Book excerpt: INTRODUCTION : Plusieurs échelles d'évaluation des risques ont été développées en population générale pour établir un score de gravité précoce lors de l'évaluation initiale des patients dans les services d'urgence et notamment les scores MEWS, REMS et ViEWS. Les personnes âgées constituent une partie importante des patients consultant aux urgences, mais ces scores ont été peu étudiés dans cette population spécifique. OBJECTIF : évaluer l'efficacité pronostique des scores MEWS, REMS et ViEWS en population gériatrique. MATÉRIEL ET MÉTHODES : étude prospective analytique monocentrique, réalisée au Service des Urgences du CHU de Nancy, incluant tous les patients de plus de 65 ans ayant consulté entre le 5 et le 12 octobre 2020. Le critère de jugement principal était la corrélation entre les scores MEWS, REMS et ViEWS et la mortalité à 30 jours. Le critère de jugement secondaire était la corrélation entre les scores MEWS, REMS et ViEWS et le retour à l'hôpital dans les 30 jours. La corrélation a été établie par régression logistique en comparant chaque groupe à l'ensemble de la population étudiée. RÉSULTATS : 266 patients ont été inclus. Les résultats étaient non significatifs pour le critère de jugement principal : X(MEWS) = 3,714 et p (MEWS) = 0,515 ; X(REMS) = 9,429 et p (REMS) = 0,107 ; X(ViEWS) = 7,571 et p(ViEWS) = 0,252 par rapport à l'ensemble de la population X(MEWS) = 1,470 X(REMS) = 7,395 et X(ViEWS) = 2,985. Les résultats étaient non significatifs pour les scores REMS et ViEWS pour le critère de jugement secondaire. Il existait une corrélation significative entre l'absence d'événement intercurrent dans les 30 jours et le score MEWS : X(MEWS) = 1,311 et p(MEWS) = 0,009 mais avec une faible valeur prédictive R2 (MEWS) = 0,029. CONCLUSION : Les scores pronostiques développés en population générale ne semblent pas être adaptés à la prise en charge des patients gériatrique, alors que cette population représente une part importante des consultations aux urgences. Le développement de scores spécifiques pourrait améliorer la prise en charge initiale de ces patients.

Book excerpt: Belgium has a unique place in the history of migration in that it was the first among industrialized nations in Continental Europe to develop into an immigrant society. In the nineteenth century Italians, Jews, Poles, Czechs, and North Africans settled in Belgium to work in industry and commerce. They were followed by Russians in the 1920s and Germans in the 1930s who were seeking a safe haven from persecution by totalitarian regimes. In the nineteenth century immigrants were to a larger extent integrated into Belgian society: they were denied political rights but participated on equal terms with Belgians in social life. This changed radically in the twentieth century; by 1940 the rights of aliens were severely curtailed, while those of Belgian citizens, in particular in the social domain, were extended. While the state evolved into a "welfare state" for its citizens it became more of a police state for immigrants. The state only tolerated immigrants who were prepared to carry out those jobs that were shunned by the Belgians. Under the pressure of public opinion, an exception was made in the cases of thousands of Jewish refugees that had fled from Nazi Germany. However, other immigrants were subjected to harsh regulations and in fact became the outcasts of twentieth-century Belgian liberal society. This remarkable study examines in depth and over a long time span how (anti-) alien policies were transformed, resulting in an illiberal exclusion of foreigners at the same time as democratization and the welfare state expanded. In this respect Belgium is certainly not unique but offers an interesting case study of developments that are characteristic for Europe as a whole.

Political thinkers reasoned that if government was to rest in the hands of individuals, then measures should be taken to understand the deepest reaches of the self, including the state of madness. Marcel Gauchet and Gladys Swain maintain that the asylum originally embodied the revolutionary hope of curing all the insane by saving the glimmer of sanity left in them. Their analysis of why this utopian vision failed ultimately constitutes both a powerful argument for liberalism and a direct challenge to Michel Foucault's indictment of liberal institutions. The creation of an artificial environment was meant to encourage the mentally ill to live as social beings, in conditions that resembled as much as possible those prevailing in real life. The asylum was therefore the first instance of a modern utopian community in which a scientifically designed environment was supposed to achieve complete control over the minds of a whole category of human beings. Gauchet and Swain argue that the social domination of the inner self, far from being the hidden truth of emancipation, represented the failure of its overly optimistic beginnings. Madness and Democracy combines rich details of nineteenth-century asylum life with reflections on the crucial role of subjectivity and difference within modernism. Its final achievement is to show that the lessons learned from the failure of the asylum led to the rise of psychoanalysis, an endeavor focused on individual care and on the cooperation between psychiatrist and patient. By linking the rise of liberalism to a chapter in the history of psychiatry, Gauchet and Swain offer a fascinating reassessment of political modernity.

Censorship was abolished, and France appeared to be on a path towards tolerance, pluralism, and civil liberties. A mere four years later, the country descended into a period of political terror, as thousands were arrested, tried, and executed for crimes of expression and opinion. In Policing Public Opinion in the French Revolution, Charles Walton traces the origins of this reversal back to the Old Regime. He shows that while early advocates of press freedom sought to abolish pre-publication censorship, the majority still firmly believed injurious speech--or calumny--constituted a crime, even treason if it undermined the honor of sovereign authority or sacred collective values, such as religion and civic spirit. With the collapse of institutions responsible for regulating honor and morality in 1789, calumny proliferated, as did obsessions with it. Drawing on wide-ranging sources, from National Assembly debates to local police archives, Walton shows how struggles to set legal and moral limits on free speech led to the radicalization of politics, and eventually to the brutal liquidation of "calumniators" and fanatical efforts to rebuild society's moral foundation during the Terror of 1793-1794. With its emphasis on how revolutionaries drew upon cultural and political legacies of the Old Regime, this study sheds new light on the origins of the Terror and the French Revolution, as well as the history of free expression.
