Son incidence est de 120 000 cas par an en France. La survenue d'une transformation h;morraLzique (TH) est une des complications majeures de l'AIC, à l'origine d'un mauvais pronostic. L'imagerie rzalisze en urgence au CHU de Rouen pour toute suspicion d'AIC est une tomodensitométrie cérébrale (TDMc) multimodalités. Elle permet de mesurer le produit perméabilité-surface (PS), reflet de la perméabilité de la barrière émato-encéphalique, impliquée dans la physiopathologie de la TH. Objectifs : L'objectif de notre travail était de déterminer les facteurs prédictifs radiologiques et cliniques de TH de l'AIC. Matériel et méthode : Il s'aQit d'une étude descriptive, prospective et consécuti\e. mono-centrique, menée du 30 juin 2009 au 31 décembre 2010. concernant les patients accueillis en urgence au CHU de Rouen pour une symptomatologie compatible avec un accident vasculaire cérébral datant de moins de 6 heures. Nous avons recueilli les données cliniques à l'admission, l'évolution clinique immédiate et à 90 jours et les données de la TDMc multi-modalités, dont le PS. IRM cérébrale, ou à défaut la TDMc sans injection, réalisée entre J3 et J5 de l'hospitalisation, permettait de confirmer le diagnostic d'AIC et de mettre en évidence une éventuelle TH au sein de l'infarctus. Résultats : Quatre-vingt six patients ont été inclus. Parmi ces patients 27 ont présenté une TH, dont 11 étaient symptomatiques et 16 asymptomatiques. Un PS > 0.84 ml/100g/min était prédictif de transformation hémorragique. Les autres facteurs prédictifs de TH étaient le volume de la lésion ischémique à la TDM de perfusion et un antécédent de fibrillation auriculaire. Conclusion : Un PS > 0.84 ml/100g/min mesuré à la TDM de perfusion est un facteur prédictif de transformation hémorragique et pourrait aider à la prise en charge des patients à la phase aiguë d'un infarctus cérébral notamment pour la décision de thrombolyse.

Méthodes : A partir de l'obseRvatoire régional des Infarctus de Côte d'Or (RICO), 8485 patients consécutifs admis aux soins intensifs de cardiologie pour infarctus du myocarde entre janvier 2001 et juillet 2010 ont été inclus. Parmi d'autres évènements cardiaques majeurs, la survenue d'un accident vasculaire cérébral (ischémique ou hémorragique) ou d'un accident ischémique transitoire était collectée de façon prospective pendant une année de suivi.Résultats : 168 (1.98%) accidents vasculaires cérébraux ont été enregistrés sur la période d'un an de suivi après la survenue de l'infarctus du myocarde. 123 patients (1.4%) ont présenté cet accident vasculaire cérébral à la phase intra-hospitalière, dont pour 87% d'entre eux dans les 5 premiers jours après leur admission. Cependant le rt-PA expose à un risque d'hémorragie intracérébrale(HI) responsable de la principale cause de mortalité précoce des infarctus. Élaboration d'un score à partir de facteurs cliniques et radiologiques indépendants et prédictifs du risque d'HI symptomatique (HIS) ou totale. Il s'agit d'une étude observationnelle monocentrique des IC traités par rt-PA de 2015 à 2019 pris en charge au CHU d'Amiens. Les données clinicoradiologiques issues d'un registre prospectif ont été analysées rétrospectivement. Les variables quantitatives statistiquement significatives en analyse univariée ont été transformées en variables binaires en fonction du seuil optimal déterminé à partir des courbes ROCs. Elles ont été intégrées dans des modèles de régression multivariées. Les coefficients du modèle final, après application de techniques de shrinkage ont été utilisées pour la construction du score prédictif. 536 patients ont été inclus sur 791. 46 (8.6%) ont présenté une HI symptomatique pour un total de 94 hémorragies. En analyse multivariée seuls le NIHSS, le volume lésionnel en séquence DWI et l'analyse quantitative du FLAIR étaient significativement associés au risque d'HIS. Une valeur supérieure à 4 du score prédictif issu de ces variables avait une spécificité de 81% et une sensibilité de 77% soit une aire sous la courbe de 0.83. Cette étude argumente l'intérêt de l'IRM dans l'évaluation du risque hémorragique après traitement par rt-PA des IC, et propose un score prédictif simple et rapide.

Book excerpt: Le bénéfice de la thrombolyse intraveineuse, traitement de référence des infarctus cérébral à la phase aiguë, est contrebalancé par sa principale complication : l’hémorragie intracérébrale. L’utilisation plus systématique de l’IRM cérébrale en pré-thérapeutique a permis de mieux sélectionner les patients pouvant bénéficier de ce traitement, mais a également permis la mise en évidence de microsaignements ou microbleeds (MB) dont la valeur pronostic est mal connue. Ces derniers sont particulièrement bien visualisés sur les séquences de susceptibilité magnétique (SWI). Nous avons étudié le lien entre la présence de MB visualisés sur cette nouvelle séquence d’imagerie et la survenue d’une transformation hémorragique dans les sept jours suivant le traitement par thrombolyse intraveineuse pour infarctus cérébral de moins de 4h30. Notre étude a été menée prospectivement entre juin 2012 et juin 2013 au CHRU de Tours. Au total, 91 patients thrombolysés ont eu une IRM 3 Tesla avec séquence SWI au moment de la thrombolyse et à 7+/- 3 jours. Des MB étaient visualisés chez 28 patients, parmi lesquels 17 (63%) ont présenté une transformation hémorragique dont 2 (7%) étaient symptomatiques. Ces taux étaient respectivement de 53 et 10% chez les patients sans MB. Il n’a pas été mis en évidence de lien significatif entre la présence de MB visualisés en séquence SWI et le risque de transformation hémorragique, symptomatique ou non sur l’IRM de suivi à 7 jours. Notre étude est la première dans la littérature utilisant la séquence SWI pour évaluer de manière prospective la valeur prédictive des MB dans la transformation hémorragique post-thrombolyse.
