D'après la littérature, 10 à 30% des réhospitalisations sont liées à un évènement iatrogène médicamenteux. En 2013, dans une note de synthèse intitulée "Comment réduire le risque de réhospitalisations évitables des personnes âgées", la Haute Autorité de Santé (HAS) préconise un repérage précoce des patients devant bénéficier d'interventions médicales visant à réduire le risque de réhospitalisation. Nous avons réalisé une étude cas-témoins appariés grâce à un entrepôt de données cliniques afin d'identifier les facteurs de risque de réhospitalisation des personnes âgées, en particulier les facteurs iatrogènes : une première hospitalisation en urgence ; la prescription de médicaments du système nerveux central en sortie d'hospitalisation ; la prescription d'antidépresseurs en sortie d'hospitalisation ; les antécédents de réhospitalisations non programmée (à 30 jours) et un retour à domicile après la sortie d'hospitalisation. Ces facteurs de risque nous permettront d'identifier les patients les plus à risque de réhospitalisation et de leur faire bénéficier d'une intervention en vue de réduire le risque de réhospitalisation, afin de répondre aux recommandations d'efficience de la HAS.

Objectifs : Déterminer le taux et les facteurs de risques de réhospitalisation précoce non programmée après un séjour en médecine gériatrique au CHU de Nîmes sur la totalité des patients hospitalisés sur une période de un an comparativement à la précédente étude réalisée sur un échantillon de malades. Méthode : Il s'agit d'une étude rétrospective monocentrique, se basant sur les données cliniques, biologiques et sociodémographiques des patients hospitalisés en Médecine Gériatrique au CHU de Nîmes entre le 1er novembre 2013 et le 31 octobre 2014. Les patients décédés au cours de leur séjour ou mutés dans un autre service, ainsi que les hospitalisations programmées ont été exclus. Résultats : Parmi les 1161 malades inclus, nous avons mis en évidence un taux de réhospitalisation à 30 jours de 12,06%. L'indépendance du patient, évaluée par un ADL haut, s'avère être un facteur de risque de réhospitalisation (OR= 1,1, IC à 95% [1,01- 1,20] p = 0,035) de manière significative, tout comme l'hospitalisation pour le motif de cancer (OR=3.32, IC95% [1.23 ; 8.95], p = 0.018). En revanche, si le patient est hospitalisé pour chute (OR=0.44 (IC95% [0.22 ; 0.88], p = 0.021) ou dans le cadre de son syndrome démentiel (OR 0.31 (IC95% [0.10 ; 0.95], p = 0.04) ce risque est significativement diminué. Conclusion : L'indépendance du malade et le motif d'hospitalisation pour cancer semblent être associés à une majoration du risque de réhospitalisation précoce. Leur identification, le plus précocement possible, nous permettrait de cibler ces patients à risque, d'améliorer la transition ville/hôpital et d'assurer une surveillance post hospitalière optimale.

Le but de cette étude était d’obtenir le point de vue des médecins généralistes dans l’Oise sur les facteurs de risque de réhospitalisation des personnes âgées et les moyens pour prévenir leur apparition. Matériel et méthodes : il s’agissait d’une étude qualitative qui a été réalisée par des entretiens semi-dirigés auprès de quinze médecins généralistes de l’Oise. Le recrutement des participants avait pris fin lorsque la saturation des données a été atteinte. Résultats : l’analyse des entretiens mettait en évidence trois points de réflexion. Le premier s’articulait autour du patient lui-même avec ses propres pathologies, sa fragilité et son contexte socio-environnemental. Le second s’intéressait à la prise en charge hospitalière avec une durée moyenne de séjour de plus en plus courte et des difficultés à accéder à des avis spécialisés. Le troisième portait sur le relais à domicile avec une optimisation de la sortie par des entretiens, l’obtention rapide de compte-rendu d’hospitalisation détaillé, la mise en place d’aides et une visite à domicile par le médecin traitant. Discussion et conclusion : les facteurs de risque de réhospitalisation étaient nombreux. Certains étaient inévitables et liés au patient lui-même. A contrario d’autres étaient modifiables et cela passait par une collaboration pluriprofessionnelle : visite du médecin traitant, conciliation médicamenteuse, passage d’infirmier à domicile, organisme pour aménagement du domicile.
