Book excerpt: Episodic Future Thinking (EFT), an exercise that involves cognitive simulation of the future, has demonstrated proximal effects on measures of impulsivity and alcohol demand. However, few studies have investigated EFT's potential to reduce alcohol use outside the lab. This study piloted an academic goal-relevant (A-EFT) intervention for heavy drinking college students. Forty-five heavy drinking undergraduated were randomized to complete a brief A-EFT intervention, or control task. Recruiment rates supported the feasibility of our approach; interest and scheduling rates were high, and booter and follow-up completion rates were good. Participants assigned to A-EFT increased the amount of time spent studying in the evening compared to students assinged to control. Within-group analyses revealed significant decreases in alcohol demand and alcohol consumption, and an increase in protective drinking strategies in the A-EFT group. The current study supports the feasibility, acceptability, and preliminary efficacy of an A-EFT intervention for college student heavy drinkers. .

The study used an independent group design, whereby one group was administered alcohol and the other received a matched placebo. Both groups were required to imagine future-oriented events in relation to a cue sentence and then perform various cognitive tasks, including an imagination task. The results indicated that EFT remains relatively robust against acute alcohol effects. The implications of this finding are considered within the context of clinical interventions for alcohol dependent individuals. EFT simulations were also found to include episodic memory content, thereby providing support for the Constructive Episodic Simulation hypothesis (Schacter, Addis & Buckner, 2008).

Limitations of PNF interventions include reactance, defensiveness, and a lack of attention to and adequate processing of the information. Adding a writing component to PNF interventions may compensate for these limitations and boost intervention efficacy. The present study evaluated whether increasing cognitive processing and reducing defensiveness and reactance in response to PNF through an expressive writing task would improve the efficacy of this brief intervention tool. Participants included 244 University of Houston students who met screening criteria, correctly answered two out of three check questions, and were randomized to receive either: 1) PNF about their alcohol use; 2) expressive writing about a heavy drinking occasion; 3) PNF plus expressive writing; or 4) attention control feedback about their technology use. One month post-baseline, 169 participants completed a follow-up survey asking about their past month alcohol use and alcohol-related problems. Results revealed that the PNF plus writing condition significantly reduced drinking via the AUDIT-C and reduced alcohol-related consequences at follow-up compared to control. Further, intervention effects were moderated by factors such as readiness to change, intentions for drinking, depth of processing of the feedback, and cognitive processing language in the narratives. Findings have implications for future alcohol intervention efforts among college student drinkers.

Book excerpt: Abstract: Excessive and harmful alcohol consumption is especially common among college students in the United States and is associated with proximal and distal psychiatric and medical morbidity. Brief, individual interventions that provide either risk and normative feedback or normative feedback alone have been shown to particularly efficacious with high-risk college drinkers. The relative efficacy of the two types of feedback has not been determined, however. The current study compares the efficacy of these two interventions with a sample of high-risk college drinkers. It also provides preliminary data on purported mechanisms of change: risk- and norm-perception. College students (n = 1197) were screened for alcohol consumption and alcohol-related problems. The top quartile of this sample (n = 299), in terms of alcohol use severity, was randomly assigned to one of three treatment conditions: risk and normative feedback, normative feedback alone or assessment only. All treatment conditions included a baseline assessment session and the feedback conditions included an in-person feedback session. One hundred and fifteen participants successfully completed their assigned condition. Thirty-eight participants (33%) completed an assessment six months later. Overall, findings did not replicate the existing literature supporting the efficacy of these feedback interventions. Neither feedback group showed appreciable, unqualified reductions in alcohol consumption or alcohol-related problems at follow-up. Primary predictors of alcohol use and alcohol-related problems at follow-up were the baseline measures of these variables. Furthermore, the relationships between baseline and follow-up measures were often qualified by an interaction effect, with individuals who consumed more alcohol or experienced more alcohol-related problems, relative to other participants, being more likely to have a negative response to treatment. Finally, there were no group differences on purported mechanisms of change. Findings may be explained by methodological and sample differences between this study and other, related studies, as well as by social and cognitive mechanisms.

The sample of freshman college students recruited from Spit for Science (Dick et al., 2014) included 153 intervention participants, and 151 control participants matched on demographics and baseline alcohol variables. Hierarchical multiple regression, logistic regression, and moderated multiple regression were used to compare intervention and control participants on post-intervention alcohol variables. Treatment predicted lower alcohol use disorder (AUD) symptoms, particularly among baseline drinkers. For non-drinkers, the intervention was associated with a decreased likelihood of alcohol initiation. Family history moderated the intervention's effect on drinks per occasion and AUD symptoms, with family history positive individuals responding better to the intervention. Readiness-to-change and concern for one's drinking were not supported as moderators, suggesting more research is needed to identify mechanisms of change.

