Book excerpt: Contexte : Depuis les années 2000, le développement des résistances bactériennes est devenu un problème majeur de santé publique du fait d'une utilisation déraisonnable des antibiotiques et ce malgré la mise en place au niveau national et international de plusieurs plans d'action. L'instauration d'une antibiothérapie adaptée a un impact positif sur la survie du patient et un impact économique non négligeable. L'objectif de ce travail était donc d'évaluer les prescriptions antibiotiques aux urgences du Centre Hospitalier de Valenciennes selon la méthode de Gyssens. Méthode : Il s'agissait d'une étude monocentrique, descriptive, analytique et prospective. Les 100 patients consécutifs adultes, pour lesquels une antibiothérapie a été instaurée dans le cadre d'une hospitalisation, ont été inclus entre le 22 septembre 2014 et le 5 octobre 2014. Résultats : Sur les 100 patients, composés de 56% d'hommes et 44% de femmes avec un âge médian de 62 ans, 34% présentaient une infection respiratoire. Les autres pathologies infectieuses étaient à 17% d'origine digestive, 16% d'origine urinaire, 14% d'origine cutanée et plus rarement d'origine stomato-ORL ou neurologique. Les pénicillines étaient les plus prescrites (40% environ) avec les céphalosporines (seules dans 13% des cas ou en association dans 27% des cas) et les quinolones (8%). 70,7% des prescriptions étaient classées Gyssens I, c'est-à-dire conformes aux recommandations actuelles. La non-conformité était essentiellement expliquée par l'existence d'une alternative plus efficace (16,1% classe IV), d'une posologie inadaptée (3% classe IIa), d'une voie d'administration incorrecte (3% classe IIc), et d'une absence d'indication d'antibiothérapie (3% classe V). Les délais médians de prescription et d'administration étaient de 3,7 heures et 3,9 heures. Ils étaient significativement allongés si le prescripteur était un junior, inchangés en période de garde, lorsqu'un avis spécialisé était demandé ou que le patient présentait des signes de gravité clinique. Avant prescription, seulement 53% des patients avaient un prélèvement bactériologique. Conclusion : Les résultats de notre étude ont révélé un taux de conformité relativement satisfaisant comparativement aux données de la littérature. Cependant, afin de préserver les molécules actuelles, il est important d'améliorer les pratiques par l'élaboration d'un référentiel local ou de protocoles, par le biais de groupes de travail en concertation avec l'équipe des urgences et l'équipe d'infectiologie du CH de Valenciennes.
