Book excerpt: Context: Community Associated Methicillin Resistant Staphylococcus Aureus (CA-MRSA) infections continue to impose an annual multi-billion cost on society with athletes being an at risk population. With the focus of sports medicine CA-MRSA research centered on retrospective outbreaks and environmental studies, recommendations and position statements spawned from the results. Although these recommendations offer effective solutions to controlling and managing these infections, they have not served to eliminate the problem. Few studies have attempted to examine infection rates, referral practices and physician management steps for CA-MRSA infections within student-athletes. Objective: To retrospectively and prospectively assess the rate of CA-MRSA infections, sport risk, management protocol and referral practices within high school and intercollegiate athletic programs. Design: Two descriptive epidemiological studies. One review of current literature. Retrospective Patients or Other Participants: 156 certified athletic trainers (ATs) from intercollegiate and high school programs within National Athletic Trainers Association (NATA) District Two. Retrospective Main Outcome Measures: Frequencies, analysis of variance, prevalence and relative risk were calculted to determine differences between settings and sports for CA-MRSA infections. Retrospective Results: The overall infection prevalence was 15.5 per 10,000 athletes. The prevalence and relative risk were significantly higher in wrestling and football compared to the general athletic population. The majority of referrals occurred to general or primary physicians. The majority of respondents indicated proper decontamination. Approximately half the respondents were unsure of physician management steps. Prospective patients or Other Participants: Eight-seven certified athletic trainers (ATs) responded to the first survey request. Over the course of the competitive academic year, sixty-one participants responded to every survey. Prospective Main Outcome Measures: Frequencies, analysis of variance, incidence rates and relative risk were calculated to determine differences between settings and sports for CA-MRSA infections. Prospective Results: The overall CA-MRSA infection incidence was 16.3 per 10,000 athletes. The incidence and relative risk were significantly higher in wrestling and football compared to the general athletic population. The majority of the suspicious bacterial lesions were referred to either primary care or general physicians with patients most likely to receive antibiotic coverage for CA-MRSA regardless of diagnostic confirmation. Conclusions: Athletes, particularly contact athletes, suffer a higher rate of CA-MRSA infections compared to the general population. Despite successful preventative meaures and recommendations, the rate of CA-MRSA infection among athletes remains high. Upon referral, many cases of suspicious bacterial lesions are issued antibiotics to treat CA-MRSA. As CA-MRSA strains begin to display resistance to antibiotic mainstays, it is critical that certified athletic trainers stay abreast of the eve changing standards of medical care.

Book excerpt: Methicillin-resistant Staphylococcus aureus (MRSA) is a serious public health problem nationwide, threatening to develop into an epidemic. Many of these patients are presenting to their primary care clinics with skin and soft tissue infections (SSTIs). The CDC has reported that in 2005, MRSA was responsible for an estimated 94,000 life-threatening infections and 16,650 deaths. The purpose of this study is to estimate the incidence of CA-MRSA within a specific family practice in Florida and to identify epidemiologic factors, classify antibiotic susceptibility patterns, and evaluate patient education in regard to disease management and prevention. This study was a descriptive, epidemiologic, three-year retrospective medical record review of all wound cultured skin and soft tissue infections that presented to a family practice between January 2007 and December 2009. Sixty-two medical records met the inclusion and exclusion criteria for the study. Of these 62 SSTIs, 44 cultures grew one or more bacterial organisms. The incidence of CA-MRSA was 66% (n=29). The mean age of those with CA-MRSA was 40 years old, with a range from 7 to 90 years old. Sixty-two percent (n=18) were male and 38% (n=11) were female; additionally 69% (n=20) lived within a 10 mile radius from the family practice, while 31% (n=9) lived in a surrounding suburb. The most frequent race was Caucasian 83% (n=24), with African American at 10% (n=3) and Hispanics 7% (n=2). Risk factors associated with CA-MRSA was obesity 41% (n=10), diabetes mellitus 24% (n=7), and a previous history of MRSA infection 24% (n=7). Skin and soft tissue infections were diagnosed as either an abscess 62% (n=18), boil 24% (n=7), pustule 10% (n=3), or cellulitis 4% (n=1). CA-MRSA isolates were susceptible to trimethoprim-sulfamethoxazole 100% (n=29), doxycycline 93% (n=27), and rifampin 100% (n=14). Clindamycin susceptibility was 65% (n=15) with resistance at 30% (n=7) and 5% (n=1) intermediate. Both cephalexin and erythromycin were 100% resistant. Documentation in the medical record on wound care was found in 45% (n=13) of the records. The incidence of CA-MRSA SSTI was 66%, which identifies this suburban community at high risk for this bacterial infection. Risk factors associated with CA-MRSA included obesity (BMI [greater than] 30), history of previous MRSA infection, and diabetes mellitus. There were no clinical characteristics that helped distinguish MRSA infection from other bacterial SSTIs. Most SSTI were treated with incision and drainage and a susceptible antibiotic. Judicious use of antibiotics not only provides appropriate treatment, but is also critical in prevention of antibiotic resistance. Lastly, patient education in adequate hygiene is essential in preventing the spread of CA-MRSA.