Book excerpt: Brief Motivational Interventions (BMIs) have been identified as one of the most effective individually focused alcohol intervention strategies for college students in the United States. Despite the central role of motivation for change in BMIs, whether BMIs increase motivation for change has rarely been investigated. The current study conducted a meta-analysis of individual participant data (IPD; 15 studies, N = 5,903) from Project INTEGRATE (Mun et al., 2015) to examine whether BMIs increase motivation for change in drinking. Different measures and responses used in the original trials were harmonized across studies, and effect size estimates were derived from a model that adjusted for baseline motivation and demographic variables for each study (step 1) and subsequently combined in a random-effects meta-analysis model (step 2). After adjustment for baseline levels of motivation level and demographic variables, the intervention effects of BMIs on motivation for change was not statistically significant (standard mean difference [SMD]: 0.026, 95% CI: [-0.001, 0.053], p = .06, k = 19). Subsequent metaregression analyses among BMI subtypes indicated that the intervention effect did not differ between individually delivered motivational interviewing with personalized feedback (MI+PF), stand-alone personalized feedback (PF), and group-based motivational interviewing (GMI). Among all BMI subtypes, only GMI had a statistically significant intervention effect on motivation compared to controls (SMD: 0.055, 95% CI: [0.007, 0.103], p = .025, k = 5). Within the first three months post-intervention, there was a decrease in SMD of 0.05 (95% CI: [0.01, 0.08]) in motivation per month. However, no statistically significant difference in the intervention effects was found between studies with short-term vs. long-term follow-up. Although the results from the current study do not support the hypothesis that BMIs improve motivation for change, the evidence as a whole suggests ways in which motivation may be improved following intervention and can be tested in future studies.

Although quantitative research with volunteer participants supports the use of enhanced brief motivational interventions and cognitive behavioral skills training in reducing risky drinking behavior (binge drinking), research with mandated students has shown inconsistent findings. The current study is a phenomenological qualitative study exploring the students' perspectives after attending a mandated college-level alcohol intervention program. Mandated students are students who have been referred to an alcohol intervention as a result of violating an alcohol related policy on campus. Protective behavioral strategies were used by the mandated students and perceived as helpful in reducing alcohol consumption and minimizing alcohol related consequences. Mandated students expressed more satisfaction with meeting face-to-face in the SPARK motivational intervention as compared to computer-based intervention programs. Factors that facilitate motivation to change are a relatively untapped field of exploration. There is a relationship between the satisfaction with the facilitator and satisfaction with the alcohol intervention. As a result, the SPARK participants reported reducing their risky drinking behavior. Mandated students should attend a face-to-face motivational intervention consisting of motivational interviewing and cognitive behavioral skills in an effort to influence a motivation to change. Nine themes emerged from the data. The findings build on previous research by providing a more nuanced understanding of mandated students' experience through a college-level alcohol intervention program called SPARK.

The severity of the situation and lack of effective alcohol programming on college campuses warranted the National Institute on Alcohol Abuse and Alcoholism to commission a Task Force on College Drinking in 2002, which has been vital in revealing drinking patterns and negative consequences which are specific to the college environment. The Task Force proposed three strategies that were empirically validated for prevention and intervention in the college setting. Of the three recommendations, implementing cognitive behavioral skills training and offering motivational enhancement interventions, while proven effective are costly and time consuming to implement. The final strategy recommended, challenging alcohol expectancies, has been validated for use in a group setting making it a more viable option for reaching larger audiences. Within the college environment there are certain factors that have shown to be important in influencing college students' drinking behaviors, attitudes toward drinking, and alcohol related negative consequences. Specifically, membership in a fraternity or sorority has revealed a unique predictor of risky drinking behavior and an increased risk of suffering from negative consequences related to alcohol. The purpose of the present study was to implement an expectancy-based presentation in Greek chapter houses to alter expectancies and decrease risky drinking behavior. Alcohol expectancies were measured before and immediately after the presentation. Alcohol consumption was also assessed in a self-report measure of drinking for the 30 days prior to the presentation as well as 30 days following it. Analyses revealed significant reductions in positive alcohol expectancies and alcohol consumption on measures of quantity (average drinks per sitting), frequency (average drinking days per week), and heavy episodic drinking (average weekly peak blood alcohol content). Therefore, the structure and effectiveness of the current intervention program proves extremely useful and practical for widespread implementation in Greek chapter houses across all college campuses.
