Book excerpt: IMPORTANCE: Overweight/obesity, dyslipidemia, and elevated blood pressure (BP) are increasingly common among U.S. adolescents and increase risk for development of cardiovascular disease (CVD) in adulthood. These risk factors are highly influenced by diet and activity levels, both of which are modifiable with appropriate intervention. School-based programs may be an ideal setting for nutrition intervention programs to improve health behaviors, however data examining the effectiveness of such programs is limited, and results have been inconsistent depending on the outcomes examined. OBJECTIVE: Among a sample of adolescents in Michigan participating in Project Healthy Schools (PHS), a multi-component school-based intervention program, the objective of this dissertation is to 1) examine the effectiveness of the PHS program at achieving favorable change in participating students' dietary consumption of foods and/or beverages associated with CVD risk; 2) examine if achievement of optimal consumption of fruits and vegetables, sugar sweetened beverages (SSB) and levels of physical activity (PA) is associated with improvement in blood lipid and BP levels and; 3) determine if change in dietary consumption of foods high in saturated fatty acids (SFA) and trans-fatty acids (TFA) is associated with change in low density lipoprotein cholesterol (LDL-C) concentrations. DESIGN, SETTING, AND PARTICIPANTS: A non-randomized, quasi-experimental pre-post design evaluation of sixth grade students from 94 middle-schools across the state of Michigan enrolled in the first year of a school-based nutrition intervention program between 2005-2019. MEASURES: Measures of dietary intake and PA were collected from a validated health behavior questionnaire administered at baseline and following completion of the 10-week nutrition intervention program. Physiologic outcome measures of a non-fasting lipid profile which included total cholesterol [TC], high-density lipoprotein cholesterol [HDL-C], triglycerides [TG] and calculated low-density lipoprotein cholesterol (LDL-C), as well as systolic and diastolic BP measurements were collected. RESULTS: Intake of fruit and vegetables significantly increased post-intervention, as did consumption of sugary beverages. Students who were more physically active following the PHS intervention had significantly higher post-intervention HDL-C and lower TG levels, whereas students with low SSB intake post-intervention experienced lower post-intervention HDL-C levels compared to students with higher SSB intake. Reduced consumption of high-risk, high fat foods was associated with a significant decrease in mean LDL-C when compared with high consumption at baseline and follow-up. CONCLUSIONS AND FUTURE DIRECTIONS: Significant increases in dietary intake of fruit and vegetable consumption following participation in the intervention program were modest. Improvement in cardiometabolic risk factors were also observed. Students who improved their intake of foods high in saturated and trans-fat experienced the largest decreases in LDL-C post-intervention. The results also suggest that PA in this age group may be an effective way to improve HDL-C and TG levels, especially among high-risk students. Future work should examine whether modest changes in dietary consumption and/or PA levels can promote a meaningful shift in physiological measures of cardiovascular risk over a longer period of time.

The added demand placed on individuals, however, may be overwhelming. The impact of targeting multiple behaviors concurrently, as opposed to one individually has not been investigated. The current study evaluated the efficacy of a computer-based physical activity only or a combined physical activity and nutrition intervention with adolescents. The PACE+ program assessed adolescents' physical activity and dietary behaviors, provided tailored feedback, and was used to create individualized behavior change plans with identified settings for change, sources of social support, and relevant benefits of and barriers to change. Subjects (N = 138, age M = 12, 65% female, 28% Caucasian) were recruited from a middle school in San Diego and randomly assigned to the multibehavioral or single behavior intervention or the control condition. All conditions involved working on the computer. Primary outcomes were 5-day accelerometer and 3-day dietary record data. Assessments were made at baseline and 3-months follow-up. Behavior change was examined overall in a combined index and for the behaviors individually. The single behavior intervention exhibited greater overall change than the control condition (p = .011) and multibehavioral intervention (p = .091). Effect sizes were in the moderate range (d = 0.34 to 0.58). Repeated-measures analysis of variance (ANOVA) tests revealed a significant treatment effect for physical activity among boys but not girls. Boys in the intervention conditions increased or maintained their physical activity, while control subjects and girls in all conditions declined. Minimal change was achieved for fruit and vegetable consumption. Theoretical mediators (decisional balance, self-efficacy, processes of change, peer support) predicted change in behavior, but effects were independent of the intervention. The findings suggest greater efficacy for interventions that target a single behavior for change.

Educating the Student Body makes recommendations about approaches for strengthening and improving programs and policies for physical activity and physical education in the school environment. This report lays out a set of guiding principles to guide its work on these tasks. These included: recognizing the benefits of instilling life-long physical activity habits in children; the value of using systems thinking in improving physical activity and physical education in the school environment; the recognition of current disparities in opportunities and the need to achieve equity in physical activity and physical education; the importance of considering all types of school environments; the need to take into consideration the diversity of students as recommendations are developed. Book excerpt: Abstract: Being overweight or obese as a child further increases the risk for obesity later in life as well as enhances the risk for developing diseases, including metabolic syndrome. Effective education-based nutrition education programs are needed to increase nutrition knowledge to combat the rise in obesity and metabolic syndrome among adolescents. Thus, a pilot summer wellness course was offered to high school students over a 3-week period. The educational intervention focused on physical activity and nutrition education to promote lifelong healthy behavior modifications among adolescents. Three sections of 30 students completed classroom learning and application projects based on the USDA's Team Nutrition curricula. Eighty-two high school students (ages 13-18) consented to participate in the research. Changes in nutrition knowledge were assessed through an examination given before and after the 3-week pilot course. The questions were divided among five domains: nutrition guidelines, energy balance, general nutrition knowledge, lifestyle choice and health outcomes. Total nutrition knowledge significantly increased from pre-test to post-test, 63.4% to 78.8% respectively. Nutrition knowledge significantly increased in four of the five domains-nutrition guidelines, energy balance, general nutrition knowledge, and health outcomes. Students' overall nutritional knowledge increased significantly over the 3-week pilot course. A school-based nutrition education program is an effective approach to develop an understanding of healthy lifestyle behaviors in adolescents. Future research should assess the changes in behaviors resulting from such education.

