Book excerpt: La présente étude, effectuée da,s le département du Val-de-Marbe durant la période de 1997 à 2000, évalue la fréquence et les facteurs de risque de transmission de la tuberculose (infection latente et tuberculose maladie) chez les enfants exposés à leur domicile à des adultes présentant une tuberculose pulmonaire active (culture positive à Mycobacterium tuberculosis). Ainsi, 39 cas index (âgés de plus de 18 ans) au contact intrafamilial étroit avec 91 enfants (âgés de 3 mois à 17 ans) ont été inclus dans cette étude. Tous les enfants ont été évalués initialement par une intradermo-réaction à la tuberculine et par une radiographie du thorax. Puis, une seconde évaluation a été effectuée 3 mois plus tard pour les enfants non infectés à la première évaluation. 20 des 91 enfants (22%) ont été infectés dont 4 enfants à la seconde évaluation. Le risque d'infection tuberculeuse n'est pas lié à l'âge des enfants, mais associé de façon significative à 3 caractéristiques relatifs aux cas index : leur jeune âge, la présence de caverne, et l'examen direct positif de plus de 100 BAAR/champ. En conclusion, nos résultats confirment l'intérêt de l'évaluation des enfants exposés aux adultes contaminateurs afin de dépister l'infection latente et d'éviter la progression vers une tuberculose active, et l'intérêt d'une seconde évaluation 3 mois plus tard.
