Book excerpt: The aim of this study was to determine the perceived competence of Emergency Department (ED) registered nurses in disaster preparedness and response. A determination of the current level of nurses' disaster preparedness competence is critical in the development of educational curriculum. This study used a quantitative, cross-sectional design to explore current perceptions of ED nurses regarding disaster preparedness. The Emergency Preparedness Information Questionnaire was used which allows participants to rate their perceived competence. Results showed that nurses felt most familiar with triage and basic first aid (M=3.9, SD=0.916) and least familiar with detection (M=2.7, SD=1.08). No subcategory received a median score of 5, nor a mean greater than 3.9 indicating low to neutral emergency preparedness familiarity. Findings from this study suggest that although nurses feel competent in some dimensions of emergency preparedness, further curriculum development is necessary to facilitate a comprehensive understanding of disaster response.

Book excerpt: Emergency departments (ED) are the entryway into the healthcare system during a disaster event and patients often arrive in the first thirty minutes after an event has occurred. This creates unique challenges for ED nurses. The purpose of the Quality Improvement (QI) project was to measure the impact of a Department Emergency Operations Plan (DEOP) on the ED nurse’s perception of disaster preparedness. This study included a convenience sample of 126 full time and part time registered nurses working in the ED at a Level 1 Trauma hospital. Institutional Review Board (IRB) approval was obtained by both Arkansas Tech University and the participating hospital. Informed consent was obtained prior to taking the pre- and post-education questionnaires. The questionnaires were created by the primary investigator with expert validation by the Emergency Nurses Association’s (ENA) 2018 Emergency Management and Preparedness Committee (EMPC). Measures included knowledge level questions based on the identified priority needs of the leadership team. Education of the DEOP was provided over thirty minutes during the January, 2019 staff meetings. Data was collected via REDCap for analysis. Thirty participants completed both questionnaires. Results indicate that the DEOP QI project significantly increased the perception of the ED’s nurse’s knowledge level regarding disaster response for each of the questions utilized from the pre-education questionnaire as compared to the post-education questionnaire. Additionally, 85% of participants noted that the education increased their comfort level with the DEOP.

Despite the increasing frequency of MCIs many healthcare organizations and nursing programs do not adequately prepare nurses to be effective responders during an MCI. This evidence-based practice pilot project was purposed to increase new orienting emergency department nurses’ confidence and ability to effectively respond to an MCI. This project involved implementing an MCI educational program consisting of a disaster nursing framework to educate new orienting emergency department nurses on emergency preparedness and disaster response core competencies. The project leader utilized a quasi-experimental design to collect data pre- and post-intervention utilizing a modified Emergency Preparedness Information Questionnaire (EPIQ). The measurable outcomes for the project included improvement in awareness of competencies relate to MCI emergency preparedness, awareness of competencies related to disaster response, and level of self-reported confidence to respond to MCIs. This project revealed a statistically significant improvement in the awareness of emergency preparedness and disaster response core competencies among new orienting emergency department nurses as well as enhanced self-reported confidence to respond to an MCI. The results of this project have important implications for enhancing MCI education for nurses to develop prepared and effective responses to MCIs.

More than any other nursing speciality Accident and Emergency work reflects change in society because it is the casualties of social upheaval, imbalance and inequality that you face every day. Here, unfortunately, A and E staff are confronted on a regular basis with child abuse, family violence, the result of terrorist activities, social violence, motor car accidents, personal loss of loved ones, the frustrations and aggression of life. Sometimes, though, some pleasant and funny episodes do occur but all too infrequently to become the norm. My own experience of A and E nursing is very limited; in fact, it goes back to the days of the Department being linked with Orthopaedics and being called 'Casualty' . It could, though, appear that in this day and age the wheel is tuming full circle and moves have been made in the last reorganization to link the two new distinct specialities again. This proposal was met with lots of protest from A and E staff which appears to have been defused. The arguments stemmed mainly from the fact that orthopaedics and A and E are two quite difficult specialities of care and skills and as such must be seen as autonomous.