These strains have the capacity to cause infections in healthy individuals in community settings, but are beginning to cause infections in hospitals. Since the original epidemiological definition of CA-MRSA is no longer appropriate, this thesis uses a molecular definition. -- Most hospital MRSA are ciprofloxacin-resistant so I used ciprofloxacin susceptibility (Cip-S) as a screening marker to determine whether putative CA-MRSA were present at Guy's & St. Thomas' Hospital (GSTT) from 2000-2006, but had not been identified due to the volume of hospital MRSA identified in the clinical laboratory. Due to the poor sensitivity of Cip-S and other screening markers for the identification of CA-MRSA, I studied all MRSA identified during the first six months of universal admission screening to investigate the prevalence and circulating strain types of CA-MRSA at GSTT. Finally, I performed detailed molecular analysis of a sub-set of CA-MRSA isolates to investigate variation in the Panton-Valentine leukocidin (PVL) genes and their associated bacteriophages. -- Cip-S MRSA were present in increasing numbers during 2000-2006 and had characteristics consistent with published accounts of CA-MRSA. The overall prevalence of MRSA among patient admission screens was 1.6%; colonisation rates were higher in medical specialties (2.4%) and lower in surgical specialties (1.2%). CA-MRSA strains accounted for up to 25% of MRSA in certain specialties. A wide variety of PVL-positive and PVL-negative clones were identified among CA-MRSA. The most common type was a PVL-negative sequence type (ST)-1 clone associated with injecting drug users I homeless patients. Sequence variation in the PVL genes tended to vary according to lineage and correlated with the PVL-encoding bacteriophage.

During this time many distinct clones have emerged independently around the world, some of which have achieved pandemic status. More recently, community-associated (CA) and livestock-associated MRSA clones have also emerged, some of which have become established in hospitals and other healthcare facilities, and sometimes have displaced previously predominant HA clones. Importantly, MRSA can frequently exhibit resistance to a wide range of clinically relevant antibiotics, which limits treatment options and complicates patient management and outcomes. Investigating routes of transmission and spread of MRSA in healthcare facilities have conventionally been undertaken by combining available epidemiological information with data from DNA-based typing systems such as pulse-field gel electrophoresis typing, spa typing, multilocus sequence typing, and more recently, DNA microarray profiling. However, these approaches can frequently lack the discriminatory ability to differentiate between MRSA isolates in healthcare environments where a relatively small number of clones may predominate. The advent of high-throughput whole genome sequencing (WGS) over the last decade with the development of affordable, easy-to-use benchtop DNA sequencing platforms, associated sequencing chemistry and bioinformatics tools, has revolutionized studies of MRSA epidemiology and evolution. The significantly enhanced discriminatory power and resolution afforded by WGS has also provided hitherto unimaginable insights into the origins, emergence and factors that drive the evolution of specific MRSA clones. Furthermore, WGS has highlighted the very significant contributions of mobile genetic elements (MGEs) encoding virulence factors and resistance genes from coagulase-negative staphylococcal (CoNS) species to the emergence and evolution of MRSA. This Research Topic brings together a collection of original research articles and up-to-date reviews that highlight the significant impact WGS is having on our understanding of the epidemiology and routes of transmission of HA- and CA-MRSA in humans and the phylogenetics and evolution of specific MRSA clones. The Research Topic also highlights the impact that WGS is having on our understanding of antimicrobial resistance in MRSA by acquisition of MGEs and the role of specific CoNS species in the origins and evolution of particular MGEs that can promote the survival of MRSA following acquisition. A descriptive quantitative research design was used to determine CA-MRSA prevalence rates among a sample population from clients who presented to a local community hospital emergency department over a six month period in the latter part of 2008. One hundred and forty three charts were reviewed for a positive diagnosis of either hospital-acquired methicillin-resistant Staphylococcus aureus (HA-MRSA) or CA-MRSA. Of those charts the actual prevalence of CA-MRSA infections was calculated by dividing the known cases by the total number of clients who presented to the local community hospital ED in the six month period under review. Also, the collected data was subjected to multiple linear regression in order to analyze predictive variables for CA-MRSA infection amongst the clients included in this study.

MRSA infections cause approximately 19,000 deaths among hospitalized Americans annually. It is one of the leading causes of healthcare associated, or nosocomial infections, particularly in intensive care units. Hospital acquired MRSA (HA-MRSA) has been a battle for inpatients since the 1960's. However, in the late 1990's, a new strain of MRSA emerged. It appeared outside of the hospital setting and has been termed, community associated MRSA (CA-MRSA). Presently, CA-MRSA has been found to be spreading into the healthcare system presenting a new obstacle for patients and hospitals to overcome. It has been suggested that employees play a role in transmission of MRSA to hospitalized patients. Since healthcare workers are at the interface between hospitals and the community, they may serve as a potential reservoir for spreading MRSA. However, there are a limited number of studies that investigate employee MRSA colonization and subsequent transmission to patients. This study seeks to provide molecular evidence supporting the likelihood that employees play a role in MRSA transmission to patients. Furthermore, with the implementation of a version of "search and destroy", an infection control strategy, we show how reductions of hospital-acquired infections are achieved using this method.
