Leur incidence, modalités de prise en charge et coûts engendrés sont mal connus. Patients et Méthode: Etude prospective observationnelle incluant les hypoglycémies sévères après appel au centre 15 de Nice pour la période de mai 2013 à avril 2014. Résultats: Population de 33 875 patients diabétiques, 195 hypoglycémies sévères soit 0,58 hypoglycémies sévères/100 patients/an. Caractéristiques des patients : âge moyen: 61,8ans ± 21,4 ; homme: 58% ; ancienneté du diabète >10ans : (67,5%) ; antécédent d'hypoglycémie sévère au cours des douze derniers mois: 57,9%. Traitements impliqués : insulinothérapie 88,3% dont 10,2% combinée avec un antidiabétique oral. Le repaglinide retrouvé dans 41,2% (âge moyen : 81,1 ans ± 21,4), et le glibenclamide 29,4% (âge moyen : 77,3 ans ± 21,4). Circonstances : 82,1% au domicile; 24,6% nocturnes. Avant l'arrivée des secours : glycémie relevée pour 29,2%. Moyenne à leur arrivée : 0,42 g/l ± 0.29. Resucrage: per os 44,1%, intraveineux perfusion 42% et injection directe 34,9%. 14,9% étaient hospitalisés et 22% transférés aux urgences. Le coût total était de 352 138€. Conclusion: Cette étude permet de caractériser les hypoglycémies sévères, la prise en charge ainsi que le coût, extrapolé au niveau national, estimé à 35 millions d'euros (0,2% du coût du diabète). La mise en place de protocoles de soins, de mesures éducatives et des réévaluations thérapeutiques sont à entreprendre pour réduire ce risque.
