Book excerpt: Over the last decades, health economics has turned into one of the most active research fields within economics. The structure of health care markets varies enormously across countries, largely influenced by competition among suppliers, the regulation of markets and patient preferences. This dissertation presents an analysis of health care markets especially focusing on price competition and quality competition. Under price competition, a pair of asymmetric pure strategy price equilibria exists in a model with income constraints for the specific case that two physicians locate at the maximum distance from each other and patients pay the same marginal transportation cost. Under quality competition, I investigate the interplay of market transparency and semi-altruism - a specific and interesting aspect unique to markets for health care. Market transparency and semi-altruism show ambiguous effects on welfare. The more altruistic physicians provides weakly higher quality than the less altruistic one. Moreover, I explore individual and social incentives for hospital mergers and their interaction with transparency and find that higher transparency does not always lead to higher quality and higher social welfare. The results indicate that quality is lower after merger. A hospital merger leads to a higher social welfare if the efficiency gains from the merger are sufficiently large. ; eng

The theoretical literature in economics on competition and quality, the theoretical literature in health economics on this topic, and the empirical findings on competition and quality in health care markets are surveyed and their findings assessed.Theory is clear that competition increases quality and improves consumer welfare when prices are regulated (for prices above marginal cost), although the impacts on social welfare are ambiguous. When firms set both price and quality, both the positive and normative impacts of competition are ambiguous. The body of empirical work in this area is growing rapidly. At present it consists entirely of work on hospital markets. The bulk of the empirical evidence for Medicare patients shows that quality is higher in more competitive markets. If volume causes outcome, then policies supporting centralization of procedures in a few facilities may make sense. Specialty hospitals may have the benefit of producing better outcomes. Antitrust analysis of hospital mergers should probably consider any improved outcomes when evaluating the impact of the merger. Accordingly, the author analyzes the causality of the volume-outcome relationship for coronary artery bypass grafts (CABGs) and finds that volume might play the crucial role for hospitals quality. This finding makes minimum volume requirements a reasonable policy which is shown by a simulation on Californian data. However, the primary focus of the finding lies in US Antitrust application. Due to a causative volume-outcome relationship, a hospital merger may have welfare enhancing effects despite possible price increases. Patients will be better off as long as the quality improvement outweighs the loss of welfare due to the price increase. This is shown by a standard merger case simulation as well as a real merger, which took place in California in 1999. Results suggest that the presence of a volume-outcome relationship might serve as an efficiency defense at Antitrust trials.

We then summarize our main results on the welfare consequences of competition. We conclude with an illustration of how our methods can be used to assess the welfare implications of specific hospital mergers, and with some implications of our findings for antitrust policy.

Consolidation, and the resulting increased market concentration, can result in the ability of health care entities to engage in monopolistic pricing behaviors. Consolidation has been found often not to benefit patients and consumers in terms of lower prices, and has mixed effects on health care quality. However, health care consolidation's impacts on other outcomes, like access to health care and physician satisfaction, are largely unknown, but anecdotal evidence from communities where consolidation has occurred indicate that these impacts exist and are of concern. This dissertation aims to augment the existing evidence base on the consequences of and policy remedies for health care consolidation in three essays. The first essay uses secondary data to provide evidence on the impact of consolidation on physician social networks, the characteristics of which are associated with measurable outcomes such as costs and quality, as well as more distal outcomes like access to information and organizational effectiveness. The stability of shared patient networks increases significantly after acquisition of physician practices by another practice, which has potentially positive implications for organizational effectiveness. However, consistent and notable effects of consolidation on the size and strength of shared patient networks were not observed. The second essay uses case study methods to do a deep dive into a single market (Pittsburgh, Pennsylvania) that has experienced an intense, protracted period of consolidation to understand the breadth of the potential effects of cumulative consolidation events. The observed negative outcomes are largely attributable to three distinct pathways: the loss of physician autonomy when employed by large systems; the accumulation of information, resources, and power by large systems; and the high expectations to which large systems are held. The third essay explores alternative policy solutions for addressing consolidation and its negative consequences. Potentially impactful solutions are increased restrictions on allowable insurance contracting language and alteration of non-profit status requirements. Understanding these impacts of consolidation is critical, not just for policymakers and regulators, but also for health care purchasers, employees, taxpayers, and other stakeholders in communities that are experiencing or anticipating consolidation in their local health care markets so that they can prepare for or mitigate its negative consequences.
