From birth, children are learning and rely on parents and the other caregivers in their lives to protect and care for them. The impact of parents may never be greater than during the earliest years of life, when a child's brain is rapidly developing and when nearly all of her or his experiences are created and shaped by parents and the family environment. Parents help children build and refine their knowledge and skills, charting a trajectory for their health and well-being during childhood and beyond. The experience of parenting also impacts parents themselves. For instance, parenting can enrich and give focus to parents' lives; generate stress or calm; and create any number of emotions, including feelings of happiness, sadness, fulfillment, and anger. Parenting of young children today takes place in the context of significant ongoing developments. These include: a rapidly growing body of science on early childhood, increases in funding for programs and services for families, changing demographics of the U.S. population, and greater diversity of family structure. Additionally, parenting is increasingly being shaped by technology and increased access to information about parenting. Parenting Matters identifies parenting knowledge, attitudes, and practices associated with positive developmental outcomes in children ages 0-8; universal/preventive and targeted strategies used in a variety of settings that have been effective with parents of young children and that support the identified knowledge, attitudes, and practices; and barriers to and facilitators for parents' use of practices that lead to healthy child outcomes as well as their participation in effective programs and services. This report makes recommendations directed at an array of stakeholders, for promoting the wide-scale adoption of effective programs and services for parents and on areas that warrant further research to inform policy and practice. Book excerpt: Childhood overweight and obesity are prevalent throughout the United States and pose significant risks to child well-being. Significant relationships have been previously reported between family functioning and caregiver influence (parent feeding behaviors, attitudes, and parenting styles) with children's weight status; however, less research has been conducted with African American families to determine if these relationships are consistent within this specific population. This study sought to understand the relationships between family demographics, family functioning, and caregiver influence through dietary and parenting behaviors, fruit and vegetable eating patterns, and child weight status in African American families with a child 7-13 years old. There were 54 child and caregivers dyads who participated in a one-time survey. The Family Assessment Device, Caregiver Feeding Questionnaire, Fruit and Vegetable Eating Patterns Questionnaire, Parenting Style Inventory, and family demographics, including child and caregiver weight status were utilized in this study. Results indicate that caregiver's weight concern for their child and caregiver self- report weight over time were significantly associated with child BMI percentile, where caregivers demonstrated more concern for their child’s weight if their child was overweight or obese compared to children who were a healthy weight. Caregivers who reported having a higher weight status across their lifespan were also more likely to have an overweight or obese child. Results were further analyzed for demographic differences on the aforementioned inventories. Future research should further explore the relationship between caregiver weight concern for their child and dietary and parenting behaviors, and its subsequent influence on child weight status in African American families.

Background: For the first time in history, the current generation of children is projected to have a shorter life expectancy than their parents primarily because of children's overweight status. Many factors place young children at risk for becoming overweight. The risk factors include: a family history of obesity, family lifestyle, diet sedentary living environments, and a lack of exercise. Purpose: To examine the relationship(s) between a mother's mealtime habits, maternal BMI, the location of maternal meals, and infant weight gain at two months of age and at six months of life. Subjects: Mother and infant dyads (N=160) from three different pediatric clinics participated in the study. Racial and financial considerations were considered in clinic selection. Exclusion criteria required that the infant was: not born younger than 36 weeks, not hospitalized, not diagnosed with a genetic syndrome, and not diagnosed with a gastrointestinal disease or a metabolic disorder. Almost 60% of the mothers of the sample are African American, 34% are white, and 2.5% are Hispanic. The age of the mothers was positively skewed, and ranged from 15 years of age to 42 years of age, with a mean age of 24. Methods: Questionnaires were administered to each maternal participant at the 2 month and 6 month well-child visit. A series of closed-ended questions assessing maternal eating habits were included. It is hypothesized that mothers who eat their main meal at the kitchen or dining room table will have infants at average weight at 6 months of age and mothers who eat their main meal elsewhere would have heavier