Simultanément, la Haute autorité de santé a publié un guide sur le bilan de médication en entrée et sortie d’hôpital, permettant d’assurer la continuité pharmaceutique et le lien ville-hôpital donc impliquant une identité d’exercice en PUI et en officine. Cet ouvrage est le premier à traiter de la place de la pharmacie clinique à l’officine. Il doit constituer une référence et un outil de travail pour le pharmacien officinal mais aussi, pour tout étudiant choisissant cette filière. La rédaction de chaque chapitre a été confiée à des bi/trinôme voire plus : pharmaciens hospitalo-universitaires, pharmaciens officinaux, autres acteurs expérimentés. Les différents chapitres sont traités de façon pratique, avec un encart systématique sur « l’essentiel à retenir ». L’ouvrage se décline en trois parties. La première concerne le contexte de la relation patient/pharmacien : le parcours de santé, l’assurance qualité à l’officine, la sécurité du patient, le dossier pharmaceutique, les bases de données, l’interprétation des informations et la médico-économie. La deuxième s’intéresse à l’exercice de la pharmacie clinique à l’officine selon les trois volets suivants : 1° la stratégie thérapeutique, les premiers soins et le conseil à l’officine ; 2° l’optimisation du traitement portant sur les posologies, les plans et voies d’administration, la prévention de l’iatropathologie et des effets indésirables ; 3° le suivi du patient avec en particulier, le rôle des pharmaciens dans le bilan de médication et le lien ville-hôpital, les entretiens pharmaceutiques et l’éducation thérapeutique. Enfin, la troisième partie aborde le rôle du pharmacien hospitalier et d’officine dans le suivi des patients en oncologie et notamment les soins de support, l’objectif 17 du Plan Cancer 2014-2019 préconise en effet d’adapter les organisations à l’essor des chimiothérapies orales. Un chapitre est consacré à la vaccination, compte tenu du décret n° 2017-985 du 10 mai 2017 relatif à l’expérimentation de l’administration par les pharmaciens du vaccin contre la grippe saisonnière, et de l’extension transitoire de l’obligation vaccinale pour 11 vaccins. Deux autres chapitres sont respectivement dédiés aux dispositifs médicaux et aux pansements essentiels à connaître pour l’aide aux patients hospitalisés à domicile. La pharmacie clinique installe chaque jour davantage le pharmacien au sein des professionnels de santé. La profession ne doit pas manquer ce virage.
