Le traumatisme psychique correspond à un évènement de vie qui se caractérise par son intensité, l'incapacité dans laquelle se trouve le sujet d'y répondre de façon adéquate, le bouleversement et les effets pathogènes durables qu'il provoque dans l'organisation psychique. Si les psycho traumatismes chez l'enfant ont été longtemps minimisés, ils sont aujourd'hui bien individualisés. La question de leur exploration, par contre, est assez récente. Les recherches portant sur le traumatisme et ses conséquences se heurtent à de grandes difficultés de définition de leurs champs et il est particulièrement difficile de préciser les mécanismes incriminés. Ces aspects sont développés dans la première partie de cette thèse. Dans la deuxième parie de ce travail, ont été étudiées les propriétés psychométriques d'un outil autoadministré spécifique du dépistage des Etats de Stress Post Traumatique (ESPT) chez les enfants et les adolescents : la version française du Child Post Traumatic Symptom Reaction Index (CPTS-RI). Ses propriétés psychométriques ainsi que la rapidité et la facilité avec lesquelles il est administré et coté en font un instrument tout à fait utilisable et intéressant, tant pour les cliniciens que les chercheurs, pour évaluer les ESPT chez les enfants et les adolescents. En plus de son aspect diagnostique, il permet de quantifier l'intensité de la symptomatologie. Il n'avait auparavant jamais été validé dans sa version française. Dans une troisième partie, sont étudiées les associations entre des évènements de vie, les préoccupations et les tentatives de suicide à l'adolescence. Il est montré que le nombre et la nature des évènements de vie ont un impact considérable sur le risque de passage à l'acte suicidaire. La survenue de ces événements de vie modifie les préoccupations des adolescents. Enfin certaines de ces préoccupations sont aussi associées à une majoration du risque de passage à l'acte suicidaire.

Nous avons fait l'hypothèse que les tentatives de suicide de ces adolescents pourraient être l'expression de la transmission du traumatisme migratoire familial, ou être liées à leur situation de vie transculturelle. Nous nous sommes appuyés sur les théories issues de la psychiatrie transcultureIle, et en particulier sur les notions de traumatisme migratoire, d'enveloppes psychiques, de contenants culturels, et de continuité culturelle. Nous avons confronté nos hypothèses à une revue de la littérature internationale, et à l'étude de cinq cas cliniques approfondis. Les liens entre culture et suicide sont analysés dans de nombreux travaux, et mettent en évidence la variabilité des taux, des attitudes et des croyances envers les comportements suicidaires. La migration est ici considérée comme un processus psychique, nécessitant un travail d'élaboration, qui s'apparente au travail de deuil. En fragilisant les enveloppes culturelles, la migration peut favoriser la constitution d'un traumatisme psychique. L'adolescent migrant va se construire sur une logique de clivages, ce qui favorise sa rencontre traumatique avec le monde. L'adolescence est période de vulnérabilité, car elle nécessite de profonds remaniements identitaires. La transmission du traumatisme migratoire peut mettre en difficulté le processus de subjectivation adolescent, ce qui favorise des passages à l'acte, comme les tentatives de suicide. Cette vulnérabilité familiale est pondérée par des évènements pré-, per et post-migratoires. La continuité culturelle semble être un facteur protecteur du risque suicidaire chez l'adolescent. Le travail thérapeutique doit donc favoriser la restauration de la fonction de contenance du groupe familial, dans un cadre adapté à la temporalité de la migration. Ce travail thérapeutique permettrait à l'adolescent de l'accompagner vers une identité adulte métissée.

This review found that these groups are exposed to risk factors for mental disorders before, during, and after migration. The prevalence of psychotic, mood, and substance-use disorders in these groups varies but overall resembles that in the host populations. Refugees and asylum seekers, however, have higher rates of post-traumatic stress disorder. Poor socioeconomic conditions are associated with increased rates of depression five years after resettlement. Refugees, asylum seekers, and irregular migrants encounter barriers to accessing mental health care. Good practice for mental health care includes promoting social integration, developing outreach services, coordinating health care, providing information on entitlements and available services, and training professionals to work with these groups. These actions require resources and organizational flexibility.
