Book excerpt: Face à l'émergence des résistances bactériennes et a ses multiples conséquences en termes de santé publique et d'économie, des mesures nationales ont été mises en place pour promouvoir le bon usage des antibiotiques (ATB). Le plan national d'alerte sur les ATB 2011-2016 annoncé par le ministère de la santé vise à rationaliser les prescriptions d'ATB et ainsi diminuer les résistances. Le but de ce travail a été d'étudier la réévaluation de l'antibiothérapie. L'étude a été réalisée dans deux centres hospitaliers bas-normands : Falaise et Bayeux sous forme d'évaluations des pratiques professionnelles (EPP) réalisées en 2013-14. La réévaluation de l'antibiothérapie entre 48 et 72 heures ainsi que la justification de toute antibiothérapie supérieure à 7 jours ont été étudiées. Le recueil des données s'est fait selon deux modalités : recherche des critères objectifs de non-réévaluation (selon la CORA) et recherche des critères objectifs de réévaluation et de leur traçabilité (selon la SPILF). Sur les 3 EPP menées, 111 dossiers ont été analysés : 5.4% des dossiers comportaient des critères de non-réévaluation, la traçabilité était retrouvée dans 68% des dossiers. Les résultats de cette étude ont montré une amélioration des pratiques (et ce depuis 2010) dans ces deux CH concernant la traçabilité dans le dossier-patient et la diminution des critères de non-réévaluation. Néanmoins, ces efforts doivent être poursuivis, notamment sur la documentation microbiologique des infections, la justification des antibiothérapies de plus de 7 jours ainsi que l'informatisation du dossier médical. Le rôle fondamental de la commission des anti-infectieux et de son référent est également à souligner.

Plusieurs questions se sont alors imposées à nous : l'EPP est- elle enseignée au cours de la maîtrise de stage ? Sa mise en application dans la pratique courante est- elle réelle ? Les maîtres de stage ont-ils des besoins de formation dans ce domaine ? Après un rappel sur les aspects réglementaires (législation), les structures institutionnelles (HAS- URML- CNFMC- CNODM- OA ...), les différentes modalités et les outils utilisés dans l'EPP, un questionnaire a été proposé à cent (100) médecins généralistes maîtres de stage de l'UFR de Marseille exerçant dans le secteur libéral. 33 médecins y ont répondu. Plus de la moitié d'entre eux ne délivre pas cet enseignement à leurs stagiaires et une large majorité exprime le besoin d'être formé à cette démarche. L'enseignement de l ‘EPP devrait devenir plus systématique avec la mise en place d'une formation spécifique pour les maîtres de stage.
