Cependant, les dernières recommandations de la HAS (2013) les relèguent en seconde intention pour la bithérapie avec la metformine, après les sulfamides hypoglycémiants. Méthode : Cette étude qualitative a été réalisée par l'analyse thématique de 10 entretiens semi dirigés auprès de médecins généralistes. Elle cherchait à mettre en évidence les différents facteurs poussant les médecins à préférer les IDPP4 aux sulfamides. Résultats : En plus de critères quantitatifs facilement identifiables, l'approche qualitative à permis de découvrir d'autres facteurs plus subjectifs. Les médecins mettaient en balance les avantages et inconvénients de chaque classe (efficacité sur l'HbA1c, effet sur le poids, coût, effets secondaires). Leur critère principal semblait être l'observance, nettement améliorée par les associations fixes IDPP4/metformine. D'autres facteurs venaient influencer la prise en compte des premiers. Il s'agit des rapports qu'entretenait chaque médecin avec les différents acteurs de santé publique, que sont l'industrie pharmaceutique et les autorités sanitaires. Ces rapports sont eux mêmes influencés par le statut du médecin, son âge, ses sources d'information et par les récents retraits de molécules et autres scandales médiatiques survenus dans le domaine du diabète. Deux profils de médecins ont émergé : les premiers, souvent les plus jeunes, semblaient être réfractaires aux laboratoires, ils rejetaient les nouvelles molécules et préféraient suivre la HAS, tandis que d'autres plus âgés recevaient l'information des différents acteurs avec moins d'a priori et voulaient se forger leur propre opinion. Ils décidaient souvent de ne pas suivre ces recommandations estimant qu'une meilleure observance primait sur le coût du traitement. Conclusion : Les recommandations relèguent ces molécules en seconde intention principalement pour leur coût et le manque de recul sur leur sécurité. Depuis leur parution, certaines suspicions d'effets secondaires ont été en partie levées par de nouveaux essais cliniques. La recherche dans le domaine du diabète s'est considérablement accélérée, de nouvelles pistes sont à l'étude et l'éventail thérapeutique risque de s'élargir dans un futur proche. Les recommandations ainsi que l'opinion des médecins devront donc s'inscrire dans une constante évolution.

En février 2013, une nouvelle recommandation de l'HAS est sortie sur la prise en charge des diabétiques de type 2. Elle est centrée sur le patient. L'objectif de cette étude est de recueillir le ressenti des médecins généralistes sur la personnalisation de la prise en charge des diabétiques de type 2 et de mettre en évidence les motifs pouvant les amener à diminuer le traitement médicamenteux. Méthode : cette enquête qualitative a été menée auprès de 15 médecins généralistes du Nord par des entretiens semi-dirigés. Elle a été conduite jusqu'à saturation des données. Un codage des données a été réalisé par théorisation ancrée sur le logiciel N-vivo10 avec triangulation des codes ouverts. Résultats : Il en ressort que les médecins généralistes appliquaient déjà les directives de la recommandation HAS 2013 bien avant sa sortie. Ils perçoivent, dans la personnalisation du traitement du diabétique de type 2, un ensemble de critères d'adaptation à la fois clinico-biologiques et socio-économiques. Ces derniers sont également les causes pouvant les amener à baisser les antidiabétiques. Dans les deux cas, ils évoquent de nombreuses difficultés à y parvenir. Conclusion : Les résultats obtenus ne font que confirmer la problématique de la prise en charge des diabétiques de type 2 par les médecins généralistes malgré les recommandations de bonnes pratiques récentes. La perspective de directives plus précises sur les motifs de diminution des traitements pourraient apporter une aide supplémentaire.
