Sa prévalence au niveau international ne cesse d'augmenter malgré les tentatives de prises en charge thérapeutique pluridisciplinaire. L'environnement de l'enfant a une grande influence sur son mode de vie et notamment sur sa nutrition et ses activités physiques. Un enfant obèse à de grands risques de devenir un adulte obèse. Nous estimons nécessaire de connaitre les facteurs environnementaux ayant des répercussions sur l'obésité infantile, et détailler leurs mécanismes d'action afin de pouvoir ajuster nos méthodes de prévention. Matériel et méthode : Nous avons donc réalisé une revue de la littérature à partir de Pubmed grâce à une équation de recherche suggérée par Cismef, incluant tous les articles traitant de l'obésité pédiatrique et des facteurs environnementaux biologiques, chimiques, physiques et cognitifs publiés de 2010 à 2016. Au total, 898 articles étaient proposés par l'équation de recherche, nous nous sommes intéressé à 130 d'entre eux et nous avons ajouté 28 articles complémentaires. Résultats : 55 articles évoquaient le rôle de l'environnement biologique sur l'obésité de l'enfant. Il était montré que plus l'impact était précoce (in utero et post natal), plus il était grand. Les facteurs de risques retenus étaient : l'environnement obésogène maternel et paternel et leurs obésités propres, induisant des modifications épigénétiques transmissibles aux descendants, la malnutrition et le diabète gestationnel et les dysbioses intestinales. L'allaitement maternel et ses vertus biochimiques semble être le meilleur facteur protecteur contre l'obésité infantile. 42 articles prouvaient le rôle pathogène des substances chimiques inhalées telles que le tabac ou les hydrocarbures, des substances chimiques ingérées, les xénobiotiques dont les perturbateurs endocriniens. La consommation d'aliments frais et riches en micronutriments est un facteur protecteur contre l'obésité infantile. 21 articles montraient le retentissement de l'environnement physique sur le mode de consommation alimentaire et la pratique de sport. La proximité avec les points de ventes de produits frais et les espaces de loisirs était protectrice vis-à-vis de l'obésité. La densité de fast-food et de points de ventes de produits moins sains dans le quartier de résidence et en périscolaire étaient des facteurs de risques d'obésité. Ces facteurs étaient souvent superposés aux conditions socio-économiques défavorables. 39 articles étudiaient le rôle de l'environnement cognitif. Le marketing alimentaire, quel que soit sa forme (par exemple les spots télévisuels, emballages alimentaires et advergames) influençaient, les envies, les goûts et la consommation des enfants. Une réglementation plus stricte et pérenne de celui-ci étaient un facteur protecteur. Le stress chronique et les troubles du sommeil sont également des facteurs de risque d'obésité pédiatrique. Les facteurs socio-anthropologiques nécessiterait un travail de recherche à eux seuls et nous avons abordé seulement 6 articles. Nous en avons qu'introduit le sujet.

Elle est responsable chez l'enfant de troubles de la croissance, d'une souffrance psycho-sociale et d'un risque de maintien de l'obésité à l'âge adulte. L'activité physique est efficace pour prévenir la prise de poids excessive. L'objectif de cette revue systématisée était de déterminer quels types d'interventions portant sur la pratique de l'activité physique chez les jeunes de 6 à 12 ans ont prouvé une efficacité en matière de prévention du surpoids et de l'obésité de l'enfant. Méthode - Ce travail a été réalisé suivant les recommandations du Cochrane Handbook of Systematic Review. Onze bases de données informatiques ont été interrogées permettant l'examen de 9345 titres et la sélection de 19 articles portant sur 15 études différentes, dont 10 à faible risque de biais. Résultats - Plusieurs caractéristiques semblent avoir eu un impact positif sur l'efficacité des interventions : l'encadrement par un professionnel de l'activité physique, le ludisme, la non compétitivité, le message éducatif associé, l'implication des parents, la durée d'activité physique de 60 minutes par jour, la fréquence quotidienne et pluriquotidienne, et l'horaire scolaire. Discussion - La principale limite de ce travail a été le faible nombre d'études sélectionnées. Néanmoins, cette revue a rassemblé des études comparables et à faible risque de biais. Il faudra attendre de nouvelles études pour confirmer ces caractéristiques. Des recommandations pourraient alors être écrites afin de guider la mise en place d'interventions efficaces dans la prévention du surpoids et de l'obésité de l'enfant.
