Book excerpt: Des études menées en 2006 et 2008 par la DRASS dans les cabinets dentaires en milieu carcéral relevaient que les porte-instruments rotatifs (PIR) ne faisaient pas l'objet d'une stérilisation régulière entre chaque patient. Dès lors l' INVS fut chargé de publier un rapport évaluant le risque de transmission croisée d'agents pathogènes dans les cabinets dentaires français. L'INVS modélisa ce risque en tenant compte du nombre de patients séropositifs au VIH,VHC et VHB en France. Dans son rapport publié en juillet 2009 l' INVS fait état de moins d'un cas de VIH, moins de 2 cas de VHC, et prés de 200 cas de VHB transmis par an en France. Devant ce risque non nul et en l'absence de consensus entre les autorités de Santé, les experts et les fabricants existe-t-il des méthodes de désinfection et stérilisation des PIR qui soient en accord avec ces nouvelles exigences en matière d'asepsie et compatibles avec l'exercice de la Chirurgie dentaire moderne? Les particularités techniques des PIR en font un dispositif médical compliqué et difficilement démontable en particulier pour ceux offrant de hautes vitesses de rotation. Étant donné que l'on ne stérilise que ce qui est propre, un des enjeux majeurs est le nettoyage des composants internes des PIR grâce à des automates et autres thermo-laveurs avant leur stérilisation par autoclave de type B seul capable de stériliser des corps creux. Plusieurs méthodes restent possibles pour permettre aux PIR de subir l'ensemble des étapes de la chaine de stérilisation, mais les temps de traitement ainsi que l'investissement financier à réaliser diffèrent, c'est pourquoi chaque praticien doit faire le choix d'une méthode en accord avec le niveau d'invasivité des actes réalisés ainsi que son rythme de travail et du matériel dont il dispose.
