Le nombre de consultations a doublé en 20 ans et cette hausse semble parallèle à la multiplication des « consultations non programmées ». L'objectif de cette étude était de décrire le parcours de soins et le devenir des patients pris en charge aux urgences afin de proposer des solutions pour que perdure notre système de soins. Méthode : Nous avons réalisé une étude observationnelle, prospective, monocentrique aux urgences du centre hospitalier de Brive la Gaillarde entre le 1 et le 7 octobre 2021. Résultats : Les données ont été analysées pour 71% des personnes ayant consulté. La classe d'âge la plus représentée concernait les moins de 30 ans. La majorité des patients sont venus par leurs propres moyens, sans avis médical préalable pour une urgence ressentie. 80% étaient suivis par un médecin traitant. Moins de 15% des consultations spontanées faisaient suite à l'indisponibilité du médecin généraliste. Les patients ont consulté plutôt pendant les horaires d'ouverture des cabinets médicaux entre 8h et 20h. 70% ont bénéficié d'examens complémentaires et 30% ont été hospitalises. 80% des hospitalisés ont été adressés par le 15 ou leur médecin traitant. Conclusion : Le renforcement de l'éducation des patients concernant le parcours de soins coordonnés et la mise en place d'une organisation spécifique dédiée à la prise en charge des soins non programmés font partie des solutions à envisager.

Book excerpt: Objectif : nous souhaitions comparer les patients se présentant spontanément aux urgences avec ceux venant par transports aux urgences afin d'apporter des éléments permettant de mieux appréhender les flux croissants de patients aux urgences en organisant de façon plus adaptée l'amont. L'objectif principal de l'étude était de comparer le taux d'hospitalisation parmi les patients venant par leurs propres moyens par rapport aux patients venant aux urgences par transport. Les objectifs secondaires étaient de comprendre et d'évaluer les facteurs associés à de tels recours aux urgences. Matériel et méthodes : il s'agissait d'une étude épidémiologique de cohorte prospective observationnelle multicentrique réalisée sur 9 centres d'urgences du département de la Gironde sur deux fois 24 heures simultanément sur tous les sites étudiés. Tout patient se présentant aux urgences a été associé à un questionnaire visant à établir le profil des patients venant consulter aux urgences du début du problème médical, son parcours de soins aux urgences jusqu'à son devenir. Résultats : sur les 1165 patients inclus dans l'étude, dont 765 soit 65,6% de l'effectif étaient venus en présentation spontanée aux urgences et 400 venant par transports soit 34,5% de l'effectif. La médiane des patients en consultation spontanée était de 42 ans et celui des patients venant par transport de 65 ans. Parmi les patients en présentation spontanée 9,7% étaient hospitalisés. Parmi les patients venant par transports 24,3% étaient hospitalisés. Discussion / Conclusion : la population représentée par les patients en présentation spontanée requièrent plus d'une prise en charge ambulatoire que d'une prise en charge hospitalière. Ceci contribue encore aujourd'hui à une difficulté de la part des services d'urgences à faire face à cette demande de soins croissante de ce profil de patients qui pourrait être pris en charge par la médecine générale libérale. Les patients venant par transports ont un profil s'adressant, eux, beaucoup plus à une prise en charge hospitalière. Cependant, pour éviter un afflux non justifié des patients aux urgences, il faut que le système de régulation de la demande soit bien organisé en amont des établissements de santé et que son fonctionnement soit connu de la population, ce qui n'est pas forcément le cas aujourd'hui.

The 2018 TZG updates and expands on the guidance in the one previous jointly-developed, zoonoses-specific guidance document: the 2008 Tripartite “Zoonotic Diseases: A Guide to Establishing Collaboration between Animal and Human Health Sectors at the Country Level”, developed in WHO South-East Asia Region and Western Pacific Region. The 2018 TZG supports building by countries of the resilience and capacity to address emerging and endemic zoonotic diseases such as avian influenza, rabies, Ebola, and Rift Valley fever, as well as food-borne diseases and antimicrobial resistance, and to minimize their impacts on health, livelihoods, and economies. It additionally supports country efforts to implement WHO International Health Regulations (2005) and OIE international standards, to address gaps identified through external and internal health system evaluations, and to achieve targets of the Sustainable Development Goals. The 2018 TZG provides relevant country ministries and agencies with lessons learned and good practices identified from country-level experiences in taking OH approaches for preparedness, prevention, detection and response to zoonotic disease threats, and provides guidance on multisectoral communication, coordination, and collaboration. It informs on regional and country-level OH activities and relevant unisectoral and multisectoral tools available for countries to use.
