département de Saône-et-Loire. Méthodes : Un tirage au sort a permis de constituer un échantillon de 200 médecins généralistes libéraux. Chaque médecin a reçu, par courrier, un questionnaire comportant 16 questions. Résultats : 143 questionnaires ont pu être exploités (71.5%). 64% ont déclaré pratiquer des infiltrations avec une fréquence moyenne inférieure à 30 par an. Il s’agissait le plus souvent d’hommes, âgés de plus de 55 ans, installés depuis plus de 20 ans, qui exerçaient seuls, en zone rurale. L’épaule, le canal carpien et l’épicondyle correspondaient aux trois sites les plus fréquemment infiltrés. L’information médicale et les précautions d’asepsie étaient majoritairement respectées. Très peu de complications graves ont été rapportées. En cas d’impossibilité d’infiltration, les rhumatologues étaient les plus sollicités. L’absence de formation et la peur d’une complication médico-légale constituaient les principaux freins. Globalement, ce geste était décrit comme accessible à tous, avec un désir de formation plus important chez les praticiens qui en réalisaient. Conclusion : La plupart des médecins généralistes semblent réaliser des infiltrations, notamment extra-articulaires. Beaucoup de praticiens se refusent à en pratiquer devant un manque de formation. Les étudiants étant de mieux en mieux formés à ce geste, nous pouvons penser que de plus en plus de médecins généralistes en réaliseront. Pour les médecins déjà en activité, il apparait nécessaire de leur proposer un plus grand nombre de FMC traitant des infiltrations.
