Levenson, Chair Emotions are an important part of marital life. Prior research indicates that patients with behavioral variant frontotemporal dementia (bvFTD) experience significant impairments in socioemotional functioning, including emotional blunting, inappropriate behavior and decreased empathy (Neary et al., 1998). These changes in patients often strain marriages and can lead to a decline in marital satisfaction and increase the risk of depression in the healthy spouse (Ascher et al., 2010, Braun et al., 2010). However, few studies have explored the emotional dynamics of couples living with bvFTD in a systematic way (Braun et al., 2009). The aim of this study was to extend our understanding of the emotional dynamics of couples where one spouse has bvFTD as well as to understand the association between emotional behavior during the marital interaction and the caregiving spouse's relationship satisfaction and psychological well-being, looking specifically at depression. This study examined emotional behavior and emotional reciprocity of couples where one spouse is diagnosed with either bvFTD (N=30) or Alzheimer's disease (N=30) in the context of a 10-minute naturalistic marital interaction in comparison to healthy controls (N=29). Emotional behavior was coded using the Specific Affect Coding System (SPAFF), which consists of five positive emotional behavior codes, ten negative emotional behavior codes, and a neutral code. Emotional reciprocity between spouses was established using the Allison and Liker (1982) statistic. The study also examined the relationship between positive and negative emotional behavior as well as escalating and de-escalating patterns of emotional reciprocity and the caregiving spouse's self-reported marital satisfaction and levels of depression. The current findings reveal a loss of humor and reciprocity of positive emotion and an exacerbation in negative emotion (e.g., criticism, sadness) and conflict escalation in couples with bvFTD during a marital interaction. Furthermore, low positive emotion, high negative emotion and conflict escalation are associated with low marital satisfaction and high negative emotion is related to increased levels of depression in the spousal caregiver.

Book excerpt: Behavioral variant frontotemporal dementia (bvFTD) is a neurodegenerative disease characterized by progressive and dramatic changes in behavior and emotional responses. In particular, patients with bvFTD have deficits in empathy, including the ability to relate and respond to the emotions of others. These deficits are important precursors for prosocial behaviors - actions intended to benefit people other than oneself - and were thus hypothesized to be impaired in patients with bvFTD. Thus far, no known studies have utilized objective laboratory tests to examine patients' prosocial behaviors. The present study addressed these gaps by using objective laboratory measures (i.e., a donation paradigm) and a multi-method approach by studying the neural and emotional responses (i.e., autonomic and somatic peripheral physiology, facial expressions) associated with prosocial behavior across a sample of patients with neurodegenerative diseases and non-symptomatic aging caregivers. In the present study, 88 participants (17 with bvFTD, 12 with Alzheimer's disease, and 59 non-symptomatic caregivers) viewed a film of others in distress. Participants were then given the option to donate their own money as a measure of prosocial behavior. Results revealed that patients with bvFTD donated significantly less money than non-symptomatic caregivers, while patients with Alzheimer's disease and non-symptomatic caregivers donated comparable amounts. When examining emotional responses and regions of neural degeneration as potential mediators of the relationship between diagnosis and prosocial behavior, neither emotional responses nor neural regions were found to mediate the findings with one exception: lower ventral striatum volume was significantly associated with lower levels of prosocial behavior. This finding aligns with research that suggests that the ventral striatum plays a central role in the reward network of the brain, and how evolutionarily, feeling reward when engaging in prosocial behaviors likely conferred benefits in the context of interpersonal relationships. The study extends our understanding of a neurodegenerative disease that profoundly impacts the lives of patients and their families and contributes important new information to the literature on prosocial behavior.

Cellular viability depends on proper regulation of RNA metabolism and subsequent protein synthesis, which requires the interplay of many processes including transcription, pre--‐mRNA splicing, mRNA editing as well as mRNA stability, transport and translation. Dysfunction in any of these processes, often caused by mutations in the coding and non--‐ coding RNAs, can be very destructive to the cellular environment and consequently impair neural viability. The result of this RNA toxicity can lead to a toxic gain of function or a loss of function, depending on the nature of the mutation. For example, in repeat expansion disorders, such as the newly discovered hexanucleotide repeat expansion in theC9orf72 gene found in amyotrophic lateral sclerosis (ALS) and frontotemporal dementia (FTD), a toxic gain of function leads to the formation of RNA foci and the sequestration of RNA binding proteins (RBPs). This in return leads to a loss of function of those RBPs, which is hypothesized to play a significant part in the disease progression of ALS and FTD. Other toxicities arising from repeat expansions are the formation of RNA foci, bi--‐directional transcription and production of repeat associated non--‐ATG (RAN) translation products. This book will touch upon most of these disease mechanisms triggered by aberrant RNA metabolism and will therefore provide a broad perspective of the role of RNA processing and its dysfunction in a variety of neurodegenerative disorders, including ALS, FTD, Alzheimer’s disease, Huntington’s disease, spinal muscular atrophy, myotonic dystrophy and ataxias.
