Il est nécessaire de repérer de façon précoce les sujets âgés à risque d'hospitalisation prolongée afin de limiter les effets secondaires. Une Evaluation Gériatrique Standardisée courte (EGS-C) en 6 items a été utilisé dès les urgences. L'objectif était d'évaluer le caractère prédictif de l'EGS-C à 6-items pour la durée d'hospitalisation chez les patients en court séjour et la contribution de chaque item dans la prédiction de la durée d'hospitalisation. Les résultats obtenus avec l'EGS-C à 6 items étaient faible, le nombre items a donc été augmenté à 10. 993 patients de plus de 80 ans ont été inclus dans une étude prospective monocentrique. L'EGS-C à 10 items comprenait l'âge 85ans, le sexe masculin, la prise de 5 traitements et plus quotidiennement, utilisation de psychotropes, antécédents de chute dans les 6mois, désorientation temporelle, pathologies aigues (insuffisance d'organe aigue et motif d'admission) et les variables environnementales (lieu de vie et utilisation d'aide à domiciles). Le modèle des réseaux neuronaux a été appliqué pour les 10 items, avec les perceptrons multicouches (MLP) et les MLP modifiés. Les performances prédictives étaient: sensibilité (62.7%), spécificité (96.6%), VPP (87.0%), VPN (87.5%) et aire sous la courbe ROC (90.5) selon MLP modifiés. L'état de santé contribuait à 51.3%, les items démographiques à 21.6% à la durée d'hospitalisation prolongée selon MLP modifiés. Les performances de l'EGS-C à 10 items étaient supérieures mais demeuraient insuffisantes pour constituer un outil de décision.

Objectif: Evaluer les performances de l'évaluation gériatrique standardisée courte (EGS-c) à 10 items dans la prédiction des hospitalisations prolongées à partir des urgences. Méthode: Cette étude de cohorte observationnelle et prospective a inclus 2370 patients de 70 ans et plus, hospitalisés dans l'Unité d'Hospitalisation de Courte Durée (UHCD) du CHU d'Angers entre le 1er janvier 2012 et le 31 décembre 2013. L' EGS-c 10-items mesurait les variables suivantes: âge ≥ 85 ans, sexe masculin, consommation d'au moins 5 médicaments par jour, de psychotrope, antécédent de chute, désorientation temporelle, présence d'aides formelles et/ou informelles, le lieu de vie, le motif principal d'hospitalisation. Le critère de jugement principal était la durée d'hospitalisation, considérée comme prolongée après 13 jours. Deux types de RNA ont été utilisés (multilayer perceptron (MLP) et MLP modifié). Une stratification selon l'âge a été réalisée. Résultats: Les performances de prédiction les plus élevées étaient obtenues par le réseau MLP modifié : sensibilité=88-89% ; spécificité=96- 97,6% ; valeur prédictive positive=87,7-92,2% ; valeur prédictive négative 96,2-96,5% ; rapport de vraisemblance positif=22,3-37,6; l'aire sous la courbe=93,8-95,5. Conclusion : La stratification selon l'âge permettait d'améliorer les capacités de l'EGS-c à 10 items dans la prédiction des hospitalisations prolongées. Les performances de cet instrument de repérage étaient compatibles avec celles d'un outil de décision clinique.
