Plusieurs études rapportent que l'inadéquation de l'antibiothérapie probabiliste a un impact sur la morbi-mortalité. Objectif: Il s'agissait d'évaluer le taux d'inadéquation de l'antibiothérapie probabiliste dans les états septiques graves et de définir les facteurs prédictifs de cet échec. Patients et méthodes: Etude monocentrique, observationnelle incluant les patients présentant un état septique grave documenté au plan microbiologique sur une période de 21 mois (janvier 2013 à septembre 2014). Résultats: 162 états septiques graves ont été inclus . Le taux d'inadéquation de l'antibiothérapie probabiliste était de 24%, malgré un taux de conformité aux recommandations d'experts de la SFAR de 80%. Les facteurs prédictifs d'inadéquation en analyse multivariée étaient, l'absence de traitement par aminoside OR 3.91[1.11-13.5] , la présence d'une flore polymicrobienne OR 4.1[1.39-11.9], la présence de levures OR 25 [7.75-250], et la présence de BMR OR 21 [5-83]. L'inadéquation de l'antibiothérapie probabiliste était associée à une augmentation de la durée d'hospitalisation, de la durée de l'utilisation des amines et de la ventilation mécanique. Par ailleurs, la prévalence des BMR était faible ( 8%). Conclusion: Dans notre série, la prescription de l 'antibiothérapie probabiliste est en majorité conforme aux recommandations de 2004 de la SFAR. Cependant, le taux d'inadéquation de l'antibiothérapie aux germes est de 25 %. La mise en place d'un protocole concernant l'ensemble des thérapeutiques anti-infectieuses adaptées à l'écologie locale, pourrait améliorer l'adéquation des prescriptions et participer à l'amélioration de la prise en charge des états septiques graves au sein du service
