Book excerpt: The implementation of the Patient Protection and Affordable Care Act (ACA) is reshaping healthcare in the United States. Four central objectives of the ACA are to improve healthcare access, improve the quality of healthcare, lower the cost of healthcare, and put in place consumer protections to ensure individuals can obtain affordable care regardless of their medical history. This dissertation focuses on understanding two pieces of the ACA aimed at improving access to care for individuals with mental health and/or substance use disorders (MHSUD): 1) Expanding parity of benefits between general medical care and coverage for substance use disorders (SUD) and 2) Requiring Medicaid to provide coverage for benzodiazepines, a widely prescribed psychoactive drug. This research evaluates two previous pieces of legislation to investigate the impact of the new ACA laws. First, new parity regulations in the ACA build upon the Mental Health Parity and Addiction Equity Act (MHPAEA) implemented in 2010, the first piece of federal legislation that expanded parity of health benefits to treatment for SUDs. Understanding the impact of the MHPAEA on SUD treatment uptake provides insight into the anticipated effects of parity expansion under the ACA. Second, in 2006 TennCare, Tennessee's Medicaid program, discontinued coverage for benzodiazepines (BNZ) for Medicaid recipients ages 21 and older. The sudden policy change makes it possible to evaluate the consequences and potential benefits of altering coverage for a widely used prescription medication that carries a high risk for abuse or dependence.

Book excerpt: The Mental Health Parity and Addiction Equity Act (MHPAEA) of 2008 is a federal act that was passed to widen the scope of the previous Mental Health Parity Act (MHPA) of 1996. The MHPAEA constructed uniform treatment regulations regarding mental health and substance use disorders. After federal parity laws were enacted, increases were evident in both behavioral health care service utilization and spending. This qualitative analysis found that health care service utilization increases were seen in insured adults and their dependents in the realms of bipolar disorder, major depression, substance use disorders, and other various mental health disorders. Spending increases were seen from insurers and insured adults and their dependents. The evidence suggests that in combination with state parity laws, the federal mandate of mental health insurance laws can aid individuals in seeking the proper mental health care they seek. Regarding spending, this study found that out-of-pocket expenses increased post-parity in relation to pre-parity data. Federal enactment of parity laws suggests more effective utilization, spending, and regulation of mental health and substance use treatment services. Future policy implications at the federal level include improvements in patient risk protection at minimal increases in costs, protection for employers that assume the risk for health care services and increases in behavioral health care service utilization alongside minimal increases in spending from all parties.

This analysis examined the history of mental illness in regards to the attitudes and treatment in America, the evolution of health insurance and historical policies for parity of mental health and substance abuse treatment. Furthermore, the analysis of the policy is conducted to further understand the objectives, effects and implications of this parity law. This policy analysis used a qualitative design, collecting data from primary and secondary sources that address this policy. A modified version of David Gil's (1992) policy analysis framework was utilized to analyze the policy. The major finding of this analysis was the MHP AE Act of 2008 had a limited impact in providing mental health and substance abuse parity in specific healthcare insurance policies.

These disorders are among the most highly stigmatized health conditions in the United States, and they remain barriers to full participation in society in areas as basic as education, housing, and employment. Improving the lives of people with mental health and substance abuse disorders has been a priority in the United States for more than 50 years. The Community Mental Health Act of 1963 is considered a major turning point in America's efforts to improve behavioral healthcare. It ushered in an era of optimism and hope and laid the groundwork for the consumer movement and new models of recovery. The consumer movement gave voice to people with mental and substance use disorders and brought their perspectives and experience into national discussions about mental health. However over the same 50-year period, positive change in American public attitudes and beliefs about mental and substance use disorders has lagged behind these advances. Stigma is a complex social phenomenon based on a relationship between an attribute and a stereotype that assigns undesirable labels, qualities, and behaviors to a person with that attribute. Labeled individuals are then socially devalued, which leads to inequality and discrimination. This report contributes to national efforts to understand and change attitudes, beliefs and behaviors that can lead to stigma and discrimination. Changing stigma in a lasting way will require coordinated efforts, which are based on the best possible evidence, supported at the national level with multiyear funding, and planned and implemented by an effective coalition of representative stakeholders. Ending Discrimination Against People with Mental and Substance Use Disorders: The Evidence for Stigma Change explores stigma and discrimination faced by individuals with mental or substance use disorders and recommends effective strategies for reducing stigma and encouraging people to seek treatment and other supportive services. It offers a set of conclusions and recommendations about successful stigma change strategies and the research needed to inform and evaluate these efforts in the United States.

The framework described in Psychosocial Interventions for Mental and Substance Use Disorders can be used to chart a path toward the ultimate goal of improving the outcomes. Book excerpt: The past half-century has been marked by major changes in the treatment of mental illness: important advances in understanding mental illnesses, increases in spending on mental health care and support of people with mental illnesses, and the availability of new medications that are easier for the patient to tolerate. Although these changes have made things better for those who have mental illness, they are not quite enough. In Better But Not Well, Richard G. Frank and Sherry A. Glied examine the well-being of people with mental illness in the United States over the past fifty years, addressing issues such as economics, treatment, standards of living, rights, and stigma. Marshaling a range of new empirical evidence, they first argue that people with mental illness—severe and persistent disorders as well as less serious mental health conditions—are faring better today than in the past. Improvements have come about for unheralded and unexpected reasons. Rather than being a result of more effective mental health treatments, progress has come from the growth of private health insurance and of mainstream social programs—such as Medicaid, Supplemental Security Income, housing vouchers, and food stamps—and the development of new treatments that are easier for patients to tolerate and for physicians to manage. The authors remind us that, despite the progress that has been made, this disadvantaged group remains worse off than most others in society. The "mainstreaming" of persons with mental illness has left a policy void, where governmental institutions responsible for meeting the needs of mental health patients lack resources and programmatic authority. To fill this void, Frank and Glied suggest that institutional resources be applied systematically and routinely to examine and address how federal and state programs affect the well-being of people with mental illness.