On one hand, many children from low-income families are not getting enough to eat each day due to a lack of resources. As a result of this combination, many children today face an increased risk for under-nutrition, obesity, heart disease and other chronic diseases. Finally, educators and health professionals agree that poor diet, eating habits and lack of PA, also affect academic performance. The fact that a hungry child cannot learn has been documented in numerous studies. Indeed, this research has found that even moderate under-nutrition and an immense lack of PA can have lasting effects on children's ability to learn and school performance. Undernourished and untrained children tend to attain lower scores on standardised tests, are more likely to become sick, miss school, and to fall behind in class. Also, hungry children have low energy, are more irritable, and have difficulty concentrating, which interferes with learning. Therefore, school feeding and PA programs were established by several schools and public and private organizations to provide proper nourishment and the possibility of practicing PA. In addition it helps preventing the negative effects of hunger and malnutrition. The School Breakfast Programs was established as a pilot project in 1966 in response to the needs of children arriving at school without having eaten breakfast. Now permanent, breakfast programs help states provide daily breakfast to millions of students in thousands of schools. The positive impact of this program cannot be underestimated. Not only do many teachers report that students are more alert and perform better in class after eating a nutritious breakfast, but published studies also found that breakfast programs are associated with significant improvements in academic functioning among school children. PA programs can substantially improve children s ability to learn and their state of health by making PA a part of their daily lives. Interventions addressing childhood obesity are best effective in the school environment, primarily due to the immense time children spend in school. Multi-component interventions that focus on several aspects of obesity interventions, including nutrition and physical activity, have shown to be more successful in influencing the behaviors and health outcomes of children. Although key components of effective school-based interventions have been identified, the frequency, type and duration of interventions provided have been inconsistent and have led to equivocal results, limiting the reproducibility and future application of effective obesity prevention interventions. Additionally, little information is known about the comparative effectiveness of school-based interventions across different School Types, such as Public, Charter and Parochial/Religious schools. The purpose of this study was to identify how a multi-component school-based obesity intervention affected the health outcomes and behaviors of children. Methods: Students were recruited from schools of the Greater Philadelphia area for a three-year study. Thirteen schools were provided nutrition, physical activity, and wellness interventions implemented by community partners. Schools were designated as Core, Level 1, or Control Schools. Core Schools received weekly interventions; Level 1 Schools received monthly interventions; and Control Schools received no interventions. Anthropometric and health behavior assessments were collected in the Fall and Spring of each year. Food preference assessments were collected throughout the study intervention. Linear mixed modeling was used to assess the influence of Level of Intervention, Sex, School, and School Type on the anthropometric outcomes of the students. Chi-square tests of independence were utilized to assess the influence of the Level of Intervention on the students' likelihood of meeting national fruit, vegetable, and physical activity recommendations. Chi-square tests of independence were also used to assess the influence of the Level of Intervention on the students' exposure to, preference for, and willingness to try new fruits and vegetables again. Results: Six linear mixed models were built to describe the influence of the Level of Intervention, Sex, School, and School Type on either Body mass index (BMI) z-score or waist circumference. Level of Intervention was significant (p=0.037) in predicting BMI z-score in all students. School Type was a significant predictor of BMI z-score in students who were overweight and/or obese (p=0.039). Families and communities are also impacted by the overweight and obesity epidemic. Current estimates of children who are overweight in the United States are 17% of children ages 6 to 11 and 17.6% of children ages 12 to 19. Although overweight and obesity rates are high among the general population; rates are even higher among minority and low-income populations. Schools have been researched for their effectiveness in reducing overweight and obesity prevalence; along with increasing behaviors such as healthier nutrition and physical activity and knowledge. However, few studies exist that examine the impact of school-based interventions on primarily low-income, minority populations. The purpose of this study was to evaluate the impact of TODAY (Transforming Obesity and Diabetes Awareness in Youth), a school-based intervention with the goal of preventing obesity and diabetes in the fifth grade population at two low-income, primarily Hispanic/Latino elementary schools in Escondido, California. The study examined nutrition, physical activity and health knowledge variables. A sample of 123 children, all 5th grade students who attended either Felicita or Lincoln elementary schools were recruited to participate in the project. The changes in nutrition and physical activity variables were measured by questions adapted from the School Physical Activity and Nutrition Questionnaire (SPAN) created by Coordinated Approach to Child Health (CATCH). The knowledge variable was measured by questions based on the health curriculum that pertained to nutrition and physical activity. It was hypothesized that children who participated in the project would demonstrate increases in healthy nutrition behavior such as fruit and vegetable consumption and decrease soft-drink consumption; demonstrate increase in physical activity behavior and decrease sedentary behavior of screen time; lastly the students would increase knowledge of nutrition and physical activity concepts taught in the intervention. Statistical analyses included Wilcoxon rank sum test and Wilcoxon signed rank test. Results from the pre/post test indicated that the children who participated in the TODAY Project, increased fruit and vegetable consumption, physical activity and health knowledge. Findings of this research are consistent with previously published articles on school-based interventions. However, unlike previous research this study was unable to demonstrate significant decreases in soda consumption and sedentary behavior. Understanding the impact of the school on the health of its students will allow for effective school based interventions that are capable of curving the overweight and obesity epidemic, especially among the most at-risk populations.
