En la mayoria de los paises, las enfermeras no solo constituyen el grupo mas importante de trabajadores de la salud, brindando atencion a todo nivel y sirviendo de enlace entre el individuo, la familia y el resto del sistema de salud, sino que ademas, varios de los sevicios necesarios para sentar las bases de la APS, tambien se consideran servicios de enfermeria. Por otra parte, es muy comun que la enfermera forme, supervise o dirija a quienes ejercen actividades relacionadas con enfermeria, ya se trate de trabajadores comunitarios de la salud o de matronas tradicionales. Por consiguiente, la enfermera representa un elemento esencial en la prestacion efectiva de APS a todo nivel del sistema de salud: desde la formacion, supervision y administracion del personal, hasta planificacion, organizacion, control y evaluacion de los servicios brindados. No obstante, para que la enfermeria pueda desarrollar todo su potencial, la organizacion y la prestacion de servicios en el ambito de la atencion de salud debe evolucionar segun los principios de la APS y habra que determinar el campo de accion del ejercicio de la profesion, tanto a nivel de funciones como clinico, para que contemple el amplio espectro de factores que inciden en la salud: culturales, sociales, del medio ambiente u otros. La calificacion de la enfermera debe abarcar, entre otras, la promocion y docencia en materia de salud; la movilizacion de comunidad; la formacion y supervision de auxiliares; la recopilacion de datos sobre epidemiologia y la planificacion, administracion y evaluacion de proyectos. Muchos problemas de salud tienen origen en el estilo de vida personal y colectivo y su solucion requiere un cambio de actitud, comportamiento y formas de vida. La creatividad, imaginacion e innovacion- basadas en teorias socio-psicologicas, psicologicas y de comportamiento y puestas a prueba mediante la investigacion en enfermeria- son necesarias para aumentar la eficacia con que la enfermera hace frente a los problemas. Los intereses creados que impiden el desarrollo de la APS estan profundamente arraigados en muchos paises; para encarar y resolver estos problemas, se necesita mucha voluntad politica, cooperacion intersectorial, inversion financiera y un gran compromiso del personal de APS. A efecto de obtener los cambios necesarios, la enfermera debe desempenar.

Book excerpt: Bajo los auspicios de la Organizacion Mundial de la Salud (OPS) y del Fondo de las Naciones Unidas para la Infancia (UNICEP), se celebro del 6 al 12 de septiembre de 1978 en Alma Ata, capital de la Republica Socialista de Kazakstan, la Conferencia Internacional sobre Atencion Primaria de Salud (APS), con la participacion de la mayoria de los paises del mundo, a fin de hacer un balance de los sistemas vigentes de prestacion de servicios de salud, los cuales deberan ser cambiados para lograr alcanzar la meta "Salud para todos en el ano 2000". Se llego a la conclusion de que el unico medio para alcanzar la meta establecida seria la introduccion de un enfoque que se le denomino "Atencion Primaria de Salud" cuyos principios fueron consubstancias en la "Declaracion de Alma Ata". Entre las 21 recopilaciones anexas a la Declaracion de Alma Ata, destaco especificamente la recomendacion N[vbar]6, que hace enfasis "en que los gobiernos fomenten la APS, de tal manera que aumente la capacidad de la poblacion de resolver sus problemas", para lo cual se requiere una estrecha y decidida colaboracion entre el personal de salud y la comunidad. Es necesario principalmente, reorientar el sistema existente para conseguir que todos los escalones del sistema de salud apoyen la APS, facilitando la referencia de enfermos y las consultas sobre problemas sanitarios, y proporcionando ayuda, supervision y orientacion ademas del apoyo logistico. Dentro de este contexto el sistema de enfermeria tiene acciones muy importantes que llevar a cabo, tanto en la estrategia de APS, como en los niveles de atencion de salud y en los tres niveles de la organizacion del Sistema Nacional de Salud (SNS); debido a que el personal de enfermeria asi como constituye el mayor numero dentro del equipo de salud, tambien conforma una solida organizacion en la estructura de prestacion de servicios, que va desde el medio rural disperso hasta establecimientos de salud de alta complejidad (Tercer Nivel). La Atencion Primaria de Salud representa implicaciones para enfermeria, las que pueden ser efectivas con su decidida participacion en la meta de "Salud para todos en el ano 2000", ya que ninguna profesion se adapta mejor a las diversas actividades de la Atencion Primaria, que la enfermeria. El presente trabajo tiene como finalidad proporcionar una informacion general sobre Atencion Primaria de Salud como estrategia para la cobertura de servicios de salud(AU).

Book excerpt: Confronted with worldwide evidence of substantial public health harm due to inadequate patient safety, the World Health Assembly (WHA) in 2002 adopted a resolution (WHA55.18) urging countries to strengthen the safety of health care and monitoring systems. The resolution also requested that WHO take a lead in setting global norms and standards and supporting country efforts in preparing patient safety policies and practices. In May 2004, the WHA approved the creation of an international alliance to improve patient safety globally; WHO Patient Safety was launched the following October. For the first time, heads of agencies, policy-makers and patient groups from around the world came together to advance attainment of the goal of "First, do no harm" and to reduce the adverse consequences of unsafe health care. The purpose of WHO Patient Safety is to facilitate patient safety policy and practice. It is concentrating its actions on focused safety campaigns called Global Patient Safety Challenges, coordinating Patients for Patient Safety, developing a standard taxonomy, designing tools for research policy and assessment, identifying solutions for patient safety, and developing reporting and learning initiatives aimed at producing 'best practice' guidelines. Together these efforts could save millions of lives by improving basic health care and halting the diversion of resources from other productive uses. The Global Patient Safety Challenge, brings together the expertise of specialists to improve the safety of care. The area chosen for the first Challenge in 2005-2006, was infection associated with health care. This campaign established simple, clear standards for hand hygiene, an educational campaign and WHO's first Guidelines on Hand Hygiene in Health Care. The problem area selected for the second Global Patient Safety Challenge, in 2007-2008, was the safety of surgical care. Preparation of these Guidelines for Safe Surgery followed the steps recommended by WHO. The groundwork for the project began in autumn 2006 and included an international consultation meeting held in January 2007 attended by experts from around the world. Following this meeting, expert working groups were created to systematically review the available scientific evidence, to write the guidelines document and to facilitate discussion among the working group members in order to formulate the recommendations. A steering group consisting of the Programme Lead, project team members and the chairs of the four working groups, signed off on the content and recommendations in the guidelines document. Nearly 100 international experts contributed to the document (see end). The guidelines were pilot tested in each of the six WHO regions--an essential part of the Challenge--to obtain local information on the resources required to comply with the recommendations and information on the feasibility, validity, reliability and cost-effectiveness of the interventions.
