Book excerpt: L'objet de cette étude ex-vivo est de tester un nouvel instrument Prototype Elliptique, créé pour l'instrumentation des canaux ovalaires, sur le plan de l'évacuation de débris et boues dentinaires. Cette instrumentation prototype mécanisée, présentant un mouvement vibratoire, a été comparée à une instrumentation rotative connue, le système Revo'S. 20 dents mono-radiculées, saines, fraichement extraites, comportant un canal unique présentant un ratio longueur vestibulo-linguale / longueur mésio-distale supérieur à 2 on été sélectionnées et réparties aléatoirement dans deux groupes. Après mise en forme, les dents ont été sectionnées longitudinalement en deux parties pour permettre d'observer la totalité du canal, de la chambre pulpaire à l'apex. Trois clichés par dent sont pris au Microscope Electronique à Balayage (tiers coronaire, moyen et apical) avec un grandissement de x500. Les soixante clichés sont randomisés et évalués par trois praticiens (deux endodontistes et un praticien généraliste), calibrés au préalable, pour évaluation de la propreté des surfaces canalaires. L'analyse des résultats est réalisées par des tests statistiques non paramétriques et nous a permis de mettre en évidence : - Un score de nettoyage du Revo'S très satisfaisant à tous les tiers de la dent - Un score de nettoyage de l'instrument prototype elliptique significativement moins bon que le Revo'S - Pour les deux instruments le nettoyage est homogène, pas de différence significative entre les tiers, toutefois les écarts types sont moins élevés pour l'instrument prototype montrant que le score nettoyage avec cet instrument est plus prévisible. Dans les conditions de notre étude l'instrument prototype semble prometteur mais nécessite une amélioration de son efficacité de remontée des débris, en particulier au niveau du tiers apical.
