En 2001, des contrôles de l'air et de biocharge sur les dispositifs médicaux existaient au CHU de Nancy. En ce qui concerne les contrôles de l'eau et des surfaces, la perspective des travaux à réaliser au sein du service de stérilisation permet d'envisager leur future mise en place. C'est dans ce contexte, et avec la perspective proche de travaux au sein du service, que la stérilisation centrale de Brabois Adultes a procédé à une mise au point sur les référentiels existants et a proposé des exemples concrets de contrôle de l'environnement à développer en pratique

L'environnement hospitalier devenant plus concurrentiel et la recherche d'efficience devenant primordiale, la mise en place d'un suivi d'indicateur est pertinente. Le service de stérilisation hospitalière, en tant qu'environnement semi-industriel avec production de biens stériles et succession d'étapes de travail à la chaîne, semble parfaitement indiqué pour procéder à ce genre de suivi. Le service de stérilisation du CHU de Lille (Stérinord) procède à un suivi de nombreux indicateurs de performance, permettant d'analyser l'activité globale, l'activité technique et son organisation, ainsi que la productivité. Dans le but de pouvoir comparer ce suivi à ceux d'autres services de stérilisation, un questionnaire interrogeant l'analyse des indicateurs de performance a été diffusé. 37 services de stérilisation ont répondus à l'enquête. Les résultats montrent que tous procèdent à un suivi d'indicateurs de performance. Si les indicateurs d'activité globale et les indicateurs techniques et organisationnels sont généralement suivis, ce n'est pas le cas des indicateurs de productivité. Bien que l'analyse des résultats soit limitée par le nombre de réponses obtenues, il apparaît néanmoins que le suivi réalisé à Stérinord est robuste, avec quelques indicateurs potentiellement intéressants à rajouter. Cette enquête permet également d'apporter des informations sur le suivi des indicateurs de performance au sein des services de stérilisation, ses objectifs et la façon dont il est organisé. D'autres travaux seraient nécessaires pour illustrer et chiffrer plus concrètement l'apport des indicateurs de performance.

Les stérilisations hospitalières, grandes consommatrices de ressources et grandes productrices de déchets d'emballages et d'effluents peuvent constituer un pôle d'action pour le développement durable. A partir d'un questionnaire adressé aux entreprises de ce secteur d'activité, ce travail propose un répertoire des actions envisageables en stérilisation pour améliorer la performance de nos unités de retraitement. Les équipements lourds de stérilisation (laveurs désinfecteurs, autoclaves etc) et les améliorations proposées par les fabricants sont analysés sous cet angle. Les emballages de stérilisation, depuis leur production jusqu'à leur élimination finale font aussi l'objet d'une réflexion. Les produits de prétraitement « écologiques » sont quasi inexistants du marché. Nous exposons ici les problématiques liées à l'utilisation de ces produits et analysons dix produits à la recherche de celui présentant la moindre empreinte environnementale. Concernant les achats, ils représentent aujourd'hui un levier majeur du développement durable. Nous proposons dans ce chapitre un questionnaire remis à jour pour les appels d'offres sur les stérilisateurs, ainsi que divers autres outils dans le domaine des achats, commandes et approvisionnements. L'ensemble de ces points est récapitulé au travers de tableaux synthétiques avant d'apporter une conclusion à ce travail.

This document focuses on exposures via routes relevant to occupational settings principally related to the production of chlorine dioxide, but also contains environmental information. The health effects and environmental fate and effects of chlorine dioxide used in the treatment of drinking-water, together with those of halogenated organics produced by the interaction between the disinfectant and other materials present in the water are covered in a recent Environmental Health Criteria publication (EHC No. 216 2000) and are not dealt with in detail here. Chlorine dioxide is an irritant and it seems likely that health effects would be restricted to local responses. The few ecotoxicity data available show that chlorine dioxide can be highly toxic to aquatic organisms.
