Ces évènements ont plusieurs causes, la polymédication, les prescriptions inappropriées, l'altération des fonctions physiologiques. Les objectifs de cette étude sont d'évaluer l'impact d'une hospitalisation dans un service de psychiatrie de la personne âgée sur le risque potentiel d'évènement indésirable médicamenteux chez le sujet âgé de 75 ans et plus et d'analyser les facteurs de risque de prescriptions inappropriées. Il s'agit d'une étude rétrospective multicentrique. Le risque potentiel d'évènement indésirable est assimilé aux prescriptions médicamenteuses inappropriées, évaluées à l'aide de deux outils, les critères de Beers et la liste française de Laroche et al. L'impact de l'hospitalisation est mesuré en comparant les taux d'inappropriation du traitement avant l'hospitalisation à ceux de la sortie. Deux cent quatre-vingt dix patients, d'âge moyen 82 + ou - 5 ans ont été inclus dans quatre centres d'investigation. Une augmentation significative du nombre moyen de médicaments prescrits par patient entre l'admission et la sortie d'hospitalisation est mise en évidence (6,7 versus 7,3, p0.0001). Selon les critères de Beers le taux d'inappropriation est respectivement de 32 % à l'admission et de 31 % à la sortie (p0,05). La liste française donne des taux d'inappropriation plus élevés, respectivement 71 % à l'admission et 73 % à la sortie (p>0,05). Aucun des deux outils ne montrent de différence significative entre les taux d'inappropriation à l'admission et à la sortie. Selon les critères de Beers, un quart des inappropriations à l'admission et un tiers à la sortie concernent des prescriptions inappropriées d'hydroxyzine. Selon la liste française, les inappropriations à l'admission et à la sortie concernent pour plus de la moitié (respectivement 59 % et 51 %) des prescriptions inappropriées de benzodiazépines et apparentés. Les autres inappropriations sont représentées par les prescriptions inappropriées de médicaments anticholinergiques (21 % à l'admission et 29 % à la sortie), et de neuroleptiques (10 % à l'admission et 11 % à la sortie). Le nombre de médicaments prescrits est mis en évidence comme un facteur de risques significatif de prescriptions inappropriées. L'hospitalisation dans un service de psycho-gériatrie n'a pas d'impact sur les prescriptions inappropriées du sujet âgé. Le nombre moyen de médicaments par patient est plus important après l'hospitalisation. Les taux d'inappropriation mis en évidence avec les critères de Beers ne diffèrent pas de ceux retrouvés dans la littérature. Concernant les taux obtenus avec la liste française de Laroche et al., il est plus difficile de les comparer. Les études publiées utilisant un outil proche de cette liste ont été faites avec des patients de milieu gériatrique, qui sont difficilement comparables aux patients de notre cohorte. Cette étude apporte des données sur la prescription médicamenteuse du sujet âgé pris en charge de psychiatrie. Ce travail pourrait permettre d'élaborer des outils et des protocoles pour faciliter l'évaluation du rapport bénéfice/risque de la prescription médicamenteuse du sujet âgé souffrant de troubles psychiatriques.

Book excerpt: It is with great pleasure that I write this Foreword to the Proceedings of the International Conference on Behçet’s Disease which was held in Berlin in June 2002. This was the first International Conference held under the auspices of the International Society for Behçet’s Disease which was founded in 2000 in Seoul. First, I congratulate our colleagues in Berlin, led by Professor Christos Zouboulis of the Department of Dermatology at the Free University of Berlin, for having organised a most successful conference and for having compiled these proceedings so rapidly. It will be realised immediately on scanning the contents of this book that the conference was truly international with 210 participants from 26 countries, as Professor Zouboulis has noted in his preface. These included basic scientists, epidemiologists, pathologists, clinicians and, importantly, representatives from patient organisations. The latter held their own conference alongside the scientific-medical conference to mutual benefit. The combined session of patients and doctors (abstracts on pp 601 – 626) gave the opportunity for an exchange of information and fruitful discussion. The wide ranging scope of the communications is evident from the index and it was most encouraging to see their origin – from all parts of the world, from senior and junior colleagues and, from many different disciplines. Many communications may be regarded as preliminary reports of research in progress and we look forward to seeing the definitive publications in appropriate journals in due course.
