Book excerpt: Contexte : La plupart des études publiées sur l'atteinte de la communication après une lésion cérébrale ont porté sur les troubles de l'hémisphère majeur, notamment chez les patients aphasiques, et peu de travaux se sont penchés sur l'hémisphère mineur et ses conséquences. But du travail : L'objectif de cette étude est de documenter l'efficacité de communication chez les patients qui présentent des lésions cérébrales droites (hémisphère mineur chez le droitier) par rapport à ceux qui présentent des lésions gauches (hémisphère majeur chez le droitier). Nous essayerons, également, d'en déterminer les conséquences sur la qualité de vie de ces patients. Méthode : Nous avons comparé 3 groupes de patients (cérébro-lésés droit, cérébro-lésés gauche et un groupe de sujets sains). Les patients avec lésions cérébrales ont été vus en entretien semi-dirigé à leur domicile et à plus d'un an de leur ictus. L'efficacité de la communication a été évaluée avec l'échelle de communication verbale de Bordeaux. Pour la qualité de vie et bien être, nous avons utilisé les questionnaires de Branholm-Fulg Meyer, SIP-65 et ADRS. Résultats : L'efficacité globale de la communication, semble être la même chez les patients lésés cérébraux gauche et droit. La qualité de vie des patients lésés cérébraux gauche semble être plus altérée que chez les patients lésés droit. L'incidence de la dépression est semblable dans les deux groupes. L'état de l'autonomie agit sur l'état psychologique et l'efficacité de la communication. Un état dépressif semble modifier l'efficacité de la communication et la dimension relationnelle de la qualité de vie. Conclusion : Cette étude nous permet d'avoir un aperçu de la communication et qualité de vie, au quotidien, des patients cérébro-lésés. Nous observons aussi certaines spécificités qui dépendent de la localisation de la lésion (par exemple : les patients cérébro-lésés droit plus en difficultés pour les "achats").La connaissance de ces particularités doit contribuer à une meilleure prise en charge de ces patients.
