In hypothyroidism originating in the thyroid gland (primary hypothyroidism), the serum concentration of THS is increased, and in hyperthyroidism it is respectively decreased (in most cases not measurable). In rare hypothyroidism originating in the pituitary gland (central hypothyroidism) the serum TSH concentration is within or below the reference range and free T4 (FT4) concentration is below the reference range. Central hyperthyroidism is very rare. In this condition, TSH concentration is within or above the reference range and FT4 concentration is increased. TSH concentration is interpreted differently in patients on thyroxine replacement therapy than in diagnostics of thyroid disorders: patients on replacement therapy usually feel the best when TSH concentration is approximately 1–2 mU/l and FT4 concentration is near the upper threshold of the reference range. Book excerpt: When the Medicare program was established in 1965, it was viewed as a form of financial protection for the elderly against catastrophic medical expenses, primarily those related to hospitalization for unexpected illnesses. The first expansions to the program increased the eligible population from the retired to the disabled and to persons receiving chronic renal dialysis. It was not until 1980 that an expansion of services beyond those required "for the diagnosis or treatment of illness or injury or to improve the functioning of a malformed body member" was included in Medicare. These services, known as preventive services, are intended either to prevent disease (by vaccination) or to detect disease (by diagnostic test) before the symptoms of illness appear. A Committee was formed "to conduct a study on the addition of coverage of routine thyroid screening using a thyroid stimulating hormone test as a preventive benefit provided to Medicare beneficiaries under Title XVIII of the Social Security Act for some or all Medicare beneficiaries."

It has been known for decades that overt hypothyroidism contributes to elevated serum cholesterol levels and cardiovascular risk and recent studies suggest this may also be true with subclinical hypothyroidism.

Preparations for the conference began 18 months earlier with the designation of a steering committee and the establishment of a dedicated, p- manent web site. The meeting took place on October 22 and 23, 2007 in Bethesda, Maryland and was co-moderated by Susan J. Mandel and Edmund S. Cibas. The discussions and conclusions regarding terminology and morphologic criteria 1, 2 from the meeting were summarized in publications by Baloch et al. and form the framework for this atlas. The atlas is organized by the general categories of “Nondiagnostic,” “Benign,” “Follicular Neoplasm/Suspicious for a Follicular Neoplasm”, “Suspicious for Malignancy,” and “Malignant,” and it includes the defi- tions and morphologic criteria of these categories as set forth by Baloch et al. The majority of the conference participants also agreed on a category of “undetermined significance,” which is incorporated in this atlas (Chap. 4). It is critical that the cytopathologist communicate thyroid FNA interpretations to the referring physician in terms that are succinct, unambiguous, and helpful clinically. We recognize that the terminology used here is a flexible framework that can be modified by individual laboratories to meet the needs of their providers and the patients they serve.
