L'objectif était de montrer l'état actuel des pratiques des urgentistes à ce sujet chez les adultes non communicants dans les urgences publiques du Languedoc-Roussillon. Matériels el Méthodes : Une étudde d'enquête de pratique. descriptive, a été conduite de novembre 2012 a février 2013. Un questionnaire anonymisé a été envoyé dans chaque service pour distribution aux médecins urgentistes thésés exerçant dans les Services d'Accueil des Urgences publiques du Languedoc-Roussillon. Les questions portaient sur l'évaluation, la prise en charge et sur la formation reçue sur la douleur aigüe de l'adulte non communicant. Les données ont été traitées par une méthode adaptée aux sondages (intervalle de confiance de 90%). Résultats: 111 questionnaires ont été reçus et analysés sur un total de 225 (taux de participation de 49%). 84% des urgentistes évaluaient la douleur mais seuls 49% utilisaient une échelle d'hétéro-évaluation. 30% des médecins utilisaient la morphine en premier ou deuxième lieu et 21 % avaient des limites à l' ulilisation de la morphine. Ces limites avaient tendance à être plus importantes en CHU (37,04% [21,26-52,82]) qu'en CHG ( 13.10% [4.15·22.04]). Conclusion : La douleur aigüe nociceptive de l'adulte non communicant aux urgences est sous évaluée du fait d'une probable sous-utilisation des échelles d'hétéro-évaluation. La morphine est peu utilisée en première ou deuxième intention, avec des freins retrouvés principalement au CHU. Ces résultats soulignent les progrès à entreprendre dans ce domaine.

Book excerpt: Introduction La douleur aiguë étant la principale plainte lors des consultations au Service d'Accueil des Urgences, elle représente un enjeu de santé publique concernant sa prise en charge précoce. Cette étude a pour but d'évaluer la mise en place d'un protocole permettant une administration précoce d'antalgique(s) dès l'IOA aux urgences de CHU de Reims. Méthode C'est une étude observationnelle, rétrospective et monocentrique conduite au service d'accueil des urgences de Reims. Il s'agit d'une évaluation des pratiques dont les critères de jugements sont : proportion de patient bénéficiant d'une antalgie précoce, délai de mise en place d'un antalgique, proportion de réévaluation de la douleur, temps de passage aux urgences. Résultats Au total, 863 patients ont été inclus (496 en 2020, 367 en 2021) sans différence significative sur les caractéristiques initiales entre les 2 années. Sur la population de patients de 2021, une augmentation significative de l'antalgie dès l'IOA (30.79% vs 8.47%) a été observée. Sur les deux périodes, on observait une antalgie précoce d'autant plus importante pour les patients présentant une douleur modérée (26.40%) à sévère (43.10%), ceux orientés vers le circuit court (38.52%) et par conséquent les patients qui présentaient une douleur d'origine traumatique (30.89%) et/ou musculosquelettique (31.67%). Concernant la réévaluation de la douleur celle-ci restait faible en 2021 (23.98%). On observait une réévaluation plus importante pour les douleurs modérées à sévères (49.66%). Au moment de la réévaluation de la douleur, l'intensité était alors très fréquemment légère (76,1%). Le délai moyen avant mise en place d'une antalgie est significativement diminué en 2021 (0.57h vs 1,45h, p=0.003) : notamment pour les patients ayant reçu une antalgie dès l'IOA (0.20h), ceux orientés vers le circuit court avec une douleur d'origine traumatique (0.32h) et/ou musculosquelettique (0.33h) et ceux présentant une douleur modérée (0.52h) à sévère (0.46h). Cependant le délai restait significativement plus long pour les douleurs abdominales (1.08h). Le temps de passage aux urgences est significativement moins long pour les patients recevant une antalgie dès l'IOA (4.21h vs 2.65h, p=0.002 en 2021). Conclusion Le traitement adapté et précoce de la douleur est un enjeu de santé publique afin de lutter contre l'oligoanalgésie ; mais est également utile afin d'augmenter l'efficience de la prise en charge aux urgences (notamment en diminuant le temps de passage). L'utilisation d'un protocole de service est un moyen d'améliorer cette prise en charge.
