Book excerpt: En 2010, le nombre de personnes ignorant leur séropositivité au VIH en France s'élevait à 30 000. Les personnes diagnostiquées tardivement ne peuvent pas bénéficier de l'intégralité des avantages du traitement antirétroviral et favorisent la diffusion de l'épidémie. L'objectif de cette étude était d'améliorer l'identification des facteurs de risque associés au diagnostic tardif des personnes vivant avec le VIH (PVVIH) suivies au CHU de Nantes, en utilisant en plus de la méthode classique de régression logistique, 2 méthodes statistiques alternatives : l'analyse multiniveau et la régression quantile. Cette étude a inclus 1179 patients PVVIH suivies au CHU de Nantes et diagnostiquées entre 2010 et 2014. Des variables individuelles et contextuelles communales (niveau d'urbanisation, niveau socio-économique, accessibilité aux médecins généralistes) ont été simultanément prises en compte dans l'analyse multiniveau (diagnostic tardif si 350 CD4/mm3). La variation de la distribution du taux de CD4 au diagnostic en fonction des variables explicatives a été étudiée tous les 5 percentiles de CD4 pour la régression quantile. Dans l'analyse multiniveau, les facteurs associés au diagnostic tardif étaient le milieu de résidence quasi-rural/quasi-urbain (RCa = 1,69 ; IC95% : [1,13-2,52]), la naissance dans un pays étranger, le mode de transmission non sexuel, l'âge élevé, l'inactivité professionnelle, être en couple et la période de diagnostic entre 2000 et 2003. Avec la régression quantile, le sexe masculin, le mode de transmission hétérosexuel et la co-infection VHB et VHC étaient également associés négativement au taux de CD4 seulement à certains quantiles. L'impact de ces variables oscillait entre 30 et 200 CD4. Les résultats de cette étude suggèrent que le dépistage VIH doit être développé dans les périphéries urbaines et les grandes villes isolées ainsi que ciblé chez certaines populations dites à faibles risque d'infection (âge 30 ans, hétérosexuel, en couple) mais aussi chez les migrants.