infants at 6 months of age. Descriptive statistics including frequencies, means and standard deviations as well as Pearson correlations will be conducted. The burgeoning field of epigenetics has led researchers to speculate that many of the observed associations between early developmental exposures and later risk of childhood obesity are mediated, at least in part, through epigenetic mechanisms. To explore the body of evolving science that examines the nexus of biology, environment, and developmental stage on risk of childhood obesity, the Institute of Medicine and the National Research Council convened a workshop in February 2015. The workshop focused on the prenatal period, infancy, and early childhood and addressed evidence from both animal and human studies. Workshop objectives were to (1) identify epigenetic-mediated relationships between exposure to risk factors during sensitive periods of development (gestation through age 3) and subsequent obesity-related outcomes; (2) explore the science around periods of plasticity and potential reversibility of obesity risk in the context of early childhood development; and (3) examine the translation of epigenetic science to guide early childhood obesity prevention and intervention to reduce obesity risk. African American families are often multi-generational, with extended family members, especially women, caring for child relatives. Yet little is currently known about the influence of these female extended family members (EFMs) on African American children's dietary and physical activity behaviors. The purpose of this study was to examine relationships between the amount of time an African American child spent with an EFM, the frequency of child physical activity and eating around the EFM, and family members' weight status. Data were collected and analyzed as part of a larger study examining relationships in African American families (n=47). Participating families consisted of the mother, her child aged 8-12 years, and an EFM. Participants were recruited from community organizations in Guilford County, North Carolina. Family Systems Theory guided the research in accordance with current literature which suggests that family-based lifestyle interventions achieve better outcomes than education or information only interventions. Eighty-eight percent of non-pregnant mothers (n=44) and 87% of non-pregnant EFMs (n=45) were classified as overweight or obese as defined by Body Mass Index (BMI) compared to 47% of the children in the study as defined by Body Mass Index For Age (BMI-For-Age). EFMs comprised 10 grandmothers, two great grandmothers, one great aunt, 19 aunts, and 15 cousins. No significant association was found between time spent with EFMs and frequency of child eating and physical activity behaviors. Despite genetic and ecological factors that may contribute to intergenerational transmission of obesity, many adolescents with an obese mother are not overweight/obese themselves. The current study examines three domains (i.e., SES/neighborhood factors, family context, and individual weight behaviors and attitudes) drawn from ecological models of health that may influence mother/daughter weight similarity. Participants included 181 low-income adolescent females and their biological mothers. Four dyadic groups were compared, including; 1. Obese mothers with an overweight/obese daughter (OM/OD), 2. Non-obese mothers with an overweight/obese daughter (NOM/OD), 3. Obese mothers with a non-overweight/obese daughter (OM/NOD), and 4. Non-obese mothers with a non-overweight/obese daughter (NOM/NOD). Multivariate and univariate analyses of covariance with planned contrasts and chi-square tests were used to test for group differences. Of particular interest were group differences in the three domains between OM/OD and OM/NOD groups. Analyses demonstrated no significant group differences in SES/neighborhood factors. In the family context domain, there were group differences in the mother-daughter communication factors and in maternal mental health variables (i.e., Depression and PTSD). In contrast, family environment (shared family meals, dyadic decision making about food, rules about eating) and mother-daughter relationship factors (maternal warmth and hostility, relationship style) were not significantly different among Jennifer Ramirez – University of Connecticut, 2017 groups. In the individual weight behaviors and attitudes domain, there were significant group differences in the expected direction, but no group differences in mother or daughter exercise behavior. In the contrast of most interest (OM/OD versus OM/NOD), maternal mental health factors were the most robust distinguishing characteristic. In post hoc analyses, potential mediators of the link between maternal depression and adolescent overweight/obesity were explored. Overall, results suggest maternal mental health may increase the likelihood for obesity among adolescent girls at elevated risk because of maternal obesity. Factors such as the functional impairment related to a mother’s mental health (e.g., low motivation) or modeling of unhealthy coping strategies (e.g., emotional eating) could be mechanisms through which adolescents are at increased risk for obesity. Increased attention should be paid to family context of adolescent females as health care professionals develop and implement prevention and intervention efforts.
