Les premières simulations du déplacement dentaire orthodontique par la méthode des éléments finis datent de la fin des années soixante dix. Depuis l'évolution de l'informatique a pemris de concevoir des modélisations dentaires approchant de plus en plus la réalité clinique. Les forces appliquées entrainent des contraintes (tension et pressions) aux interfaces desmodonte / os alvéolaire et dent / desmodonte. L'intensité et la localisation de ces contraintes permettent de prédire le déplacement dentaire immédiat. Les études portent sur différents types de mouvement (version, translation, ingression et égression) ainsi que sur certains appareils orthodontiques (forces extra-orales, multi-attaches, dispositifs de rétraction des dents antérieures, traction sur minivis d'ancrage...).

Book excerpt: Objectif: L'objet de cette revue systématique et méta-analyse est d'évaluer l'effet de plusieurs interventions pour accélérer le mouvement dentaire en complément d'un traitement orthodontique. Contexte: Depuis plusieurs décennies, plusieurs études ont été conduites pour évaluer l'efficacité des nouvelles techniques d'accélération du mouvement dentaire en orthodontie, malgré cela il n'y a pas de recommandation disponible à ce jour. Matériel et méthode: Une recherche électronique a été réalisée à l'aide de PubMed, Cochrane, Embase et Web of Science du 12 février 2021 au 13 mars 2021. Uniquement des études contrôlées randomisées et des essais cliniques contrôlées ont été sélectionnées. Leurs objectifs principaux sont d'évaluer l'effet des interventions pour accélérer le mouvement dentaire sur des personnes en bonne santé. La recherche bibliographique et la sélection ont été réalisées par deux auteurs (BP et EC) indépendamment. Résultats: Quarante-quatre études évaluant la photobiomodulation, la microvibration, la micro-pstéoperforation(MOP),la corticomie, la piezocision et le concentré plaquettaire riche en fibrine (PRF) ont été incluses dans l'analyse qualitative. Une méta analyse a été réalisée pour 3 méthodes: la photobiomodulation, la microvibration et la micro-osteoperforation. Elles ont montré une augmentation significative de la vitesse de déplacement dentaire pour la micro-ostéoperforation mais pas pour la photobiomodulation ni pour la microvibration. Les six études incluses concernant la piezocision relèvent toutes une augmentation significative de la vitesse de déplacement dentaire, concernant la corticotomie, trois études sur quatre montrent une augmentation significative. Quant au PRF la revue systématique ne montre pas d'augmentation significative. Conclusion: Sur la base des informations actuellement disponible, de faibles preuves montrent que la micro-ostéoperforation, la piezocision et la corticotomie augmentent la vitesse de déplacement dentaire, en revanche la photobiomodulation, la microvibration et le PRF ne semble pas l'augmenter. D'autres essais contrôlés randomisés devront être réalisé pour déterminer l'effet sur le long terme ainsi qu'un protocole optimal pour ces interventions.

The report makes extensive use of the Iraq Household Socio-Economic Survey, the first nationwide income and expenditure survey since 1988. IHSES data is complemented income and expenditure data from a wide range of other measures of living standards, allowing us to analyze living standards in a holistic way. The analysis presented here was performed with two main goals first, to inform the Government s Poverty Reduction Strategy; and second, to serve as a baseline for future assessments of changes in living standards and the identification of critical issues for deeper examination. Iraqi living standards have two unusual characteristics. First, they have fallen over the past generation. Second, they feature surprisingly little inequality. These characteristics are both rooted in Iraq s recent history of authoritarian government, war, military occupation, insurgency, and civil strife leading to infrastructure destruction and population displacement. There have been few opportunities for individuals to prosper from professional or entrepreneurial activities. Decades of neglected investment have resulted in deterioration of social services and economic infrastructure. Consequently, individuals have lacked capabilities to prosper and an investment climate conducive to prosperity. School enrollment and life expectancy have declined. Extremely low returns to education reflect the combination of poor educational quality and lack of employment opportunities. In terms of economic infrastructure, access to reliable electricity and water, and even access to paved roads are low, are further reflections of decades of neglect. While the upper end of the distribution has been pulled down by a lack of opportunities, the lower end has been supported by direct government provision of food. The Public Distribution System (PDS) provides 85 percent of food needs. While PDS has been useful as a safety net for the poor and the vulnerable, the system is expensive, inefficient, and fiscally risky. Indeed, PDS food rations account for a far greater share of public spending than does education or health. Going forward, Iraq faces two main challenges. First, although Iraq does not have to develop from scratch, it faces a formidable challenge in re-development. Second, a shift by the Government is required from direct provision of basic subsistence toward investment in human capacities. The Government can provide an enabling environment through investments in economic infrastructure and services to business and citizens, thus allowing the population to make productive use of education and their own labor. Both challenges are now being taken up by the Poverty Reduction Strategy, which articulates a detailed set of required actions and outlines priorities for government spending.
