This book was released on 2011 with total page 87 pages. Available in PDF, EPUB and Kindle. Book excerpt: Deglutition or a swallow begins as a voluntary act in the oral cavity but proceeds autonomously in the pharynx and esophagus. Bilateral sequenced activation and inhibition of more than 25 pairs of muscles of mouth, pharynx, larynx, and esophagus is required during a swallow. A single swallow elicits peristalsis in the pharynx and esophagus along with relaxation of upper and lower esophageal sphincters. Multiple swallows, at closely spaced time intervals, demonstrate deglutitive inhibition; sphincters remain relaxed during the entire period, but only the last swallow elicits peristalsis. Laryngeal inlet closure or airway protection is very important during swallow. Upper part of the esophagus that includes upper esophageal sphincter is composed of skeletal muscles, middle esophagus is composed of a mixture of skeletal and smooth muscles, and lower esophagus, including lower esophageal sphincter, is composed of smooth muscles. Peristalsis progresses in seamless fashion, despite separate control mechanism, from the skeletal to smooth muscle esophagus. The esophagus's circular and longitudinal muscle layers contract synchronously during peristalsis. Sphincters maintain continuous tone; neuromuscular mechanisms for tonic closure in the upper and lower esophageal sphincters are different. Lower esophageal sphincter transient relaxation, belching mechanism, regurgitation, vomiting, and reflux are mediated via the brain stem. Table of Contents: Introduction / Central Program Generator and Brain Stem / Pharynx-Anatomy, Neural Innervation, and Motor Pattern / Upper Esophageal Sphincter / Neuromuscular Anatomy of Esophagus and Lower Esophageal Sphincter / Extrinsic Innervation: Parasympathetic and Sympathetic / Interstitial Cells of Cajal / Recording Techniques / Motor Patterns of the Esophagus-Aboral and Oral Transport / Deglutitive Inhibition and Muscle Refractoriness / Peristalsis in the Circular and Longitudinal Muscles of the Esophagus / Neural and Myogenic Mechanism of Peristalsis / Central Mechanism of Peristalsis-Cortical and Brain Stem Control / Peripheral Mechanisms of Peristalsis / Central Versus Peripheral Mechanism of Deglutitive Inhibition / Neural Control of Longitudinal Muscle Contraction / Modulation of Primary and Secondary Peristalsis / Neural Control of Lower Esophageal Sphincter and Crural Diaphragm / Lower Esophageal Sphincter / Swallow-Induced LES Relaxation / Crural Diaphragm Contribution to EGJ and Neural Control / Transient LES Relaxation and Pharmacological Inhibition / Compliance of the EGJ / References

Available in PDF, EPUB and Kindle. Book excerpt: The third triennial congress of the International Society for Diseases of the Esophagus (lSDE) was held in Munich in September 1986 under the presidency of J. Rudiger Siewert. The very well organized and epoch-making meeting was part of the International Esophageal Week and was held in conjunction with the International Conference of Esophageal Diseases, the European Study Group of Diseases of the Esophagus (GEEMO), and International Organization for Statistical Studies of Esophageal Diseases (OESO). I am very pleased that it has been possible to publish the proceedings of the congress so rapidly. The problems associated with diseases of the esophagus are multifaceted, varying considerably according to locality; their study thus neces sitates a large international forum, which is one of the founding aims of the ISDE. We hope that this volume, by virtue of the in-depth and up-dated reports it contains, can playa role in advancing the work being done in this field. Finally I should like to express my heartfelt appreciation to Professor Siewert and Dr. Holscher for their contribution toward the success of the congress. Kiyoshi Inokuchi, M.D., F.A.C.S. President of the ISDE Preface The scientific knowledge about esophageal diseases and their treatment has rapidly developed and increased within the last 10 years. The present book reproduces for 1987 the topical state of the art about pathology, pathophysiology, diagnostic, and therapy of benign and malignant diseases of the esophagus.

The burning sensation caused in the chest due to acid reflux is a very common esophageal disease. Prolonged exposure to the gastric acids may erode the lining of the esophagus, leading to Barrett's esophagus. It is a clinical condition which is considered a high risk factor for adenocarcinoma. Some of the other conditions of esophagus are Chagas disease, esophageal cancer, esohageal dysphagia, esophagitis, Killian-Jamieson diverticulum, neurogenic dysphagia, Zenker's diverticulum, etc. An X-ray of swallowed barium is generally used to assess the shape and size of the esophagus and any underlying mass. Through a procedure called endoscopy, it is possible to visualize the interior of the esophagus. It also allows a biopsy to be taken that is useful for the diagnosis of cancer. Esophageal cancer is usually managed with chemotherapy, radiotherapy or the partial surgical removal of the esophagus. The topics included in this book on esophageal disease are of utmost significance and bound to provide incredible insights to readers. This book presents researches and studies performed by experts across the globe on the diagnosis and treatment of esophageal diseases. It will prove to be immensely beneficial to students and researchers involved in gastroenterology.

A number of surgeons publish and focus in malignant diseases of the esophagus, but this text emphasizes the benign ones, their problems and how to face them with the new available resources and technology. This book describes the different diagnostic tools available, as well as the different therapies for the management of gastroesophageal reflux disease (GERD) and its potential complications like Barret’s or strictures, and the associated anatomical abnormalities such as hiatal hernia or short esophagus, and more. Motility disorders of the esophagus, esophageal diverticula and perforations are also discussed. Different devices and approaches can be used to treat these diseases and they are covered and explained in different chapters of the book. Written by experts in the field, Benign Esophageal Disease provides surgeons and physicians with a concise and comprehensive summary of the current state of the field and will help guide patient management.
